I didn't get any foam in my kit? Bought the one sold by the hunting public. I can get foam from a hardware store too but your pre cut piece with adhesive would be the best.
Not so much, many have commented that it acts as a stabilizer weight. The mount itself only weighs a few ounces but the phone is the real added weight. If you feel it being too heavy you can remove some of the weight on the end of your stabilizer or try adding a rear stabilizer bar to level it out. That is what our crew does. Another great option is to just practice a lot with your phone on the setup so you are very familiar with it! Hope this helps.
You definitely don’t want them touching. When the phone is on the mount for the first time, I would put an arrow in and just do a test draw before you let an arrow fly to make sure there is clearance. I’ve never seen that problem occur where there is an issue!

What can be added to reduce the shock
Every kit comes with a foam dampening pad to help reduce recoil.

How much arrow/broadhead clearance do you need from the top of your phone to the arrow/breadhead?
Just enough for the blades/vanes to clear?
