They forgot the one where you can die in the safe on the bridge, jump on the lockers and like with the other falling glitch on the locker, just walk off at the right angle and you hang off the ladder instead of the lockers but this one you don't get up from and need help getting up. Then once you are helped up you get stuck and will teleport out. I'm not sure if I was the first to find that or what but it is rather amusing that you can die in the saferoom.
found that YEARS ago on the xbox version, its also one of those spots where you can automatically unhang and get up without help AND just.. never unhang for some reason and eventually die
