    There is definitely a lot of uncertainty with labrum tears and when you do or do not need surgery. I have a labrum tear on the lower rear of my shoulder but my pain is in the front of my shoulder. So I’m very hesitant to have surgery

      Hey man I see your comment is a month old but just wanted to reach out to speak. My situation is quite foggy due to uncertainty of doctors and what my diagnosis actually is. Never the less, one of my suspects they've told me is a slap lesion/tear. However the pain is in the front of my shoulder. Not at the top either. Further down the arm on the front. Just wanted to ask how your situation is going

    "It's the function of the muscles "..man that was awesome..The best advice yet as just continuing to work on movement instead of all these crazy agility tests that may or may not tell you exactly where the problem lies...That's where I need to start then just getting mobility back.,I look back over all the shoulder injuries I've had and that's basically all I ever done to get well again...

    So if I cannot pullmy shoulders back and down ..per Chiro ,.....due to pain how am I supposed to fix this labrum. My pt was worthless ..wall washescoh boy...my shoulder was not frozen...I didn’t need wall washes..

    Please explicitly differentiate between degenerative related tears and acute injury tears. They can present with drastically different symptoms and I would hate for people with acute injuries put off surgeries due to videos like this.

      No kidding right. My dog pulled my arm super hard and it fractured my humerus in two places, tore my supraspinatus, and apparently my interior labrum. Happened 7 months ago and now I have to have surgery because I’m in pain all day (esp at night).

    The uncertainity that a patient goes through is not fun. My MRI results, based on the radiologist and the doctor readings, say that I have less than 24% labral tear yet my shoulder keeps nagging me on daily basis.

    Do all labral tears cause pain? No. However, for the ones that do, surgery is the best option. I literally put off labral surgery for 10 years because of shit like this, and finally pulled the trigger after further instability was causing rotator cuff / biceps issues and even some early onset arthritis.

      Yeah, as someone who is unsure of whether or not I would get the tenodesis my Ortho recommended, this video actually reassured me it's the best move. This guy just inserts doubt without adding anything of value to the conversation. Might as well get your body repaired and see what happens next. It's all a crap shoot anyways, and why waste years wondering when you can find out in months?

      ReSa Chase The bad hip is what’s causing the shoulder pain on that side. I have same issue. I have to do some very unorthodox stretching to keep my back straight and free of pain. And I have to walk a lot.

    I have had the same thoughts. Especially when the labrum is described as a 'deepening' of the glenoid, to provide more stability (so its passive tissue then?) If we needed deeper shoulder sockets wouldn't we have evolved them?

    I have a shoulder labral tear and a cyst on spinoglenoid notch and measures approximately 1.5 X 1.4 x 1.3 Cm. I was given a cortisone shot for pain and it has relived 95% of the pain, but the cyst is definitely pressing up on my nerve cashing severe pain when i didn't have cortisone. Doc suggested shoulder surgery but i cant do that RIGHT now. seeking to try Ultrasound-guided cyst aspiration... then we can talk surgery but i'm cannot do surgery right now.

    3 months ago I hurt my shoulder playing tennis. I felt a pop followed by alot of pain. Initially I thought it was dislocated but xray was negative. Ortho think is the labrum but MRI didn't show any abnormalities.. I am very frustrated at this point. After 3 months, I still have pain raising my arm above my head and constant pops which are painful

    There are levels to this. If there was a traumatic injury and not a degenerative one, that you could have a pain from labrum tear.

      I fell on my shoulder back in 1999. I’ve had pain on and off. It has been getting worse the past 3 years. Finally had a ct arthrogram and it showed a superior labral tear

    If I move my shoulder suddenly I get " hit my funny bone" shoiting feeling/ pain . Sometimes my arm is numb and tingles other times. Sucks. Does this sound like a labrel tear?

    Hello, thank you for your video. I am a Resident physician and I am 27. I was working out at the gym one year ago and suddenly I started to feel a click on my right shoulder. I tried physicial therapy and I felt it helped me but I still feel that click on my shoulder, I also feel some pain when sleeping at the right side or when I move my shoulder in some positions.
    My Orthopaedic Surgeon diagnosed me with a probable SLAP lesion but I did´t want to have an RMI to confirm it. I feel it is getting worse over time. As a doctor I have being thinking about the possibility to have surgery but I don´t want to, my job and my studies take all my time and if have surgery I will have to stop my activities at least for a couple of months. Do you think your program will really help me?

      @Walter Insuasti I would visit a local regenexx clinic if I where you and have them eval your shoulder under their high resolution ultrasound, under load. Then you'll be able to see for yourself exactly where the instability is and whether it is correlated with the SLAP lesion. PRP therapy is way less invasive then a surgical labrum repair. The most pain you'll have is the day of the procedure and you'll be able to work the next day. Hell I drive 3 hours after getting my shoulder done because of where the clinic is and it's fine.

    So do you think getting surgery for a slap tear causes more pain in the long run?
    I had my shoulder operated on for this in 2014. I've been focusing on increasing the integrity of the joint with strengthening exercises and ROM.
    Would love to hear your thoughts on this

    This is my MRI report please guide me i have lot of pain 1) Mild supraspinatus and infraspinatus insertional tendinosis with subtle bursal surface fraying. Mild to moderate subacromial bursal fluid and edema. The subacromial bursitis is disproportionate to tendon changes and correlation with inflammatory markers/recent subacromial bursal injection is recommend.
    2) Nearfullthicknessbicepstendontearfromitsglenoidattachmentretractedinferiorto the bicipital groove by approximately 4.5 cm.
    3) Moderate rotator interval scarring with mild inferior capsular thickening and edema mostly suggests adhesive capsulitis and needs clinical confirmation.
    As compared to previous MRI dated 18-01-2022, bursitis is a recent finding. The biceps tear is also an interval development. The changes of capsulitis are grossly unchanged.

    My direct experience with such injuries is that they heal naturally with or without muscle training, it just takes a long time, nearly a year to even notice any improvement and an additional half year to year for it to go the last 10-20% and feel 100% normal again. The pain itself often is all the motivation one needs to keep stretching the area, as one naturally likes to test the area to desperately look for signs of improvement. Direct muscle training while injured carries the risk of reinjury, an issue with which I also have direct experience. General cardio has been best for me in those cases as it seems to pump up circulation in the shoulder without placing the injury under directed mechanical stress. The overall muscle pump from cardio alone seems to isolate the injury more and by itself will produce some transient pain relief. (There could also be some help from endorphin release too).

      This is utterly false information. Any decently sized laberal or rotator cuff tear will never heal on its own. Your body will adapt and you may get scar tissue and periods where it is tame, but it won't ever heal without surgery

      ​ @Alexxf35 It would be "utterly false" if it didn't heal, yet you yourself already admit adaptation and scar tissue forms. That is healing, by definition, of a sort. Therefore your own statement is "utterly false" by your own statement. My last rotator cuff injury was so severe the humerus head was literally rattling around in the socket, and absolutely no muscular effort could normally lift the arm. I could only lift the arm if my good arm lifted it up. That is pretty severe. It may be true it may not have been a 100% tear, but several key tendons attaching at the head were severely damaged and beyond any function initially. Yet it healed to 100 % function. I certainly still feel scar tissue in movement years later, but it did heal by itself. The scar tissue really is only felt on cold days, and any sense of it disappears during exercise which clearly causes an inflation of the distances between bony surfaces, and minimizes the impact of the bulk of any scar tissue. Function though, returned to 100 of prior range of motion. The last 5% of range though normally will impinge on the scar tissue and be felt.

      Surgical intervention was required. My point is saying injuries like this heal on their own is incredibly misleading.

Most rotator cuff injuries can heal by themselves, the main issue is whether the patient can coddle the area for the length of time necessary, and that is the main issue. Patients tend to be impatient and will reinjure the area over and over, potentially making the area worse.

    I had some suspected labral tears for years. I’m gonna get an arthrogram to confirm this. I’ve had it for about 5 years now. Here’s what helped keep my shoulder strong.
    1. Lift weights extremely slowly and carefully. I don’t even go to a gym. Everything I do is at home
    2. Use forced negative pull ups to strengthen your back without hurting the joint. Weak back muscles contribute to shoulder instability. Use a resistance band attached to the pull up bar to take more strain off your shoulder. Lower yourself very slowly. Your back and bicep will feel sore which is a good thing (Don’t do this heavy if you have a slap tear). Use bands and go slow.
    3. Plank position 5 sets of 1 minute a day
    4. Incline pushups fixed range of motion
    5. Rotator cuff exercises using a band.
    Can’t say this is a cure but I’m better now than a few years ago. The MRI will reveal all

    I ve been having pain from last 15 months while bowling. I had latest MRI done showing Superior Labral tera extending upto equator. Doctors are suggesting surgery. Is tear actual reason of pain? Is the part torn responsible for providing power while throwing? What should I do?

      get the surgery. I had avoided surgery for 5 yrs with my SLAP tear but pain never got better. Finally had the surgery, and no pain now, stronger than ever.

    All the pt in the world wont help me. Ive done 3 yrs of it and shots. I have ehlers danlos and it just constantly. Dislocates. Im hoping to avoid a replacement but was told. I have greater then 50 percent cartiladge loss and. Osteoarthritis.😢. And a bone spur in the humeral. Head. Confirmed via ct arthrogram

    I had a fall last November and my should was injured after MRI following are the analysis : I have a Glenoid Labrum torn from 1 to 4 o'clock position. Hyperintense singal is also noted i superior Labrum-SLA2-tear The long head of biceps tendon is normal. Rest of Glenoid labrum and articular cartialage is intact. Cortical irregularity is noted in naterior aspect of glenoid without significant flattering. The acromion is of Type II. Mild acromioclavicular arthrosis is seen. There is mild glenohumeral effusion. Visualised bones appear normal.
    I have taken physiotherapy for 3 months and some of the pain is some positions have gone. but now the remaining pain still exits. My Physio have told me that I cannot push anything above my shoulder by I can puul. I can do anything below shoulder level.
    I want to know if I join GYM and exercise, which exercises are not to be done and which are to be done on light weights until other muscles get strong and support the shoulder.
    OR
    Surgery is required?

    My pain is coming from my labrum. Maybe I'm the one percent but massage/trigger point, rom, PT does not make it better. I wish I could have surgery but they told me it wouldn't get rid of the pain. I'm about to get another Doctors opinion. I'm open to doing surgery even if it doesn't fix the problem.

    Ive been having severe pain in my shoulder for a month. I had an arthogram 2 months ago and they discovered i have a superior posterior labral tear over 180 degrees, do you think surgery is my only option? Ive been in therapy but I can barely do the moments they’re giving me.

      Just curious how you are doing 7 mos later? Did you end up having surgery? I subluxed my shoulder about 3.5 months ago in softball. I can tell it will be a long road, slowly able to do more and improving. But wondering if will ever be 100% . At the 1 month point like you were, I could barely do anything with it.. I hope you are doing better.

    Your videos tend to have a anti-surgery bias. You even have a problem with PRP. You can't solve every problem with PT. OK the labrum isn't entirely intact but the study says nothing about the elasticity of the tendons, ligaments, overall strength, and general activity level of the participants so it's a big "whatever". Some bodies can compensate for "gasket tears" and some can't.

      He's financially motivated.. Selling snake-oil. I've had physio for a lot of things, it rarely worked, physiotherapy is for prehab and rehab after surgery. Surgery was Always a complete succes and ended up taking less time than pt.

      Get the surgery man I’m getting mine next week . 6 weeks in a sling and many months of pt . Recovery is anywhere between 3-6 months . You’ll have most of your strength back in a year from what I’ve seen and have been told

    I am sure I have re-injured my five month old SLAP repair. I get clunking and pain with most movements and an indentation at the top of my shoulder when my arm hangs by my side.

      Just got this injured yesterday from not resting enough and pushing myself with the bench and floor press after..
      Now both of my shoulders have it, right from mma and left from benching, not to mention I have rotator cuff problems in both lol Now its gonna be fun fixing this...

    PLEASE HELP ME! I just had an mri and the doctor said it’s a labrum tear he said my labrum had been worn down. No specific injury that I can remember caused this. I’m 16 and don’t know what to do should I get surgery like he wants me to? I played football with this injury and my shoulder has never dislocated I’m not returning to ball but I do like working out. Is surgery necessary?

      BostonGamer just over use of the shoulder, if you haven’t had a dislocation I wouldn’t get surgery. Work on strengthening your shoulder yourself and know your limits

      BostonGamer no surgery is not necessary you just need to gain functional balanced strength and mobility through the joint. Surgery is actually a bad idea there’s no guarantee it works and reinjury can easily occur if you don’t strengthen, stabilize, and mobilize ... So that’s all you need to do in the first place.

    I have a labral tear that ONLY hurts when I throw a ball. Nothing else I do hurts it. Lifting weights, golf, hockey, nothing. JUST throwing.

      Learn how to throw with your opposite hand and go easy on your injured shoulder. You don’t want to reach the point where you need surgery

    Yeah it may not be the direct cause of pain . That being said the lack of stability definitely causes certain muscles having to work harder . It’s more less the root cause to some ppl’s pain

    I wish I had this information when I fronted a panel of orthopaedic surgeons for my insurance claim. The outcome was due to the legislation whether i have didn't constitute an injury.. crazy.
    I came off my motorbike and fell on my shoulder, I've been in pain for nearly a year... Arseholes.
    I might take it the the full industrial relations tribunal

    Been having issues in both my shoulders for months now. Burning and aching pains that suck unless I constantly stretch/massage. I’m about 10 % better this month then I have been lately. I’m a Thai boxer so I’m used to getting roughed up so I can take the pain. I’ve returned to boxing and the pain has not improved or gotten worse. I’m gonna work with my PT and I’m going to see orthopedic, praying I don’t need to get surgery. I’m over that shitty mental hump at this point

      I'm on the same situation as you, 1 question tho does it hurt you more in the morning as soon as you wake up?

      @@likoltarypv4478 yeah it does. But once I “warm them up” they return back to their normal state. Honestly what has helped me the most is shoulder mobility exercise and trigger point therapy/release. I do these everyday now and I am in better shape than I was a couple months ago. No where near 100% but I’m managing.

      @@SuperVince105 Same they pinch like crazy in the morning and it's so painful but after doing more research I think i might have bicep tendonitis. It's painful in the morning and when I warm up it's gone. It's still hurt sometime during the day just not as bad as when I wake up. Either way ima go see an orthopedic tomorrow and see what's going on. Hope you get better !

      @@likoltarypv4478 you too man! Just keep pushing don’t ever give into it. I’m back boxing again I’m just laying off upper body exercise. And yeah ortho said I had bicep tendinitis and thoracic outlet compression. They will give you PT exercises likely

    A tear in the labrum will cause pain. This is the consensus among physical therapists. Get your facts straight dude. This video is deceiving.

    Don’t listen to this guy. Go to a qualified sports ortho who works with pro athletes. If you have a tear, depending on how bad it is you should always give PT a shot and probably get a second opinion. The tear will not heal on its own and if symptoms persist surgery might be best option depending on the level of function you want/need (surgery is never 100% guaranteed to have successful outcomes).
    Do people have tears and experience no symptoms, sure. Can you experience a minor tear and rehab it back to full function, yes. I did multiple rounds of PT for over a year, didn’t work, had the surgery and now I’m 100%. If you’re interested in good content, research prehab guys and read their articles about the labrum and listen to their podcasts with qualified doctors.
