How about measuring ketones?
This is a great idea, my mood Changed so much since I reduced sugar/ simple carbs. Like feeling less depressed and getting rid of negative thoughts about myself, this thing called self critic.
However homa-ir is not always accurate when it comes to people with accurate BMI, especially people on the thinner side.
I had insulin resistance symptoms, when I take mino-inositol it helps with my symptoms. It makes your body more sensitive to insulin. Yet, my homa-ir was in healthy range, when I checked it.
Hi Dr, I have been running a keto diet in insulin resistance patients for 8 years. That is some 12,000 patients now.
I study insulin resistance and have noticed that about 40% of patients have homa ir below 1 but on a 3 point insulin Kraft test comes out massive insulin resistance. In my opinion, Homa IR is not an effective test for assessing insulin resistance
For the past year, I have observed that about 60% of patients with isnulino resistance are taking antidepressants.

Would love to ask, have you had any female patients with PMDD and seen an improvement in their depression/suicidal ideation/anger/irritability when putting them on a keto diet? I assume a large amount of women with PMDD are insulin resistant.
I am so glad this comment exist. There are studies that show that people with right BMI, may have not accurate results with Homa-ir.
I am one of these people, I got symptoms of insulin resistance, when I take mino-inositol it helps with symptoms, yet my homa-ir was in healthy range. Always look at symptoms first.
What is your opinion on the Triglyceride / HDL-C ratio of 3.5 or greater being an indicator of insulin resistance?

Hello DrS. I'm glad you are working on mental health and I'm sure you will do a lot of good for a lot of people. Then I hope you will move on to gerontology. As a 60F I'm fighting sarcopenia and other issues of aging. I appreciate Dr Gabrielle Lyon's point of view of the contribution of muscle to metabolic health and healthy aging, but that's just one factor. Would appreciate your attention given to healthy aging, lifespan=healthspan! :)

If it's waist circumference, then this should not be measured at the hips (that's hip circumference). The waist is further up, around the area of the belly button. This video here explains exactly how that is measured:

Also, it should be waist divided by height, not the other way around.

Much better. Rather than just a fasting measure you get a post-prandial response which we can then measure the peak of insulin and glucose at 30, 60 minutes and two hour potentially three hour mark. Much greater information gained

I requested a HOMA-IR from my doctor. She asked me what the test was for. When I explained she declared she never orders tests that she can’t read. And told me to go to my naturopath.
Good summative overview. If you have a W:H ratio of 0.47 and optimum lipid profile, is it still possible to have high visceral fat? And if so, how can you find out?
Thanks for your question. In that case, it is still possible to have elevated visceral fat, although less likely. One of the bests tests to find out is a Body Composition DEXA Scan. Hope that helps!
You forgot one other test equivalent to HOMA-IR, the Triglyceride / HDL-C ratio. A value of 3.5 or greater is associated with insulin resistance.
I think you divide your waist by your height. Dividing your height by your waist circumference will always be greater than .50.

Thank you for your great work. One question I couldn't find an answer for, also elsewhere is, if insulin resistance can occur only on the brain level and not showing up in the HOMA-IR. If so, what marker could be used as a surrogate to get a first indication of that brain tissue insulin resistance before having to do more invasive tests? Domingo
Thanks a great question, and one we don't have a great answer for yet. But researchers are looking at this. It could be blood-brain-barrier imaging like Dr. Calkin is doing, or other similar tests. Let's see where the science goes on this!

TG:HDL Ratio less than 1 is another way to tell apparently
TyG index is easier than HOMA-IR with better sensitivity and specificity.
Thank you for your comment. That's a really good point about using the triglycerides/glucose index. It can be a very helpful marker as well, especially since they are standard blood tests. I appreciate you mentioning it.
@@metabolicmind I love it, and what is very fascinating is almost everyone is insulin resistance and/or has a high propensity for NAFLD. No wonder everyone is so sick!
I love the idea of a lab test to measure the existence and severity of insulin resistance but the doctors where I live don't want to go through the cost. They just rely on visual signs like abdominal fat and skin discoloration at the skin folds. It's very frustrating.

Ask your doctor for that and the answer will either be no or wtf is that. My endocrinologist won't prescribe, he says it's not necessary.

I told my Dr. about HOMA-IR, and she said, "what..?". And refused to get me any tests other than the basic panels SHE wanted.

Thank you! But clarification. I just divided my height by waist and got a score of 1.8. To get a score of 0.5 I'd need a waist of 130 inches which would make me Insulin Resistant for sure. Don't you mean to divide waist by height?
You are correct! It is the waist to height ratio, not the other way around!
What if you dont have metabolic dysfunction in the body but have bipolar? Could it be possible that insulin resistance can be just within the brain and not the whole body? Is only someone who is insulin resistant able to benefit from the ketogenic diet for mental health? Thank you for all you do by the way🙏
Thanks for your question. Ketosis can benefit brain function by improving insulin resistance and providing ketones as a fuel source. So, in theory, ketosis could still benefit people even if they don't have insulin resistance. But as you alluded to, it's possible that brain insulin resistance could exist even if it isn't detected systemically. We need better ways to detect brain insulin resistance to know if that is the case. But this is a long-winded way of saying we have more to learn from a mechanistic standpoint, but clinical practice and theory suggest that someone could benefit even without detectable insulin resistance.

What is your opinion of using the Triglycerides/Glucose Index to measure insulin resistance and NAFLD? I read an article that said that the TyG index is a better indication of insulin resistance than HOMA-IR.
I think it is a very good marker. it is hard to say for sure which is "better," but I think the more you look at, the better idea you have.
So my height is 65 inches and my waist is 30 inches. That gives me a reading of 2.2. I'm 120 lbs and you want me to lose weight???
Sorry, there was a spot where I misspoke. It is waist to height ratio, so 30/65=0.46 so based on that calculation, you're less likely to have insulin resistance.

Why not just test your fasting insulin? Can you have low fasting insulin and high fasting glucose? If not, then a low fasting insulin is all that is needed, no need for homa ir.
Why ? Because Most Drs will not order fasting insulin unless you are diabetic and taking insulin ( Standard of Care Insurrance rules ).
But you can pay for it yourself at Quest labs out of pocket.
Dr. Scher, I appreciate all the work and dedication to helping people over come mental illness, nutritionally. And this is a good video. However, because most people are going to do the waist divided by the height test you need to correct your instructions on how to do that. I have been on a low carb diet for a number of months and I have been no to low carb high fat diet for about 2 1/2 months. I followed how to measure the waist on this clip.

And then I divided my waist measurement of 35 inches where I took the tape measure all the way around my waist as she instructs, by my height of 5 foot 4” turned into inches. So 35 Divided by 64, which gave me a result of 0.546875
I hope I did that right and if so, then I have a ways to go to get below 0.5, but not nearly as much as I would have three or four months ago.
