Root cause analysis (RCA) is a structured method used to examine serious mistakes. Originally for industrial accidents, it's now common in healthcare. RCA finds problems causing errors, not just individual mistakes. It looks at both active errors (mistakes when humans interact with complex systems) and hidden problems in healthcare systems. RCA follows a specific process: collecting data, reconstructing events, and talking to those involved. A team then analyzes how and why the mistake happened to prevent future harm by fixing underlying problems.
