Peter Attia, a "longevity expert" who doesn't like to talk about one of the leading contributors to longevity, diet. It is because he doesn't have a leg to stand on, he hates talking about it because he doesn't have a good response to the overwhelming data on pro vegetarian diets and health. The fact that he is happy to use far weaker epidemiology to support his positions on exercise just cements the hypocrisy.
He used to recommend a keto diet for longevity before back-peddling so it is safe to say his understanding of nutrition is shaky at best.

I tell you a story how Attia became the reason why I (finnaly) got my critical thinking on and escaped low carb / keto bubble after 5 years (well, with LDL of 140, insulin resistance and some other usual outcomes of such diet). I was following him in the early years of his popularity, where he was semi-decent, pro-fasting. But with the years he was becoming more extreeme, stated to drop such statements that it's ok to take statins in order to be able to eat animal protein. Or that we need to max out animal protein, so he is himself snackin on deer jerkeys throughout the day (and btw, he is investor in deer jerkey company). And the last drop for me was when he said that is person is eating plant based for the health reasons, we need to re-educate such person back to animal products. So I though such un-intelligent person cannot be reliable and started to learn more about WFPB. And here I am after 6 months, 22 kg lighter, body fat from 22% to 12%, BMI 19, feel better than even and - even if I was afraid - no loss in muscle, strnght or endurance.

Hi dear @ZmogusJaponija, I was very happy to hear that you are feeling better and that you have found your inner compass and critical thinking. No matter what lifestyle one follows the above is truly critical to have. As an ethical not health vegan just wanted to alert you, PLS make sure you have all your self-care stuff in line for the long run. WFPB is an excellent start and if you can afford do take some blood tests, PLS do to check yourself time to time. And if you have no access to "dirty but bio" veggies (for natural B12), and enough sun, etc you need to take supplements. According to research in our modern lifestyle in dense cities and with our santitized food system we simply cannot get certain vitamins/ minerals that easy (this goes to any diet though). I hope your journey will be long and brings everything you hope for. (Hugs, Anna, healthy vegan for over a decade 😅)

I have known about Dr Attia for years, and I think he does promote critical thinking, but has also come to conclusions that I don't agree with, and for a while he was keto. He went through a period where he seems to have questioned the lipid hypothessis, which at the time was a little more reasonable, but it is even more clear now about the role of lipids in heart disease. I have heard pro carnivores talk about him positively but they don't realize that Dr Attia is taking cholesterol medication to keep his own lipids under control. On nutrition he doesn't seem to want to make any of the diet tribes mad. So he is mixed bag, but might be a bridge to move some out of the Keto/Carnivore lifestyle

From Wikipedia: From 2001 to 2006, Attia began a residency in general surgery at the Johns Hopkins Hospital in Baltimore, Maryland, but never completed his residency, nor completed a fellowship or became board certified. During this time, he also undertook research at the NIH in Bethesda, Maryland, focusing on cancer immunotherapy for melanoma. After dropping out of his residency program, Attia joined the consulting firm McKinsey & Company in the Palo Alto office as a member of the Corporate Risk Practice and Healthcare Practice. In 2014 he founded a private clinic dedicated to longevity medicine.
An interesting resume but not exactly confidence inspiring. It looks to me like he jumped from medical school straight to lifestyle guru. Yes, I know ad hominems do not disprove Attia's statements, but Mike can do that much better than I can.

I’m going to defend him on this point a bit. I’m a doctor and residency is very challenging. That goes double for surgery at Hopkins. Burn out is not uncommon. A lot of doctors switch specialties or even quit medicine altogether. It’s not necessarily that they couldn’t hack it, they just lost their desire to further pursue medicine.
That being said people who use their medical education to push bullshit to people are scumbags.

Attia is really inconsistent. He claims that high V02max reduces all-cause mortality, so you should raise your V02max. More beneficial than quitting smoking. All based on observational studies. But then he dismisses observational studies showing the association between red meat and saturated fat and cardiovascular disease. Note that he flogs lots of products, including red meat jerky.

he claims what he claims about VO2max and longevity because that data is quite different; the HR is huuge compared to eating red meat, meaning the the difference between low and high vo2max is (substantially)bigger than between low and high red meat consumption. Plus virtually every study on vo2max shows this, while with red meat that is not the case.
Also you are missrepresenting his claims on smoking. What he says come from a graph in Mandsager et al where a HR's are shown for low vs high vo2max and for not smoking and smoking and it demonstrates that the HR for low/high vo2max is bigger than the HR for no smoking vs smoking. This is a completely different statement

You might want to consider looking at the "Hierarchies of Evidence Applied to Lifestyle Medicine (HEALM) presented by Dr. David Katz. www.ncbi.nlm.nih.gov/pmc/articles/PMC6701153/. He recognizes the limitations of the current model for the hierarchy of evidence when it comes to nutrition and other lifestyle habits. The current model (the pyramid) was really designed for drug studies, so the double-blinded placebo-controlled trials are considered the highest quality of evidence.... however, nutrition isn't quite that neat. Thus, prospective, population studies are useful when looking at long-term eating practices and morbidity and mortality outcomes, which you don't get from RCTs. So they are useful for looking at long-term exposures in lots of people. RCTs thus have their limitations and are used to answer different questions. Hence, I like his proposal to examine how we evaluate study quality.

Absolute vs. relative risk is probably not sufficiently appreciated by many. A risky treatment might need to be evaluated with an appreciation of absolute risk of what is being treated, but relative risk may be more important for apparently safe dietary options like vegan foods. That is, if you have a risky treatment, perhaps an mRNA therapy, for an illness which has a low absolute risk of death, then you might do better to consider the absolute risk of that illness when you are calculating you risk-benefit. But, if you're considering kale therapy, consideration of relative risk reduction may be sufficient without needing to evaluate the absolute risk of getting a potential illness, since kale therapy itself carries no known risk. With a truly safe therapy, you have nothing to lose, so relative risk reduction should be sufficient, even if the relative statistical benefit is small.

Once again, well done on the video. Thanks, for the good work. It is interesting that those who actually do research in the area of longevity consider that high intakes of animal protein (and even very high intakes of plant protein) promote aging and cancer (likely via increased levels of IGF-1 (V. Longo). His argument that you need 180 grams of protein to maintain muscle mass is inconsistent with the research on maintaining muscle mass in older adults. The data are inconsistent, with some studies showing that protein intakes above the RDA are associated with improved outcomes and others showing no benefit from the extra protein. The study of protein needs is nuanced and difficult to do. However, plant-based diets appear to be advantageous when it comes to preventing sarcopenia. A 2022 review study observed that consuming plant-based protein was associated with a lower risk of sarcopenia than consuming a Western, meat-heavy diet. They noted that older adults on a high-fat/high-energy diet are at a greater risk of sarcopenia even when protein intakes exceed the RDA (Coelho-Junior et. al. 2022). Moreover, a recent 2022 paper from the Nurses’ Health Study, which followed 85,871 nurses over 22 years, found that animal protein intake was associated with a higher risk of frailty and higher plant protein intake with a lower risk of frailty after adjustment for other lifestyle factors. Animal protein intake was also significantly associated with an increased risk of having five or more illnesses (Struijk, EA et. al.). The fact that he is unaware of this research and the nuances suggests he hasn't done his homework.
And to continue my rant.... He is also a big advocate for having a high VO2max as that is associated with improved outcomes (notice associated, not causal). Yet these studies are also rife with problems of confounding variables. Those with a high VO2max tend to not be overweight (as it is a weight-based measure), they likely don't smoke, they exercise, they probably don't have diabetes, and there is a genetic component to having a high VO2max. And, do those who exercise enjoy a higher socioeconomic status? (they probably aren't working two jobs). Yet, this is what he touts as being the best measure of longevity. I think he is a smart guy and says some good things, but if you are going to throw out (or put in the garbage, his words) epidemiological studies of nutrition, then throw out most of the studies on exercise and smoking.

  • @Carlosconga

1.3% increase to 1.7% is 40% increase in relative risk, which is not insignificant but could well be within the absolute risk margin of error. Not a big enough difference to matter.
For exercise, you chose a review that doesn’t specify which adaptations from exercise are more life prolonging, which are strength and a high Vo2max. When those are looked at directly, the life prolonging effect are more pronounced.

Healthy user bias is a serious issue that shouldn't just be dismissed automatically. As a scientist, it's blatantly obvious that "controlling" for those variables after the fact is a very weak control (both statistically and in terms of biological relevance) and you can only control so many things. These variables also aren't linear. Healthy user bias is impossible to avoid, and even with strong correlations it's difficult to have strong confidence in causality from even the best designed epidemiological studies and meta analyses.
That being said, it's all we have when it comes to nutrition research since properly designed RCTs are extremely impractical, and one has to make a choice based on the available data, flawed as it is. In the case of red meat, the rest of the data (including mechanistic data, while useless on its own) does increase confidence in the epidemiological data.

A better question for Dr. Attia that he heretofore has answered in the negative is "Does the consumption of red meat result in animal suffering?" That answer is, of course, nonsense. He is a game hunter, and he would have one believe that dispatching large game from afar is humane since the animal doesn't know what hit it him/her. Well, Dr. Attia, what about the emotional loss suffered by the deceased animal's family members? He seemingly can't make this obvious connection. Perhaps he believes that the animals he dispatches via bullet aren't sentient. Thus, he engages in a risible degree of speciesism. Not to mention that the industrial meat industry is destroying the planet. Come on, Dr. Attia, stop engaging in confirmation bias long enough to contemplate what you are recommending.

I have tried for years to get my husband to transition his unhealthy meat laden meals. I have been vegan for years and recently totally raw fruits and vegetables. Wed morning, my beautiful husband walked into Walgreens by our home, told the cashier he wasn't feeling well. He collapsed and had no pulse. After paramedics tried for over an hour to revive him, he was declared deceased. I can tell you that steak, hamburgers, chicken, fried foods, Taco Bell, dairy, etc. will clog your arteries. Now I have to bury my husband in 2 days. I am overwhelmed with grief right now.

So saddening about your loss. I’ve been Whole food plant-based for 3 years and have never felt or looked better but my wife it’s much like your husband. I’ve tried to talk to her and reason with her but she says she’s going to enjoy herself until she’s 50, then she’ll change. People are always putting things off till tomorrow; the problem is, tomorrow never comes.

@@paulcohen6727 We had been trying to change the diet however, it was too little too late. His health has been a danger for years. I miss him so much but I couldn't compete with his gut microbiome and those foods had hijacked his brain.

@@paulcohen6727 I was joking with him weeks ago. I told him if he didn't stop eating the way he was eating, I was taking a life insurance policy on him. We laughed. He thought he would live forever. Now he will never see his children marry or have grandchildren.
