“Pharmacists and doctors do it better together”: on deprescribing

Elizabeth Su, Senior Project Officer at the Medicines Optimisation Service at Austin Health in Melbourne, presented at #MM22024AdPha about the development of an effective partnered workflow between pharmacists and doctors to identify, action and communicate deprescribing recommendations. It has led to more and better deprescribing of potentially inappropriate or harmfull medications, like sedative and anticholinergic medications which are associated with an increased risk of falls and decreased function and cognition in older patients.
