    KEYNOTE-564 study showed that adjuvant pembrolizumab improved disease-free survival (DFS) compared with placebo after nephrectomy in patients with ccRCC who are at increased risk of recurrence.
    Now, the updated report at the 2024 American Society of Clinical Oncology Genitourinary Cancers Symposium showed that it improved overall survival in addition to DFS.
    OS benefit was observed across key subgroups, including patients with M0 disease (HR, 0.63; 95% CI, 0.44-0.90) or M1 with no evidence of disease (HR, 0.51; 95% CI, 0.15-1.75), with a PD-L1 combined positive score (CPS)

    At 33:00, I said adjuvant therapy is not indicated because we do not have date proving the benefit of adjuvant therapy after metastasectomy.
    However, as I described in the section of adjuvant immunotherapy after nephrectomy, adjuvant immunotherapy x 1 year improved survival (KETNOTE 564: NEJM 2021 Aug. 385:683) in intermediate and poor risk RCC patients. In this trial, they included M1 NED which means completely resected metastatic lesion (mestastasectomy). As such, I believe, adjuvant immunotherapy is indicated after complete metastasectomy.

    I have a 4 cm cancer tumor on my right kidney. August 30th 2023 they will. Urology of Virginia will remove the tumor. And save the kidney. Urology of Virginia surgery will be done in Virginia Beach. And I hope I live longer than 5 years after this.

      Absolutely you will live as long as you wish. 5 years life span only for a 4 cm renal cell carcinoma without metastasis is as absurd as saying that an oak tree will not grow taller than 5 feet. It will grow taller than 5 feet and you will live longer than 5 years for sure!

    Dr. God bless you for this video. I am 39 with 1.7cm tumor on North Pole of kidney. Doctors suggest cryoablation or monitoring. Any advice would be greatly appreciated.

    my father has 7cm tumor on left kidney and pulmonary nodules with mildly enlarged mediastinal lymph nodes and no symptoms at all doctors have advised cyto reductive nephrectomy for my dad but the tumor is only on the kidney and has not invaded any of nearby organs or lymph nodes not even renal vein or inferior vena cava....is it possible that nodules in lungs are maybe due to some old infection and not due to metastasis? I have seen the films of my dad's ct and the nodules in lungs are smooth and round but the radiologists here in Pakistan are calling it metastatic renal cancer....I have read on internet that malignant nodules in lungs are rough......Please reply I am really worried and we can't even think of living without our father.

      I have a 4 centimeter cancer tumor on my right kidney. They will remove the tumor not the whole kidney. I hope everything works out for your father. Your post is 4 months old I hope everything worked out.

    Hi Dr Stanley, my mum is a stage 4 rcc patient, she is on Pazopanib 600mg. But she kept having liver inflammation which caused her to have the pazopanib 600mg dose, on and off. Have you come across a case as such? Would happy to seek for your opinion. Thank you.

      If she doesn’t tolerate 600 mg, it should be reduced to 400 mg a day.

    Four -six years should not be considered long term survival......20+ years is long term survival.
    Why has there been no studies to locate what causes the kidney to develope cancer ?

    Hi Dr, 1 months before I diagnosed with ct scan found my right kidney 4.9 cm rcc, one urology told me he will do robotic surgery but not sure he can save my kidney or not, can you tell me what I should do please, god blessed you.

      It is very difficult to give you an advice without knowing your disease in detail.
      I would follow the urologist recommendation. If you want more, I would see a medical oncologist to have the opinion from medical aspect

    How much higher is the risk of reoccurrence with cryoablation than surgery?

      For the small size tumor (T1), I expect cryosurgery will have similar outcome of cryosurgery, over 90% survival.

      Renal cell carcinoma usually do not respond to conventional dose of radiation. High dose radiation, such as stereotactic body radiotherapy may be used for a small lesion, but 4.5 cm tumor is too big for that.

      I have a 4 cm cancer tumor on my right kidney they have to remove the tumor no radiation will help it. They well remove the whole tumor but not the kidney
