How to read your heart monitor trace

  • Heart monitor traces can look daunting at first but reading them is fairly straightforward. In this video, I talk you through how to read a heart monitoring trace and work out what the different rhythms are. Heart monitors are often done to look for abnormal heart rhythm disturbances. A common heart rhythm disturbance is AF or AFib. In this example i show you what a heart monitor trace in a patient with AFib looks like

You couldn’t have explained it better. Thank you so much! I have a Kardia monitor and the odd flutters, skipped beats and others when I am not in full afib show exactly what you described. It makes me feel better having even a little more understanding of it all.

Excellent information as always. Following a stroke at the end of 2019 I started with episodes of AF in Jan 2020, they always start at night just as I am falling asleep and the first time it happened I thought the end had come! After several further episodes and visits to A&E still not told what was going on but not to worry. Finally Paramedics in the May got a strip recording which they gave me a copy of and explained that I had AF. Found Sanjay's web site and it has helped me a lot to get to grips with what is happening and also what questions to ask. This explanation of an ECG trace has also helped understand the traces from my Holter monitor so I can explain easier to GP and hospital. Wish some of the consultants I have seen over the last year would spend the time to explain what is going on!

Yes - I also want to recognize dr. Gupta for his tremendous contribution to the countless people out in the internet-land. Also thanks to youtube for posting those videos and making them available to the broader public. I happened to be patient who at age 72 experienced my first cardiac event diagnosed as Atrial Flutter. And I found dr. Gupta videos very useful, especially that none of those type of explanations came from the cardiologist and electrophysiologist I've seen. They both (I feel) were pushing ablation on me, without explanation why in my particular case, just after one event, such a drastic procedure is recommended. Based on videos of dr. Gupta and seeing the charts of my heart monitoring and echocardiogram described as "Unremarkable" I decided not to proceed with ablation.

@@AlexM-jd2ro I agree with you - it is nearly 6 month after I had atrial flutter. No other cardiac events. I'm doing just fine. Yet those two scumbags with the medical titles in front of their names, one cardiologist, the other one electrophysiologist, wanted to sent me for ablation. The only explanation is that is how they are making money. Ripping off Medicare on one hand by prescribing unneeded procedures and endangering lives of the patient on the other. All in the name of the mighty dollar.

@@SUGAR_XYLER I've seen electrophysiologist today for a follow-up visit. Again he was pushing on me ablation claiming that I would not have any atrial flutters after procedure is done. That is despite that my atrial flutter on 6/30/2021 was traced to botched dental procedure and despite that I did not have ANY cardiac events after 6/30/21. I think it is simply a malpractice. He was presenting to me ablation as nothing special, as an outpatient procedure, but then he told me that I should be able to walk in two or three days. Is that really outpatient procedure? . I told him that I do not plan to cut my body for no reason. MY follow-up wit the cardiologist is scheduled for three months from now.

I have a layman’s hypothesis that those who are most annoyed by PVCs as adults, are those who throughout their life have had ‘above average rhythmic’ beats, so that they (we) never had really any skipped beats, and thus are much more anxious when they start occurring. Just 5 per day can ruin someone’s life, and had you been desensitized throughout life, it perhaps would not bother you one bit. I could be speaking nonsense though :)

Sometimes it feels like I'm breathing in brisk air when I'm inhaling and I have pain under my lower pectoral. I went to have a chest x-ray and EKG done at the ER and they found something interesting; they said that my heart rhythm was normal but was a little off compared to one I had about a year or so ago. Also, the X-ray showed a swelling in-between my heart and my lung. I went to the ER recently because I had tachycardia with a heart rate if about 120-140 BPM but they sent me home because they did a few tests, including blood work, and found nothing wrong that they could see. However, I've been experiencing pain where the chest x-ray showed swelling and I have been having issues with cognitive Impairement and focusing which has only seemed to get worse. Personally, I don't trust the hospital since they're notorious for screwing things up and I just wanted to get your professional opinion on the matter.

Dr Sanjay Gupta first of all let me say I’m a huge fan and appreciate you greatly, somehow even though we have never met I feel like your informative videos allow me to at least sometimes feel a little safer with my experiences. Please do reply to this as it would mean the world to me. Quick life story, I’m 30 years old. Don’t smoke never taken drugs of any kind other than prescribed, I do not drink alcohol whatsoever. I only drink water no juice or fizzy drinks no coffee no tea literally water. I am over weight and working on this currently. I have suffered severe health anxiety since the age of 15 that I have tried all sorts for but by far the worst trigger for panic attacks is my what I believe to be ectopic beats. I feel a thud in my chest now and then. Now this started for the first time when I was 16 and had no idea what it was. I used to get this very very infrequently maybe 2 times a month or so. Just recently I’ve been having huge bouts I’m talking maybe 2 times a minute then nothing then 3 times a minute then nothing for the rest of the day. I do take gaviscon very regularly due to acid reflux disease I.e GERD. I one million percent notice these beats get much worse and much more frequent when my acid is bad or after eating. I have been to the emergency room around 4 times and each time I’ve had an ecg done but nothing happens whilst hooked up to the machine lol, blood tests every time which shows nothing but they just don’t seem to understand how terrified I am. I even asked if I could stay on the machine until it happened but they said no as other people also need to be seen which I totally understand. I have since actually actually bought myself a heart rate monitor that attaches to your finger. Luckily I managed to capture exactly what I’m feeling or should I say the exact moment that I feel what I’m describing as an ectopic beat so I will post the video with this reply. Lease ignore a lot of the video as I was having a bad panic attack. If you skip to the 2.45 mark this is 3 seconds before the actual incident I’m describing occurs at 2.48. This can happen around 30 to 50 times a day and I’m honestly believing my heart is going to give up. Please do reply Dr I’m more than happy to pay for a response I feel you hold the key to me feeling normal again yet I cannot contact you any other way other than your secretary who of course has told me how busy you are and it’s very easy to see why with how amazing of a person you really do seem to be. This is slowly destroying my quality of life and I feel like at any moment my life is going to end because of these Beats. I will add one more thing. If I get up and try and go for a brisk walk whilst having a panic attack because of the beats it does seem to help and for a short while they actually disappear them as soon as I try to relax they start again. Please help me. I’ve just realised I cannot post my video as a reply. Ok the heart monitor my heart looks to have a good rhythm then all of s sudden a very very short flat line for a split second then what looks to be a larger beat then back to normal. This however is happening a lot and makes me feel like my heart is basically going to say no thanks I’ve had enough times up.

I've been watching a lot of your older videos this past week - thanks for all the uploads - you make it very easy to understand. I have had afib for a number of years - it is under control right now with Flecainide, thankfully. I was getting tachy 150/min for hours and brady 44. Afib would last approx 12-15 hours and then just quit. I would get these 2-3 times a month. If Flecainide stops working, will look at ablation. Saw where it could be connected to Vagus nerve - your opinion? Also have other symptoms related to that.

Very useful detailed explanation indeed. Thank you. I've noticed that you had made a talk on Statins on primary prevention of heart disease. What is your opinion on using Statin for secondary prevention, and how low should the LDL go, if the LDL should go down at all? Thank you.

I just want to thank you for your kind, informative and compassionate videos regarding POTS, I can’t explain how beneficial they’ve been to me.
I’m wondering if you would be interested in doing some research on Lyme Disease? I would love to hear your thoughts on the chronic active infection vs. easily treatable debate.
Thank you for all you do Dr. Gupta, you are a true healer.
P.S. I myself have been suffering for 3.5 years now after that fateful day in June of 2017,
when my life forever changed. I am constantly searching for answers and hope.

I was taken to hospital by ambulance yesterday because I suddenly couldn't walk or stay awake. They found I had a heart rate of around 200. Emergency did a chemical cardioversion which didn't work followed by an electrical one which did work. In the recovery room I read my chart (and got in trouble with a nurse who said I wasn't allowed to read it - thankfully I had already taken photos of it, so I have a copy). Now I'm learning to understand it .Thanks,

Awesome and well articulated treatise on looking at the traces produced by the heart monitors.
Curious as to what the additional leads from the more comprehensive monitors provide in addition to those from single trace monitors.
Are there medical conditions where it might result in the spike indicators actually shifting in timing from one another, or is the additional information contained in the amplitudes and waveshapes of the different trace leads?

I have a Kardia device that takes a single line EKG. Connects with an app through my cell phone and has a device that you put two fingers on. It has the lined chart with the squares as well. That enabled me to follow my afib in ways that could not be picked up by the holter moniters. My doctors love that I have that and have found a lot of value in it. Can print the results out or send directly to your doctor office. Some of the apple watches do the same thing now. Important for whatever you use that it actually has the squares on the readout. There are many heart monitoriing apps out there, but important that they have the little squares or it will not benefit actually reading it.

When or what was the moment when you decided to start doing all these videos, giving us advice, understanding, free consultations?

Thank you, you gave some very good detail I was not already aware of.
I wonder if you could make the time to discuss bigeminy a bit further? I have been having long runs of bigeminy (normal atrial shape) which can continue for up to three hours. They seem always to start during eating a meal, and sometimes simply peter out but at other times lead into full paroxysmal AF which can last up to 14 hours. Due to the pandemic, I have not been able to ask my EP about these sessions, and thankfully they seem to have stopped happening so often now.

Thank you so much. i am having a holtor monitor put on me on the 27th of Jan 2022 . i suffer from very high Bp 200/120 and seem to be resistent to Bp medications The last three days my left arm is aching and its weird because it has like a dead arm feeling and it goes cold and clamy . I also suffer from fast weight gain and edema, My question is, Does the holter monitor pick up on things when your arm is aching or clammy if it is related to the heart. Thank you if you read this And hope you have a wonderful and happy New Yr. Peace
