Sir, one of the key point of All Inside acl reconstruction surgery is it's ability of less invasiveness and less incision on the bone. But if you include a screw in the smart acl technique, wouldn't it require the same amount of bone incision as it requires in the traditional acl reconstruction method? So wouldn't it lower it's benefits? What I mean to say is, when you put a screw, you have to dug a tunnel on the tibial side (which is avoided in the all inside technique making it superior) and so ultimately the same amount of bone is removed as done in the traditional method. So how is it better than the all inside technique? Please share your opinion Sir 🙏

very nice question, long term results will depend on solid fixation and decreased long term retear rates which will be very less with smart technique . it has been shown in many studies that button alone is a relatively weak construct and has increased risk of late laxity. with this technique we combine advantages of both the techniques for best long term outcomes. In our practice it doesn't have any increased invasiveness, our incision is less than an inch . We do a lot of all inside ACL surgeries as well and haven't seen any significant difference in immediate pain as well. In fact our center launched Stryker Versectomic RR system first time in india. it is a very advnaced all inside retroreamer for socket preparation in all inside technique.
