As a health researcher, I enjoyed this discussion. To add to this discussion, there are other compelling models in countries such as Switzerland, Singapore, and Australia. The Us model is failing miserably in many ways because it's considered a business than a service. This approach has created a severely and chronically ill population in the US. Also, the insurance-based health model has evolved since the world wars. So, it is difficult to do a direct comparison in terms of implementing it in a country like Sri Lanka. Regarding education models, the student debt crisis has been a decisive factor in the US presidential races; Manju should know better about this. Also, there is a significant quality difference between private and public US universities because of the amount of funds they receive. As Malinda mentioned, ownership is a key factor that should be emphasized when implementing a new system. Sri Lanka, as a welfare state, has never thought of periodic and systematic evaluations and has not evolved with the rest of the world because the population has been made to believe they are better than others and living in a comfort zone. Sadly, we have not challenged our thought processes to ask serious questions for intellectual discussions.
I am in USA. What you are saying is true in general. But "Us model is failing miserably" compared to best education systems not compared to the education system in SL. I would say US system is in middle whereas SL system is hitting the bottom. Education systems in Switzerland, Singapore, and Australia may be in the top. Also, good thing about USA is they are not hiding their failures and working towards better approaches. You need to understand USA is very different to Switzerland, Singapore, and Australia, in terms of history of slavery (African Americans have to struggle extremely hard), we need to take into account all these geo political and historical aspects too. I work at a USA University. I agree with you on US health care because it is relatively expensive and may not be affordable to some. But I read that Canadian and France’s insurance models are more affordable, but still, the quality of the USA is top-notch if you can afford it. (In France, insurances are sold by not-for-profit organizations; in Canada, the government & private companies finance the insurance.) The student debt crisis may be a political narrative except for some “African-Americans” and a few who borrowed from for-profit private organizations. The Dept. of Education loans are very reasonable, and no interest accumulates for a certain period. In addition, according to College Board, the US student received around $139 billion in scholarship grants (federal ($30B)+state + other) in 2020. The bottom line is Sri Lanka needs a system to absorb those who pass the A/L exam and want to pursue higher or technical education. We cannot do it without private & public partnerships + subsidized student loan programs.
In US any high school graduate has the oppotunity to persue higher education. Just after graduation or any time in life he or she thinks fit can start higher education. Of course it has a cost but the important thing is availability of the oppotunity. In Sri Lanka even those who are near the top tier who do not qualify for the full ride in the Government university system are left high and dry. It's only through a comprehensive public private partnership that this injustice can be corrected.

I like the long-term loan system of UK’s higher education, where every student gets means tested assessment for eligibility for a loan and whatever the amount they get, they will pay back once they graduate, and get a job (salary deductible). If they decide leave (migrate) the country before the loan is paid off, they have to settle the full amount before leaving. Universities in turn gets the money directly for each registered student and it is therefore in the interest of that institution to provide a good, quality education to retain and recruit students
I am really looking forward to see these models practice in my mother land. I think the combination of private, insurance and public for basic needs, where the poor or accidental emergency services in health care. At the same time in education sector, both public and private schools with more practical learning curriculum and focusing for technical education where there is more vanue to satisfy our job market is the more needed model to my mother land.

Rohitha Australian system is probably the best in the world and is similar to what they've mentioned here and seems like you are living in the US or SL which neither have an effective system

Great discussion! Regarding the government funded student loans, there should be a very effective and smart way to recover the loan, the normal bank loan repayment methods should not be used, objective should be to recover the loan without allowing the loan recipient feel that he or she is paying off a loan. One method which is used quite effectively by several countries is by applying a special Levi/Tax based on the income thresholds once the loan recipient start earning, first income threshold amount should not incur any levi/tax, once the income reaches subsequent thresholds then it should start incurring a levi/tax, this levi/tax will continue to incure until the full loan amount is recovered. By doing this the loan recipient doesn’t feel/ know that the loan is being repaid as repayment amount is not paid out of his pocket.

Insurance part.. Employers should do it for their employees.. They can deduct it from the salary and make sure all their employees are covered.. If the employees require, they can apply for their family members as well.. And that too can be deducted from their Salaries.. 50% of the issues will be covered..
of cause those models are very practical. I think before execute models like this, need to change social mentality. also too much political interference MUST BE stopped....
The third system you discussed is available in Australia and which is called medicare system. In the medicare system which provides certain procedures 100% free and bulk billed, some procedures there is Medicare gap. Which means a percentage is paid by the prepaid tax system and the balance is out of pocket expenses. Once you have the treatment you have to pay only the gap fee. However, the government promotes to have health insurance to support facilities for the people who can afford. This enables public to have quality health care, access to private billing practices, quality specialist care and high amount of options to choose.

One good aspect of a health insurance system that was not discussed is that the Insurance Company is vested in the good health of the customer. So they promote more preventive care in terms of anual checkups, colonoscopy, mammogram etc. This way they can avoid large claims later by treating people before getting them seriously sick. Of course they will make profits, but the customer will also be benefitted if he can avoid a colon cancer or a heart attack in advance.
They are also vested in selecting better off customers, basically they'd rather have a customer who's not sick to begin with rather than someone who's currently sick or is going to be due to genetic or underlying conditions. This lets them run analysis on patterns on their own customers can weed out people who may get sick. No government has the funds to regulate such insane practices that actually came to effect just because insurance companies were invited to solve an issue that'd not exist in a purely "prepaid" fund model. If a patient is not going to be treated or going to be denied funding for some treatment the logic behind how that decision was taken should be open and it should be fair (e.g. not funding smoking related illnesses). Mandatory tests are just the tip of the iceberg where you'll find more goodies later on once the health/life insurance business has had their way for a few decades -- when they have enough data and experience they'll predict who to fund, adjust their policies accordingly, fund and lobby politicians to get laws passed that'd eventually let them make even more money by rejecting payment in "fair" claims. The only difference then is that you just arrived at a shitty health system through many steps.

Machan America is the worse example to take for healthcare.. I have friends who are doing decent jobs in the US who have a decent income who comes to Sri Lanka or even go to South America for dental care.. recently a friend who works in the IT field who came down asked for a dental recordation... and I was like why not get it there cos insurance covers it right.. this was for a root filling.. he was like even after insurance cover I still have to pay around 1500 dollars. so insurance doesn't cover all the cost unless you have one of those super premium insurances... this leads me to the second issue in America... 1500 bucks for a root filling after insurance.. in Sri Lanka with 1500 dollars if you go to a premium paid private doctor how many root fillings will that 1500 get you... probably the whole mouth... this is cos of privatisation of healthcare and then the healthcare and insurance lobbiest who are one of the most powerful groups lobbying government to deregulate restrictions with pricing and all that.. so they can charge insane amounts of money for medicine and healthcare.. look at how much insulin cost in the US... if I am not mistaken even a ambulance pickup cost around $2500 dollars... how many people in the US don't get essential medial procedures cos their insurance doesn't cover it.. you can TH-cam US healthcare to see how many issues it has.. Sri Lankan healthcare we have no from the people's perspective is much better than that.. And also for people who have money Sri Lanka has enough insurance companies that offer healthcare insurance.. and there are enough of private companies that give their staff medical insurance... when I was at MAS they even pay for you for your eye glasses which most people abuse and buy sunglasses and get a different invoice cos amount they pay for glasses is a high amount.. Machan there is no පිං healthcare.. as you say nothing is for free.. so tax payers pay for it... so no one has to go with a පිං mentality.. I go to Kalubowila hospital when I have some issue even when I almost cut my fingers to the bone... and I have got a decent service.. can it imporve? for sure.. my mother does a lot of charity work with Apeksha.. you go on a Tuesday morning and see how many low income people come to meet them to get their bus travel cost reimbursed.. cos they can't even afford the bus fair from their village to apeksha that they have to spend to come for their weekly or monthly clinic.. and if there people are not given this then they don't come for clinic cos they can't afford it.. go to a post office on a පිං පඩි day and talk to the people who come to take something like 300-600 bucks and see how helpless they are.. my mother's organisation gives buns and tea on one day of the month at a post office to those people who come.. there are some old people who will eat half the bun and put the other half in their bag and take it home to give to a kid or eat later.. which means they can't even afford to buy a bun for a kid.. with the subject of healthcare what you guys are saying is "there is so much mismanagement and no motivation in the current system, so let's privatise with insurance backing" what I say is for the middle class and upper class insurance is available and they get a decent service form those providers at private hospitals.. what needs to be done is clean up the processes and funding and methods in government healthcare.. what's the total % of the budget we use for healthcare.. I am not sure but I don't think it's even 10% probably around 6%.. I know education is less than 6% of that what ever % how much is paid for idol staff.. you know some years back at Apeksha a elevator door (a new elevator was bought to be fixed) went missing from the hospital.. recently we had a case where a person wanted to give a Sego dene to the patients and gave cashew and plums to ass to the sego to the kitchen.. in the morning they noticed that the sego that was distributed has no cashew or plums.. there is chemotherapy drugs that are emptied and filled with water.. the answe is not privitation. that answer is proper regulations and management and systems.. a couple of years ago a new head was put into apeksha.. this guy stands near the sign-in book at 8.30 or what ever time people are suppose to sign in at and at the exact time draws a line so people so sign in after can't cheat..(yes it's still a physical book and not digital finger print) you know what the staff did.. they spoke to the local politicians and got the guy transferred... these are all mismanagement issues.. that are leading to all other issue including finance.. for healthcare why not open up a separate fund that will finance healthcare.. this fund will be filled with taxes.. say we tax private hospitals right.. all the tax money from private medical care goes to that fund.. we tax alcohol and tobacco so much.. when asked one of the answers you get it because they cause long-term medial issues and we provide free healthcare.. so why not dedicate a % of those taxes to the same fund? what we need is proper regulations, proper management and proper oversight.. politics not getting to just hire people and overcrowd hospitals with giving their voters jobs.. we can do what you guys are suggesting but what you don't see is if we do we will be exactly like the US or worse when it comes to the middle and low income people cos don't forget it's still the same caliber of Sri Lankanpeople we have managing these too. so if they are to be done there has to be proper oversight and laws and controls and all that which in Sri Lanka again is all corruptible so then your again stuck with a whole help of new problems.. side note.. if you see how much money some of Sri Lankas leading healthcare insurances pay as commissions you will see the issue with privatisation.. මං හිතනවා රජයේ සහ පුද්ගලික විශ්ව විද්යාල වෙනුවට full scholarships, half scholarships and fully paid ඔක්කොම හැම course එකටම විශ්ව විද්යාල වල තියෙන්න ඕන කියලා. මගේ batch එකේ මේ සියලුම සමාජ තල වල අය ඉන්න ඕන. මට යමක් බැරි නං full scholarship එකක් තියෙන දක්ශ ලමයෙක් ගෙන් pay කරලා tutoring ගන්න පුලුවන්. එතකොට course එකක standard එක තියාගන්නත් පුලුවන්. ඒ වගේම fee එක ගෙවන අය නිසා විශ්වවිද්යාලය පවත්වා ගෙන යන්නත් පුලුවන්. Full scholarship ගන්න අයත් ඒක එයාලාගෙ මහන්සියෙන් earn කරනවා.
සහ එකිනෙකාට ස්වාධීනත්වයක් ඇතිකරගන්න ඒ දේ වටිනවා එතනදී නවකවදය අසීමාන්තික පක්ෂ දේශපාලනයත් මේ ආයතනතුල පාලනය වීමක් සිද්ද වෙනවා.

Regarding the government funded student loans, there should be a very effective and smart way to recover the loan, the normal bank loan repayment methods should not be used, objective should be to recover the loan without allowing the loan recipient feel that he or she is paying off a loan.
One method which is used quite effectively by several countries is by applying a special Levi/Tax based on the income thresholds once the loan recipient start earning, first income threshold amount should not incur any levi/tax, once the income reaches subsequent thresholds then it should start incurring a levi/tax, this levi/tax will continue to incure until the full loan amount is recovered. By doing this the loan recipient doesn’t feel/ know that the loan is being repaid as repayment amount is not paid out of his pocket.

The third system you discussed is available in Australia and which is called medicare system. In the medicare system which provides certain procedures 100% free and bulk billed, some procedures there is Medicare gap. Which means a percentage is paid by the prepaid tax system and the balance is out of pocket expenses. Once you have the treatment you have to pay only the gap fee. However, the government promotes to have health insurance to support facilities for the people who can afford. This enables public to have quality health care, access to private billing practices, quality specialist care and high amount of options to choose.

One good aspect of a health insurance system that was not discussed is that the Insurance Company is vested in the good health of the customer. So they promote more preventive care in terms of anual checkups, colonoscopy, mammogram etc. This way they can avoid large claims later by treating people before getting them seriously sick. Of course they will make profits, but the customer will also be benefitted if he can avoid a colon cancer or a heart attack in advance.
They are also vested in selecting better off customers, basically they'd rather have a customer who's not sick to begin with rather than someone who's currently sick or is going to be due to genetic or underlying conditions. This lets them run analysis on patterns on their own customers can weed out people who may get sick. No government has the funds to regulate such insane practices that actually came to effect just because insurance companies were invited to solve an issue that'd not exist in a purely "prepaid" fund model. If a patient is not going to be treated or going to be denied funding for some treatment the logic behind how that decision was taken should be open and it should be fair (e.g. not funding smoking related illnesses). Mandatory tests are just the tip of the iceberg where you'll find more goodies later on once the health/life insurance business has had their way for a few decades -- when they have enough data and experience they'll predict who to fund, adjust their policies accordingly, fund and lobby politicians to get laws passed that'd eventually let them make even more money by rejecting payment in "fair" claims. The only difference then is that you just arrived at a shitty health system through many steps.

Machan America is the worse example to take for healthcare.. I have friends who are doing decent jobs in the US who have a decent income who comes to Sri Lanka or even go to South America for dental care.. recently a friend who works in the IT field who came down asked for a dental recordation... and I was like why not get it there cos insurance covers it right.. this was for a root filling.. he was like even after insurance cover I still have to pay around 1500 dollars. so insurance doesn't cover all the cost unless you have one of those super premium insurances...
this leads me to the second issue in America... 1500 bucks for a root filling after insurance.. in Sri Lanka with 1500 dollars if you go to a premium paid private doctor how many root fillings will that 1500 get you... probably the whole mouth... this is cos of privatisation of healthcare and then the healthcare and insurance lobbiest who are one of the most powerful groups lobbying government to deregulate restrictions with pricing and all that.. so they can charge insane amounts of money for medicine and healthcare.. look at how much insulin cost in the US... if I am not mistaken even a ambulance pickup cost around $2500 dollars... how many people in the US don't get essential medial procedures cos their insurance doesn't cover it.. you can TH-cam US healthcare to see how many issues it has.. Sri Lankan healthcare we have no from the people's perspective is much better than that..
And also for people who have money Sri Lanka has enough insurance companies that offer healthcare insurance.. and there are enough of private companies that give their staff medical insurance... when I was at MAS they even pay for you for your eye glasses which most people abuse and buy sunglasses and get a different invoice cos amount they pay for glasses is a high amount..
Machan there is no පිං healthcare.. as you say nothing is for free.. so tax payers pay for it... so no one has to go with a පිං mentality.. I go to Kalubowila hospital when I have some issue even when I almost cut my fingers to the bone... and I have got a decent service.. can it imporve? for sure.. my mother does a lot of charity work with Apeksha.. you go on a Tuesday morning and see how many low income people come to meet them to get their bus travel cost reimbursed.. cos they can't even afford the bus fair from their village to apeksha that they have to spend to come for their weekly or monthly clinic.. and if there people are not given this then they don't come for clinic cos they can't afford it.. go to a post office on a පිං පඩි day and talk to the people who come to take something like 300-600 bucks and see how helpless they are.. my mother's organisation gives buns and tea on one day of the month at a post office to those people who come.. there are some old people who will eat half the bun and put the other half in their bag and take it home to give to a kid or eat later.. which means they can't even afford to buy a bun for a kid..
with the subject of healthcare what you guys are saying is "there is so much mismanagement and no motivation in the current system, so let's privatise with insurance backing" what I say is for the middle class and upper class insurance is available and they get a decent service form those providers at private hospitals.. what needs to be done is clean up the processes and funding and methods in government healthcare.. what's the total % of the budget we use for healthcare.. I am not sure but I don't think it's even 10% probably around 6%.. I know education is less than 6% of that what ever % how much is paid for idol staff.. you know some years back at Apeksha a elevator door (a new elevator was bought to be fixed) went missing from the hospital.. recently we had a case where a person wanted to give a Sego dene to the patients and gave cashew and plums to ass to the sego to the kitchen.. in the morning they noticed that the sego that was distributed has no cashew or plums.. there is chemotherapy drugs that are emptied and filled with water.. the answe is not privitation. that answer is proper regulations and management and systems..
a couple of years ago a new head was put into apeksha.. this guy stands near the sign-in book at 8.30 or what ever time people are suppose to sign in at and at the exact time draws a line so people so sign in after can't cheat..(yes it's still a physical book and not digital finger print) you know what the staff did.. they spoke to the local politicians and got the guy transferred... these are all mismanagement issues.. that are leading to all other issue including finance..
for healthcare why not open up a separate fund that will finance healthcare.. this fund will be filled with taxes.. say we tax private hospitals right.. all the tax money from private medical care goes to that fund.. we tax alcohol and tobacco so much.. when asked one of the answers you get it because they cause long-term medial issues and we provide free healthcare.. so why not dedicate a % of those taxes to the same fund?
what we need is proper regulations, proper management and proper oversight.. politics not getting to just hire people and overcrowd hospitals with giving their voters jobs..
we can do what you guys are suggesting but what you don't see is if we do we will be exactly like the US or worse when it comes to the middle and low income people cos don't forget it's still the same caliber of Sri Lankanpeople we have managing these too. so if they are to be done there has to be proper oversight and laws and controls and all that which in Sri Lanka again is all corruptible so then your again stuck with a whole help of new problems..
side note.. if you see how much money some of Sri Lankas leading healthcare insurances pay as commissions you will see the issue with privatisation.. I worked for a bank for 8 years.. towards the last 2-3 years this bank tied up with a multinational insurance company to promote their products.. when I was leaving and even now after around 6 years (cos I still have friends at the bank) management of the bank spends more effort on promoting insurance policies that promoting fince products cos the commission they get out of that is much higher than what they earn from promoting banking products...

Lol im a srilankan who lives in usa .i think us health care is topnotch compared to srilanka but it is expensive while our srilankans are on long quese try to go to hospital 5 times . I think srilanka should follow scandavian health care model which is farbetter than us and srilanka

America is great is you have money or a good insurance.. but if your low income or even a SME then access to healthcare is not the greatest.. you always have to end up paying and that paying is not cheep...
