    I was laid up in the ICU for 2 weeks with this and DKA! Worst pain I’ve ever experienced. After significant lifestyle change I am still non diabetic 4 years later. Great explanation

      @@heystara7318I had it in July & was in hospital for 3 day. Then got it again on Monday & just got home yesterday. They asked what was my pain level 1-10 when I arrived I said 28 & it took me multiple doses of liquid dilauded just to get the pain manageable. It’s an indescribable pain

    I was recently hospitalized with acute pancreatitis due to excessive drinking. When I got out, I did the best to explain to people what I was told by the hospital staff, but between the pain and being sleep deprived and them talking quickly I didn't really get a full understanding.       The same thing happened to me twice now in the past three years.. I'm not even an alcoholic by the standards of withdrawals or shakes, I just like to drink to calm my anxiety and constant worrying. I unfortunately have been drinking again very lightly (just a couple small glasses of wine versus my five vodka nips a night.). I'm feeling a little discomfort right now as we speak and that's why I'm here watching this, so please wish me luck just as I hope everything worked out for you my dude. The pain is fucking unbearable

    I want to tell you something. I had my gall bladder out 6 years ago. I have suffered increasingly for those 6 years. 3 weeks ago I went to the emergency room where they blood and CT scan I had a stone in my pancreas. I was rushed to a hospital where they went thru my throat .. stomach and into my pancreas and took out the stone that had ( what the doctor said) looked like it had shot itself into the flesh of my pancreas. Pancreatitis is the most painful and life altering problem that affected me eating, sleeping and every day living. It was HORRIBLE!! Now I am finally pain free and feel like I have a new lease on life. What a fiasco!!!!!

      @@leemcdermott1770 the stone evidently had lodged itself into my Pancreas before they took it out. Since it went thru the duct and shot into the flesh of the Pancreas. It was awfully painful for all those years!!

    If you allow me there are some informations I'd like to add to your great video and I hope to be useful to everybody . For example the importance of CAT scan with contrast to differentiate between edematous and necrotising pancreatitis . The necrotising type is the fatal one because it's going to cause septic shock . Also the importance of fluid resuscitation and vasopressors in such cases . Acute lung injury ( ALI ) term is not used anymore , it was replaced with another one which is mild, moderate or severe ARDS based on P/F ratio and the other ventilatory parameters .

    I used to be a heavy drinker for 20+ years and 2 and a half years ago I started getting pain in my right quadrant so I went to the ER! My Amylase and Lipase levels were a bit high and they diagnosed me with Acute Pancreatitis! Scared me to death! They didn’t even keep me overnight which was strange! Just told me to cut back on alcohol! I did one even better and completely quit! I continued to feel pain even 6 months later in those same areas despite quitting drinking! My doctor performed blood tests, CT scan, ultrasound, and X-ray! All normal findings! Normally people would be happy but I knew they were missing something! I stopped getting pain up until 2 weeks ago I started getting pain AGAIN in those same exact areas! I made a doctor’s appointment yesterday! It’s been 2 years since my tests that were all normal! Haven’t had a drink since my AP diagnosis! What is going on? I’m so confused.

      Hope that you are doing well. I had AP 2.5 years ago as well, went to low-fat diet and quit drinking and complete lifestyle change, and i too at times have felt that “stabbing” or almost as if my pancreas is being grabbed middle diaphragm/left lung area. Doctors say that i am fine and in MUCH better health then before i ever had AP. They have said that i am free to eat what i want and have no damage to my pancreas but it is frustrating to still know where pancreas and feel it at times when everything should be back to normal.       @@theo.g.441 hi, i started drinking at the age of 18 when i joined the military, would take a year off during deployments etc. Around the time and for a few years before the A.P. I would drink maybe 2 glasses of wine one or two nights a week, and normally Thursday nights, and sundays during the day i would binge drink. So it would normally be 1 or 2 days a week where i would have let’s say 10 total drinks throughout the day. Either drinking Jameson, or Tito’s vodka. And when i had the AC i went to a concert on a Tuesday night and had about 4 drinks, no drinks on Wednesday, Thursday morning i was in bad pain and went to the hospital.

    Great Vid, thanks!!! But FYI and for others, a nasogastric tube is not recommended for patients with pancreatitis. The pancreas is stimulated by cephalic, gastric, and intestinal phases of digestion, including vagal stimulation as food empties from the stomach into the duodenum, the release of cholecystokinin from the presence of luminal fat and protein, and the release of secretin in response to acid. The more distal in the GI tract nutrition is infused, the less stimulation of the pancreas by these factors. Thus, EN into the jejunum, beyond the Ligament of Treitz (LOT), is recommended. I really appreciate your vids! :)

    Ye, morfine shoudnt be used because it contracts sphincter of major duodenal papillae, makes harder to excrete bile and pancreatic juice.

      @@BUTTA170 The large pseudocyst that had previously shrunk to 5 millimeters has now regrown to 8 centimeters. They will be doing an EUS to go from inside my stomach into the pseudocyst to install a connecting shunt to drain and biopsy the fluid. Then if all goes well, two weeks later they will repeat and close the opening in my stomach.

      abh Rajput It’s been delayed until June. I haven’t had the surgery yet. I have had two EUS previously with no issues though.

    I had acute pancreatitis at the age of 19 . It was found that it was because of hereditary.
    I had very severe pain in my back and stomach. My lungs were not working properly and my kidneys were also failed. By god's mercy I recovered in 20 days being in ICU . At this time I was not knowing this can happen to me again. But when I was 20 year's old . That is 1 year after I had 2nd attack of pancreatitis. Same pain. Just like someone squeezing your pancreas . I was hospitalized for 15 days this time. recovered.
    From my side there wasn't any mistake . Still it happened. May be my past life Karma were too bad.
    Please tell me how can this will not happen to me again.
    Any suggestions and warnings.
    I am on almost zero fat diet with 6 small meals a day.
    Please tell some more suggestions.
    Worst thing:
    My brother also has same problem he is younger than me .
    He had pancreatic attack when he was 17.
    At his time it took 2 days for diagnosis. But because of him I understood at my time within seconds that next few days I am going to hell.
    Pain level 💯.
    Please help 🥺

    This pain for me is upper left body abdomen, I get sharp/dull pain every couple of hours. It gets worse after eating, it’s just there... the pain. I’m not sure if I should see a doctor yet because it’s coming and going and it could be otherwise as I have stomach issues already. I’m tired of going to the doctors regarding my stomach pains, it’s taken my youth away and as a young adult, my life revolves around how I am feeling. I’ve missed so much opportunities and just feel like giving up because I feel like my stomach issues will never go away. I feel defeated these days and I am 25 years old.

    Doctor In post operative status of gallbladder, after a span of 2 years can pancreas be still enlarged and enlargement /inflammation is the same thing ? Or is it like this organ might be enlarged when there was gallbladder disease and after removal of gallbladder it is only enlarged but not inflamed .
    Kindly please clarify this doctor

      I had bad gallbladder infection and then got my liver enlarged and doctors had to perform an emergency surgery to save my life as I was in sepsis
      But after removal of gallbladder I started having mild pain in right upper abdomen part and I was very cautious of my liver which showed mild fatty infiltration
      But there have been 2 too worst pain episodes in my life where I had consumed milk or some fatty food I had life taking pains
      When I went through various scans it shows
      Mildly enlarged pancreas showing loss of normal pancreatic lobulations with normal signal intensity noted. No peripancreatic fat stranding/collection noted. No focal lesion seen . MPD is not dilated

    I just got out of hospital after being diagnosed with this through blood tests. I’ve never experienced so much pain in my life . They kept me in for a day and a half and sent me home with painkillers and said it will repair by itself. Painkillers help but the pain is always there. I also suffer from peripheral neuropathy so having this on top is just too much for me to bare.

    Hey, correct me if Im wrong, but I think you shouldnt give morphine to these patients as it causes smooth muscle contraction and might worsen the pain especially if there is obstruction in the ducts.

    My mother died of accute pancreatitis.It was so fatal that within 24 hours she died of multiple organ failure. The problem is she didn't have any symptoms and she was also not knowing that she will be dying.It was shock to our family.

    I've been diagnosed with Acute Pancreatitis and was in hospital for 11 days the worst pain in the upper part of the belly and lower back pain then the Nasuea I knew the moment my mouth like my siliva would taste like blood and start getting the worst Nasuea the vomit 🤮 for two ✌🏾 days continues. Horrible times but the staff were excellent in the hospital gave plenty of pain relief 😌 but I have been advised that they will like to remove the gallbladder by surgery. As I had slush in the bladder

    Couple of things. My pain from my post ERCP was so bad morphine was NOT sufficient. It only touched the pain for half an hour before I started screaming in agony again. Fentanyl was used. I had very severe acute pancreatitis. 2) you forgot to mention pseudocysts as a complication. Otherwise excellent video.

    Wow, I suffered for 3 days and eventually went to the doctor. Did anyone here have a warming sensation in their legs a few days before this happened to them? Another issue I had noticed was my stool didn’t look as it normally did either. After a liquid diet for a few days I now have very sore thighs (muscles) and incredibly foul smelling urine. Maybe removing toxins out of the body?

    Hey! I love your videos! Just one detail: you can´t use opioides in pancreatites because they can cause a spasm of Oddi's sphincter and due to their vagal aspects, which would make the case worse.

      Natassha Katerine pethidin is opioid. From opioids morphin is contraindic ,why it makes oddi spasm, and pethidin have a bad effect on paralitic bowel which already exists , when pancreatitis is there. Thats why peridural catheter analgesia can also been used.

      I think the NG tube is used for gastric decompression. So it removes gastric secretions to promote bowel rest and prevent vomiting/aspiration

    Just adding a couple things:
    - the management of acute pancreatitis changed. Whether patients can or cannot eat depends on their symptom severity, in no way every patient with pancreatitis should get a nasogastric tube.
    - there is no advantage of serum amylase over lipase, thus guidlines nowadays recommend only to check for lipase, as it is the strongest marker for pancreatits
    - to discriminate between mild and severe pancreatitis check: CRP, Calcium (if it drops, it's foreshadowing a worse progression), LDH, AST, and look for hyperglycaemia
    - cannulation and administration of fluids if key(!) over every other primary intervention
