I think the letter has to be signed off using "faithfully", not "sincerely". This is due to the fact that the greeting you have started off with is "Dear Sir/Madam" which we don't know who the receiver is. I suppose the word "sincerely " can be used only if we know the receiver of the letter. In other words, if the greeting is started off with someone's (receiver) name.
