How to record an incident with Behaviour Smart

  • Welcome to Behaviour Smart! In this tutorial I will show you how to record an incident using Behaviour Smart. Let’s go!
    First select ‘New Incident’ from the main menu.
    Select the name of the person we are writing the report about. Select the date, and time the incident took place. Now type the location of the incident.
    Now select the name of the person completing the report. and the name of the staff involved in the incident. Select other service users involved if any.
    Note that you can select multiple staff or services users if more than one person was involved.
    Type the name of any witnesses.
    Select the reason or reasons we intervened.
    `You can also report if you made a dynamic Risk assessment. Followed a plan, and if your action was in the best interest of the service user.
    Now click next page.
    Write a Brief Description of the incident.
    Did you see any low, medium or high-level behaviours? How did you respond? Can you think of any alternative strategies that might be tried next time. All these field will be added to the service users Smart Plan.
    The next page should be completed if physical intervention was used or medical attention was needed.
    Now you can record the effectiveness of any de-escalation techniques or physical techniques that were used. The effectiveness scores will be updated on the service uses Smart Plan.
    The incident report can be saved for later at any point.

    We record the recover behaviours, the service users views, feelings alternative strategies the next time they feel that way, likely triggers and reasonable adjustments could be made in the future.
    And that’s it.
    When you click submit the report will automatically be sent the behaviour leads you that were created by admin and stored in the directory. you also have the option to send the report to key contacts.
