Promoting Meaningful Electronic Health Information Exchange: Inclusion of IDDSI in SNOMED CT

Every day, electronic records around the world record a vast amount of health care data. So, the real challenge is ensuring professionals have the right ideas for communicating the care they provide and can ensure safe, effective, and high-quality care, particularly in transitions of care. This webinar will establish what professionals need from Electronic Health Records using the clinical terminology SNOMED CT to support the care they provide when working with persons who have dysphagia, how consistent nutrition processes and terminology ensure meaningful communication, and how nutrition standards aim to offer the necessary guidance and structure for electronic health record exchange. Opportunities and challenges in the current state of nutrition terminology and standards and ways for professionals to become more knowledgeable about this work will be explored.

1.Explain the rationale for inclusion of IDDSI concepts in the clinical terminology, SNOMED CT
2.Illustrate through a case example of how structured nutrition orders and terminology facilitate communication and improved quality of care across health care settings, such as, from the inpatient to a post-acute setting
3.Describe the role of nutrition standards to support electronic health records
