Hi Neil. Sorry you're going through this.
I waa diagnosed in November 2010 at 52 years old and had a prostatectomy in January 2011. My PSA remained undetectable for just under five years when it came back at 0.05.
It bounced around between 0.04 and 0.08 for 18 months before it started a steady but very slow upward climb. Because it was climbing so slowly, we agreed to monitor without additional treatment.
It took 6 years to climb to 0.2 and, at that point, we decided to take action.
I was quite concerned about radiation side effects and I really wanted to know where the cancer was before we started zapping.
I went for a Ga-68 PSMA PET scan in November 2021 when my PSA was 0.22. It didn't show anything.
By spring 2022, my PSA climbed to 0.36 so i received a six-month dose of Eligard (ADT) in May and started 7 weeks of salvage radiation therapy in July.
The ADT knocked my PSA down to 0.05 in November 2022. After the Eligard wore off, it was 0.13 in March 2023 and 0.11 in May.
But in October, I was shocked when my PSA nearly doubled from 0.11 to 0.21. Five weeks later, it was 0.33.
We're in the process of setting up another PSMA PET scan and PSA test.
My post-surgery Gleason was 3+4, and my prostate came out cleanly--negative margins, no ECE, SVI, LNI.
If you can, find one lab to run your PSA tests and stick with it. Doing so eliminates a variable.
Also, try to establish your PSA doubling time to see how aggressive the growth is.
Was i a fool to wait 6 years with a rising PSA before initiating salvage radiation? Maybe. Maybe not. In those 6 years, I had a high quality of life that let me do a lot of things. (I just turned 66.)
I will admit that I was probably more worried about salvage radiation side effects than I should have been. 18 months out, they have been very minimal. (A little increase in stress incontinence and slightly worse ED.) But i also know that it can take 3-5 years for some radiation side effects to show up.
All the best and remember to be your own advocate.
If you're interested, I've been documenting my treatment on a blog: dansjourney dot com.

You are not alone. At 51 had radical prostatectomy and pathology came back P3b N1 Mx Gleason 4+3=7 tertiary 5. PSA at six weeks post was 0.03. Referred to radiation oncologist and he hit me Hard with 18 months of ADT and 6 weeks beam radiation. My 10 year check up is next month and PSA is undetectable. I am grateful to be here and mindful of the toll paid. You can do this.

I was diagnosed in 2018 with Stage IV prostate Cancer with Metastasis. I have Mets in my left rib and pelvic lymph nodes. It was worked up with a PET scan & PSMA scan after my MRI pelvis & Chest/Abdomen CT. I was treated with prostatectomy, radiation to pelvis and left rib/chest. I have been on Zytiga/prednisone since. PSA/Testosterone test every 3 months. Good luck my friend. KZ🙏🏼

Best of luck. This past August I had a PSA of 24 and the Gleason scores on 12 biopsies were eleven 10's and one 8. I just finished
44 doses of pelvic radiation and my second hormone-suppressing shot. Those shots wreak havoc on you. Next week
I go for a blood test to find out my PSA and my Testosterone levels. I am certainly a little nervous but what can you do. I sympathize

Hi Neil , I was diagnosed in 2009 with locally advanced PC . . I am now 65 . My PSA was 28.2 but was doing a lot of cycling around this time so that may have affected it . I had
Hormone therapy ( Decapeptyle😪 ) in order to shrink the Prostate for 3 months before I was give 15 sessions of Radiotherapy . Looking back at my PC Diary over the 10 years after treatment with a PSA test every 6 months , they ranged from 0.1 to 0.5 , the last one being on 21/06/19 which was 0.2 . ✌

I had a prostatectomy November 2022. After 8 weeks PSA was 0.12 so watch and wait. Stayed there until August 2023 when it had doubled to 0.24. Then had PSMA Pet scan November 2023 which picked up cancer in the lymph nodes. Started on Hormone therapy (Bicalutamide for one month with Prostap injections for next 2 years) and have to have 33 rounds of Radiation later on. Seeing Oncologist tomorrow to find out his plan going forward. My big problem is that I am still incontinent after 15 months and several sessions with Pelvic Floor Therapist and I must say I was making progress, but these new drugs seem to have knocked me back to square one. Possibly muscle weakness despite exercises. Don't know what will happen with the radiation as I understand a full bladder is required, but I can't do that. I hope for the best for you. Keep at it, the cure seems worse than the disease at present, with the stress of wondering but in the end I am sure all will be well.

You are doing the right thing paying attention to your condishion and trying to keep on top of it. I am coming to you from a 12 years journey of cancer and I was found to have double kidney cancer in 2021. I had two partical nephrectomies spaced 6 months apart and another operation on my right kidney in September. Please note that I had at least 5 or so MRI scans and at least 2 CT but it wasn't until they did an MRI with contrast in Mid summer that another tumor could be seen, to which I had Cryoablation which seams to have do the trick for the moment, but I have been told that I will likely have to live with cancer for the rest of my life, and seeing as I am now down to 2 half kidneys plus a cyst on my left one, I am much like yourself not in a really ideal position to be in. I don't know if they have issued you with a cancer nurse but I would be asking for follow up blood tests, you should be able to ask your GP for the forms and go into the hospital or Health Centre to do them, I don't see why you should be paying for them when you really need to push your concerns to your GP and consultant. Time is critical in terms of getting an out come that is good for your future, all I can say to you is you are not alone, but please please push ...its your life and your body and you are always intitled to change your GP if you have trust problems or seak another opinion. I wish you well best wishes Owen.

@@neildavidson7768 I was going to say that Macmillan are very good to talk to and very helpful. It sounds by what you say a s have been told that you still have some rumor left. I don't know that spending money out for another blood test is going to be helpful at this time apart from proving the fact that you sell have growing cancer tissue within you. I think pressing them on speeding up the treatment and maybe an ultrasound or MRI with contrast might be helpful if it's visible. I understand your worry and stress but you need to focus on what can and needs to be done. I don't know how your health Service is where you live but I am very lucky to be close to Southampton General. All I can say really is good luck but keep pushing it's your body and your life. Cancer is an insidious disease which can't be taken lightly or off hand, usually it is very time sensitive and only early and efficient treatment Leeds to a good outcome. You know your body and how you feel/feeling. I had a chat with my consultant before he cut away two halves of my kidneys what I wanted and when. He came up with a plan of how it would be done but this is the sort of reasoning that I think might help you. Best wishes Owen

Morning Owen again thanks for the comments. I do agree that spending a lot of money isn't the way to go. But the company I'm using charges £25 compared to £125 Spire wants. I don't mind spending £100 on 4 tests in the upcoming months. It will tell me where it's going. I'm still a little annoyed my local hospital haven't replied with some questions I posed to them a month ago. The secretary even said she would get back to me within the week. I just like to prepare myself if the next stage will happen so I can research everything on TH-cam etc. I don't owe the NHS nothing so will keep pushing them for answers etc. Again sorry for getting you up early mate and thanks again.

Hi Neal, just ran across your channel. Don't despair. Here in USA, radiation therapy is recommended if PSA reaches 0.2 ng/ml. Treatment at this PSA level has shown survival advantage over waiting for PSA to rise further. PSMA PET CT may not show a lesion with very low PSA but should be done. Also, agent used for PSMA imaging is important, Piflulastat F18 has excellent imaging

Please do not consider using a variety of labs for your blood tests if you are looking for reassurance. If your PSA rises to 0.2, you can be reasonably confident you have a biochemical recurrence. Your consultant can record the PSA doubling time and consider hormone therapy and a PSMA scan at an approximate time. My PSA went from 0.03 after surgery to 0.4 after 10 months. I was prescribed bicalutamide, had a PSMA scan and eventually had radiotherapy to my prostate bed and lymph nodes. I stopped the hormone therapy after 12 months and PSA has been undetectable since July 2022 when I stopped the hormone therapy. Recurrence after prostatectomy occurs frequently but PCa can still be “cured” after recurrence which makes it significantly different to other types of cancer. Just stick with the normal blood test and monitor the doubling time as this is the most important factor. Recording to decimal points is not helpful in this respect. I know it is challenging but if PSA scores rise, they rise and there is nothing you can do about it apart from monitoring the rate of increase and planning treatment accordingly. Hope this is helpful!

Alan, if I were you and I know I'm not I would consider intravenous high dose vitamin C several times a week, like 2-300 grams. Myself I am fighting heart disease on the Linus Pauling Heart Protocol with 30 grams of ascorbic acid powder in divided doses per day with Lysine and Proline and it's reversing. One intravenous treatment I'm told can kill up to 50% of the cancer cells in your body. Linus Pauling had prostate cancer and died from it but he said with high dose vitamin C, he felt it gave him an additional 20 years of life.
I would suggest Dr. Thomas Levy's book "Curing the Incurable " with over 1200 medical references. It just may surprise you. Information that has been withheld from the public. I also drink Hydrogen infused water. Cancer hates Hydrogen!

Sorry to hear that Neil. K Here. Get a PSMA Pet scan it’s a full body scan showing where cancer may be in the body.
You are correct with the psa test, you need to have a more accurate test, think of it as an investment. In Australia all tests are 0.00 as far as I know. I am running at .02 at the moment after the removal it went up to 0.03 they wanted me to have radiation.
Then it went down to 0.02 they are over servicing a little here I feel. Stay strong stay informed.

I had a biopsy 10 years ago saying I had prostate cancer. I chose not to treat it. I did go on a Flowmax and Cialis combo to increase my urine flow and blood circulation in the prostate. It has seemed to keep me in a current state of not spreading. The extra blood flow and more oxygen to the area possibly is the reason it hasn’t spread. I get annual mri and pet scans. My current age is 79. My PSA has always been high even before the biopsy. I believe taking the combo meds and watching might be a better idea. Most people panic or their wives panic and decisions to go all out treatments might do more harm. Just saying. Plus a positive attitude might be a alternative.

At your age, that was absolutely the right call. A treatment plan for a 75-yr-old versus a 60-yr-old needs to take into account quality of life and expected years remaining.

Keep in mind that prostatectomy often leaves a small amount of normal prostate tissue behind. Might be from bladder neck sparing at the anastomosis site, or bilateral nerve sparing, or a wide surgical margin. It doesn’t mean this tissue is cancerous. Any such tissue can generate PSA, and as your testosterone recovers, the amount of PSA such tissue generates will increase in proportion. It should level off when your testosterone level stabilizes. If PSA continues to higher and higher levels in the next year or two, then you likely have a recurrence and need a PSMA-PET scan to find out where it is so it can be treated. Best wishes that your PSA will stay constant so you won’t have to fret about it.

In 2013, I had prostate cancer with half of my prostate cancerous. i decided to go with robotic prostate removal, and selected a doctor with experience of over 250 operations of that type. Other than, the discomfort for a week to ten days and operation recovery, I can report the operation was a success.

All the best. I had mine out 3 weeks ago and am celebrating small steps at the moment. Eg I lasted all night without a nappy for the first time last night !

salvage radiotherapy if appropriate." Hope for good outcome for both of us. Best wishes.

Had my Prostrate removed in 2015. Had a 4 + 3 for a Gleason score. Shortly afterwards, PSA started rising. Make a long story short, had to have radiation treatments. Right now I am having hormone therapy. The cancer seems to be a hard thing to get rid of 100%
Best advice would be, is to relax. The cancer is fairly slow growing. Your next step would most likely be the radiation if the PSA keeps rising.
    Take care on your journey.

Hi. Sorry to hear your PSA is rising slightly. Not the best of news. The good thing is that there will be plenty of treatment options if there is a recurrence. I believe the general course of action if your PSA rises after the op is hormone treatment and radiotherapy. Not the greatest things to have to go through but they can knock the remaining cancer on the head for good. And even beyond that there are more treatments. My husband had a radical prostatectomy in Aug and his PSA was undetected at the six week check. Next test in Feb.. Fingers crossed your PSA stops rising or rises very slowly. Take care. Mags

Surgery in general is fraught with problems. I found a way to go all the way from from 7 or 8 trips a night to only 2 without any procedures and without any prescription drugs. What worked for me was a daily combination of beta-sitosterol, cortisol suppressant chemicals (Cortisolv was the brand used), pygeum, moringa powder, and reishi mushroom. I got mine stopped early before there was any evidence that enlarged prostate had morphed into cancer. Now most of the symptoms of benign prostate enlargement are missing but I have to keep taking this cocktail to keep it where it is now.

At the time aged 56, Gleeson Score 5+4. It doesn’t get any worse unless metastasis. Radical prostatectomy using robotic surgery (Da Vinci Surgical System). Removal of seminal vesicle and lymph node. No incontinence. I think I know what I am talking about. You are in danger of becoming self obsessed and focused on a doomsday scenario. PSA is not an exact science. Score is low. Any negative development would be treatable. Shopping around for PSA tests is the worst thing you could do. Trust your consultant. You are absolutely right to get the 2dp test. There is a study that shows that salvage therapy started before the psaa reaches 0.2 gives a 10% better outcome. Waiting till it reaches 0.4 is not good. Dont rely on the surgeons view. Talk to an oncologist. The idea of wairing till 0.4 comes from the desire to see the location on pet psma scan. But by this point it could be too late. This is what my oncologist told me (im stage T3a).

In addition to some prostate tissue perhaps left during surgery to save bladder function etc., perhaps the biopsy could have spread normal and/or cancerous prostate tissue along biopsy tracks? In this case 3+4, in 90% of cases it never goes from the prostate to the surrounding tissues. New types of prostate treatment are coming. I still don't understand the rush to tear an important organ from the body.

It's natural to want to fixate on early post-op PSA numbers, but it's data OVER TIME that tells the story. 0.1 versus 0.14 says almost nothing. Trendlines become apparent over months, although I get your desire to "know" right now.

Off topic . are doctors to quick to remove ? Do not know or how to react what was told by doctor recent . he said your 75 prostate slow growing that takes 10 years . That would make you 85 and if worth doing anything if cancer detected . This was before digital exam where he found nothing thankfully. However would write script for slow flow urine . Asked him about rezume as alternative to drug . Would not even talk because told me insurance would not allow until took the drug first .

First - a digital exam is useless. I don’t know anyone who has had prostate cancer detected by a digital exam. If your Gleason scores are low you might be right that treatment is not needed. But don’t make that decision based on a digital exam.

Unfortunately, that rules out the NHS.
I can sympathise with your position, but an independent review of your case might be very necessary.
      I wish you luck.

    Hi Neil,
I live in South Africa. The sun is always shining here and there is always the risk of developing skin cancer if you have a light skin. I developed a growth on the lower eye lid of my right eye. I do not know if it is cancer or just a benign growth but I went on a high nutrition diet for about two months and the growth have shrunk noticeably.
I would like to share the diet with you.
    The soup
Ingredients: - two handfuls of Chopped broccoli, one chopped onion (purple), two bags of green tea, two pinches of mixed herbs, two sticks of cut celery, Two handfuls of frozen and crushed spinach or kale, half a table spoon of crushed garlic, Three shakes of black pepper, seven shakes of turmeric, two tablespoons full of apple cider vinegar (with the mother), one bag of white onion powder soup for flavour, a hand full of cooked red speckled beans and a handful of cooked mince meat.
Just add all the ingredients in a pot, add boiling water and switch the stove on high till the water boils. Then lower the heat until the water simmer, Let the soup simmer for an hour. The soup is now ready.
I would only drink water in the morning. At 12h00 I would start eating the soup. There will be about two helpings that you can eat in the 12h00 to 18h00 time frame. I also ate a fruit salad every day in the 12h00 to 18h00 time frame.
(Note: Apparently turmeric reduces the effect of some chemotherapy drugs and thus should not be taken during chemotherapy.)
During the rest of the week I would also eat food that contains vitamin D like avocado’s, mushrooms, liver and eggs in the 12h00 to 18h00 time frame.
It seems to be working for me, maybe it will also work for you.
I pray to God the Father in the name of Jesus Christ that you will be healed completely and that the cancer will not return. Amen
    Best regards
    Chris

@@neildavidson7768 I had a RP in 2021 and PSA undetectable since. But I stay abreast of the next step to take in the event of a recurrence. If PSA rises to 0.2ng/ml then I would likely begin ADT with salvage radiation treatment with the hope of killing off any remaining localised cancer cells (in the pelvic area) before any distant spread and metastasis. However, before pulling the trigger I would get a PSMA PET scan at 0.2ng/ml to try and visualise and localise any detectable tumours because if it has already metastasized then there is no point in whole pelvic radiation. It then becomes a case of managing/containing stage 4 PCa as opposed to treatment with curative intent.
