Character vs. Nature, I think. The movie _Wit_ is a good example. On one hand, it's a sort of survival story with cancer as the antagonist. But like most good stories, there is also an important internal struggle, which is character vs. self.
I wanted to leave room for the probability that narrative conflict can't really be reduced to these six categories. In fact, when I was young, I was taught only five. Wikipedia only lists four. There's room for opinion.
