    4 years after this video was made and my doctor (who professes to study all the time) still tells me low-fat, cut out red meat, and exercise.... these people AREN'T learning.

    Social pressure is easy to deal with. 70 years old, down 100 lbs, off all metabolic meds, A1C is 6.1 and going down. I am omead, carnivore. "Thank you, but I am not going to mess that up by eating something or not on my schedule. Please understand. ". I still need to drop 20 to 30 lbs. if they do harass me, which they do not, they know I will tell them where to stick it. My health comes first.

I am already applying your advice regarding the consumption of fats, butter, sardines, etc. and already after three weeks I see the beginning of the change for the better. 🤩

    You should really try to get some courses registered for CE's. I'm a newly licensed Chiropractor and am very interested in learning more of this and in much more detail. I'm sure many other providers like MDs, DOs, nutritionists, dietitians, PTs, and nurses would agree. This is incredible information that is not taught in schools at this point.

    found you a few months ago. Have been ketovore for several years. Lost 70 lbs, off metformin, but still insulin resistant. This has been very helpful for me. Better late than never. I am 76 yo female. Many years of undiagnosed insulin resistance and metabolic issues.

    Missed my fast this week. Wow, what a difference! My body feels very sluggish. Lesson learned - I will be back into my once weekly 40+ hr fast per week. Wishing you continued success.

    Its been a while still keto from February...BUT just got my blood work 2 weeks ago:) Thank you Doctor! My A1c went from 9.4 to...ready wait for it... 5.4:) Have done 3 or 4 intermittent fasting (longest 34 hrs) and that really clears the mind and invigorates the body:) Still hoping for your new book WITH ten meal recipes (what we used to call breakfast,, lunch and dinner) that you personally designed for nutritional health.     Read your book in Kindle format. Then last week I downloaded the audible edition after finishing Keto Continuum. I love listening to your voice as I go to sleep. I'm a chronic/acute insomniac and listening to you is the first thing that has helped me fall asleep in many years. I do have to keep backing up a chapter in case I dose off. I've been keto off and on for a year now and just added IF to the mix. 14 pounds gone in 5 weeks!

    Thank you so much Dr. Boz for being so generous with your presentation. It is an added information to me☺. I've been doing OMAD and water fast also in fact I suffered T2D for more or less 30 years with HBA1C of 15.8 (on Aug.23, 2018) with medication of Galvusmate 50/1000 mg and Actos 15mg. My Doctor told me to inject insulin, but I did water fast instead 5-7 days each month and consuming raw vegies, nuts, Virgin coconut oil, on my I.F. or OMAD days with HIIT exercise. After 3 months my HBA1C went down to 8.3 ( Aug. 25, 2018) and after another 3 months my HBA1C =5.4 (Dec 01, 2018). I am still doing OMAD and 24-48 hours (Friday & Wednesday) fasting each week with acceptable HBA1C results without medications☺. I am so interested with the Keto test machine hope I can find it here in Saudi Arabia☺.May God bless you Dr. Boz.

    Hi Dr Boz , don't forget that people with high insulin resistance, when they are in ketosis for a long time their cells become so used to ketones (especially muscle cells) that they have no incentive to improve their insulin resistance. To solve this it is advisable to have about 2 days with about 150g of carbs a day, then go back to ketosis for two weeks. During every transition the insulin resistance improves. Please try it and you will be amazed. Kind Regards.

    Jerry Taylor, 77yrs old, diabetic 40 years, your Jerry needs to clean up his diet' My DBR stays below 40. Only check weight once a month. Started 2023 in the hospital (CHF, the flue) >310lb. I am down to 269lb. My Ketones down right 2.8, ranges up to 7+. I am a new carnivore (5 weeks). Started with 72 hour fasts first two weeks, did a 7 day fast ended a week ago. Just completed 72 hour with sardines. My main concern is damage from years of really bad diet.

    I have also found many of your videos extremely helpful. Especially when trying to achieve low fasting glucose levels, and higher ketone levels.
    I am trying to help my wife achieve this in relation to her pre diabetes and Alzheimer's disease.
    Her fasting glucose (after 40hrs), this morning was 73.8, and ketones 0.6. Whereas mine were 63 and 2.6., giving me a Dr Boz ratio of 24.2.
    It's obvious that our metabolisms are still different.
    I think that it is due to her remaining slight hyperinsulinaemia and the amount of fat that is still being stored in her liver.
    Her weight is coming down slowly, but she needs to reduce it by another 7lbs.
    I have also found your book inspirational, and am extremely grateful for the amount of time and energy that you give us in these videos.

    Apparently, I'm a bit late to this party. I eat 100% Lion Diet on Monday, Wednesday and Friday.
    I fast in-between. Out of 168 hours a week, I fast (but drink distilled water spiked with Redmond Real salt) 162. I chew my cows thoroughly (32+ chews per bite) and make my meals last 2 hours.
    My results are nothing short of AMAZING !

      @dantheman9219 Originally, I was OMAD but of late, I've switched that up to 2MAD. The reason for that is probably not valid. I think it stems from a 70+ year addiction to carbs. I "can" do OMAD, but I "feel hungry" at 1 pm and at 10 pm. Eating late isn't an issue, as I often can't fall asleep (too much energy) until around 4 am.

I struggle with blood sugars around 100 to 118 in the morning but now I understand why, after watching your videos. I need to fast longer to get it down into normal ranges.     Greetings Fellow Warriors into Great Health and THANK YOU, Dr Boz!
    Started with IF on 5jul2023 by skipping Breakfast and now doing OMAD.
    Been struggling with both high & low blood glucose for the last 1.5 year since my heart bypass. Those times below 3.9 mmol/L or even below 2.9!
    I had about 4 occasions since started IF. Twice during sleeping at night; praise God He always wakes me up without fail😊.
    I would like to progress to OMEOD and ultimately to 3 or even 5 days fasting as I have developed a few Diabetic Complications like Retinopathy, Neuropathy and currently in stage 3a of kidney disease.
    The last time I tried to push my Fasting to 36h, I was cut short due to low blood sugar.
    If I understand correctly, Keto in a Tub should help me here; or should I be looking at something else.

    I do the keto strips you pee on. The darkest color is after 18 hours of fasting. I did not lose hardly any weight for several months until I went to eating between 12 and 6pm, added MCT oil, keep the total carbs well below 20 and I started losing weight. 13 pounds in almost 2 months. At 68 I think this is good.

      Hi @@sharmilakarki2875 from Nepal! Its an automatic process that accompanies nutrient deficiency.
      In fact, I was in Nepal for the first time I conducted a guided fast and stomach cleanse yoga.
      Basically, when you stop giving your body food, it will begin to eat itself, starting with parts of the body that are considered "trash" such as the clump of misfolded proteins and the plaque that grows in your brain.
      If you would like to immediate benefits of autophagy, then do a 48 to 72 hour fast drinking only water. Make sure you speak to a doctor about this.
      However, the more approachable method is intermittent fasting. For example, you eat in an 8 or 6 hour window, and give your body 16 or 18 hours of rest or "fasting." Studies have shown that even 12 hours of fasting is enough to begin the autophagic processes. This is much easier to sustain longterm.

    Why, when I OMAD KETO and eat around 4-5pm, I can do it. But if I eat OMAD KETO at 6-7 when I first get up-I want to eat all day long and usually end up eating again.

    Jerry needs to walk 30 minutes a day. I just started and found it really lowers my glucose reading. We went for cheeseburgers and onion rings two weeks ago. Two hours later my glucose reading was 169. EEK! I went out for my first walk in several years. Went just 24 minutes, four trips around the condo complex, glucose reading dropped to 101. Try it.

    Hello Dr. Boz, love your passion for teaching, the subject matter that you know so well and you’re a great storyteller, a breath of fresh air I am 65 and I am taking a cold shower every morning cycling one minute of hard, one minute of cold, including in the winter, which I’ve slowly cultivated over the years doing this every day seven days a week is it too much? I also fast 72 hours once a month. I’m also an athlete and I strength train every day with cardio twice a week swimming laps

The Lipedema Project Has been doing some amazing research on keto and it's effects on lipedema and this video got me wondering if using the Boz ratio would have an impact

      Janna Koenig Dr. Boz® RATIO: Glucose divided by ketones.
      This math was easier for my mom to do ... and for my patients.
      Dr. Boz® RATIO LESS THAN 80 [ comparable to a GKI of 4:1] = This is the weight loss zone. Some autophagy may be happening.
      Dr. Boz® RATIO LESS THAN 40 [ comparable to a GKI of 2:1] = This zone has strong benefits with deep tissue/organ inflammation and auto-immune problems. (Leaky gut, brain fog, Crohn's, Rheumatoid Arthritis, Hashimoto's) More autophagy should be happening.
      Dr. Boz® RATIO LESS THAN 20 [ comparable to a GKI of 1:1] = This is the zone I use for patients with Cancer & extreme health issues. This was my mom's category. This zone is really hard. No playing around. Make sure your doctor is watching you closely. Almost all that reach this zone are likely to have autophagy at this stage.

    the ketones administration is particularly beneficial for those with a high glucose level, because supplementing ketones lowers the glucose levels. However focusing on lowering insulin is way more important in those with diabetes2 and pre-diabetes.
    This is truly a beautiful info graph.
    I am only shocked that their insulin has not been monitored too. when that goes too little or too much it is the worst. Stacey definitely struggled from hyper insulin, feeling she needs to snack all the time. furthermore if she is a caffeine fan she can snap back to insulin resistance when drinking coffee on eating days so it reverses her fasting benefits to none. Caffeine reduces insulin sensitivity. it helps loosing weight and speeds the digestion but it is counter effective for true regeneration ( autophagy) and stabilising insulin and sugar in a long run- so you may lose weight but your pancreatic insulin release won't get stabilised that well and cells sensitivity to insulin will not be boosted. Therefore every time when a caffeine user starts eating again there will be a relapse to pre diabetes state, over producing insulin and inability to lose weight efficiently, especially if not much of it is left- because body will be in a constant "save that calorie and store into fat" survival mode.
    When fasting and sugar stabilises and ketones are going up the insulin is definitely going down and stabilises. and that is good. steady low sugar and lowered stabilised insulin = ketones going up.
    Ketogenesis is regulated by insulin.
    Great that you pointed out the correlation of cortisol and sugar/insulin when waking up. Also cortisol and sugar and insulin go up when exercising and when stressing. it is important to exercise with the natural rhytm of rising cortisol - in the morning not at night which is a time when cortisol should be falling.
    Also I wanted to point out that autophagy is not that good when drinking caffeine, the ketones might be up but the used cell cleanup and cell renewal goes very much south. Tea and coffee even a decaf ones- truly negatively affect the autophagy, even though they may boost the digestion and ketogenesis.
    Water fast is really the best for the fast...Herbal caffein free infusions are ok but only on the eating days or strawberry or lemon infused water.. or apple cider vinegar water... and electrolyte sugar free water 1 h before omead meals.
    overall I am glad I found your channel, you are a truly wise and particularly focused on cause treating and seeing results doctor. Great video! Thanks! HAppy NEw YEar Dr.!

    Note to Sea Doc. Tachycardia relative to Keto. I too experienced this last year after switching from low carb to strict Keto. I was not managing my electrolytes well. Pulse jumped to 150 - 160 for the first time, over 2 hours, at ER once a saline IV was pumping pulse dropped to normal and stayed. Assumed dehydration, but Tachycardia came back about 24 hours later. ER Doc again, IV didn't help. Doc told me to go to see Electrophysiologist, recommended a name. My GP the following day knew the recommended name and sent me to him. Long story short, Medication to regulate heart, multiple tests showed healthy heart and arteries. Surgery scheduled for "Ablation" run up through arteries, and cauterize specific extra pathways in heart that are "misfiring". Fixed the problem, 3 more months on meds and now 9 months later, I am back on full Keto with no problems, no meds, and normal heartrate. All the best to you. I think that Keto does somehow trigger pending Tachycardia when it is already likely, but not a primary cause. I have heard several others that had a similar experience to me.

      She didn’t answer in the video but typically standard American diets of 100-300g carb/day means it’ll take 3 days whereas keto diet + fasting > as little as 12 hours - 36 hours. Also her ratio which indicates what stage of autophagy you’re at is based on glucose / ketones, so more ketones + lower blood glucose would mean a lower score which is better

      First succeed at 48 hours. Prove to me he can handle that. Keep ketones high to protect for muscle loss. If that succeeds... then we can talk.
      He did really well at 48 hours. Why push it .. success matters

    When you are using exogenous ketones and looking at your numbers. Are the numbers completely accurate since your body didnt create those ketones that you just measured? Doesn this give you a false sense of how your body is going?

      Yes... and no. The absolute values can be misleading but the trends, when you take into account when and how much of the BHB you have ingested still tell you what is happening. But the important point is that BHB "primes the pump" by helping to start your body to actually use ketones.

      Brandy A, no. You are correct. Readings should be taken a good while after consuming ketones. I like to wait about 20 hrs & usually more before I take ketone reading. 2 days after taking is more accurate imo.

    Doc, I just finished any way you can. Great book! I have a question about this diet. I just started the c8,c10 mct oil . I'm wondering how do I stop the explosive diarrhea when I eat high fat. I thought I had it figured out eating beef,chicken, and staying away from cheese and eggs. I finished your book and tried eating half of roll of pork sausage. I want to increase ktones because I have my own low-grade b cell lymphoma. I have been carnivore/ keto for two years. I also bought your c8,c10 soft gells. Thank you for all the information on health.

      Are you sure that it isn’t the MCTs that are causing your problem? If I put just one drop (from an eye dropper bottle) in my morning coffee I get diarrhea…big time!

      @@HappybyChoice yes, to avoid diarrhea one needs to very gradually increase MCT oil to a tablespoon or so (over a couple of weeks or more - like 1/2 teaspoon/day for 5 days, then 1 teaspoon/day for 5 days, etc, on up to total of 1 Tablespoons/day and then keep it there daily. If stopping it, gotta start all over again.

    Please HELP @Dr Boz, or anyone more experience than I am, what about the slender, diabetic who has just found they have Stage 4 cancer in the sinuses and around Coratid arteries? I heard what you said about ketones building muscle mass, but they have for many years had Diabetes and are so slender, already! I heard they need 21 DAYS of fasting to interrupt cancer. They would, seemingly waste away in the meantime.

One thing I'm still so confused about: how LONG do you recommend we stay under these ratios - 80/40/20 - like, once we get to say, 40, what's the goal? to stay there for 24 more hours, or to just stop fasting...???       Kara Kelly - if my patients are fighting for their life .. I instruct them to stay under 20. For months. Usually this is not the intensity most of us could do. So I like people to hit it as often as they can. Personally I get there once a week ... that’s enough for me ... for this season of my life

    I dont understand what you mean when you say Jerry has a lot of stored glycogen. I thought that the liver can only hold about 1400 calories worth of glycogen. where else is he storing sugar?

    I’m from Florida and love your videos. I hate pricking my finger too! It feels like a bee sting. I have to put lancet on highest setting to get blood. After 3 wks on keto, I checked my blood glucose. It was at 113 for 3 times in a row... I thought the meter was broken! Finally after fasting for 16 hours it was at 97, but I felt shaky and dizzy. After 6 wks, I’ve lost 20 pounds and ketone urine strips always show moderate or higher ketones. (I check urine 4-5 times a day.) My average blood glucose is between 101-113. I still have a lot of inflammation, sore joints. This surprises me given other stories. 👉Any idea when the inflammation will improve?
