    As a neurosurgeon in the US, I would say this a great and very informative video. The only criticism I have is that you keep mentioning the surgeries are crude and less precise. I also did plastic surgery rotations as well as rotations in various other surgical subspecialties as a med student and I chose neurosurgery for the technical nuances of the surgeries. Micro vascular decompressions, skull base surgeries as you mentioned, surgical ligation of aneurysms, av fistulas, endoscopic surgeries, intramedullary spinal cord lesions, endovascular procedures (pipeline stents, selective embolizations, thrombectomies,)duroplastys, cerebral bypasses(ever see 10-mm prolene sutures used to anastamose vessels 2mm in size)? Even XLIF approaches for the spine with small working corridors are much more nuanced than most surgical procedures. DBS, stereoEEG placement (2-3mm off target can make a huge clinical difference in outcomes). Corpus callosotimes, selective amygdalohippocampectomies, functional hemispherectomties... the list goes on and on. The big difference in neurosurgery is that if you make a mistake, the stakes are much higher: you can cause a stroke leading to aphasia, plegia, blindness, coma and death. Hit the spinal cord during surgery and your patient will never walk again. The stakes are much higher. I’m not sure where you did your neurosurgery rotation but your exposure must have been very limited. to say it’s crude or imprecise in any way is an injustice to the field. Nonetheless I think your video is very informative and insightful. You guys did a great job. I will definitely recommend it to any medical student who is considering the field.

    • @MedSchoolInsiders
      @MedSchoolInsiders  4 ปีที่แล้ว +44

      I admittedly do not have more than a few weeks of exposure to NSGY. When I was rotating a NSGY resident and attending I was working with mentioned it was more crude than people expected, and not as precise and meticulous (depending on what type of case you’re doing).
      I’m all for differing opinions though, as long as my viewers get both sides to it. I like to think I also highlighted some of the more precise aspects in the video

    • @noh4241
      @noh4241 4 ปีที่แล้ว +30

      Med School Insiders - you did a fantastic job highlighting the field. I am 100% recommending any medical student interested in neurosurgery to your video. That was the only critique I had. You pretty much nailed everything else to the tee and I commend you guys for that. You did a great job highlighting the subspecialties as well, I just feel that there are many procedures that are very nuanced and technically difficult. Perhaps I see them more then most general neurosurgeons being at the institution I’m currently at, but I do feel that our field is very technical and a lot of it has to do with the delicate “real estate” of the anatomy that we’re dealing with. Nonetheless I did not mean to come off as critical when my intentions were constructive at heart. Most people do not understand our hours, call schedule etc and as I said before, you nailed it with flying colors and you also described it from a layman’s perspective which I feel is extremely helpful for anyone who is or may be interested in the field in the future. You did a great job and I commend you for it!

    During _high_ _school,_ I was shadowing some neurosurgeons because one of the physicians there was a family friend. One surgeon was notorious for being the perfect stereotype of the arrogant, large-and-in-charge hardass who was simply better than you… and he’d make certain you knew it. So I’m standing a few feet away from him fully scrubbed in the OR, and he starts rattling off some polite questions, ignoring most of what I said in response. Clearly he missed the part when I told him I was a _high_ _school_ student lol. So eventually, once he reached whatever part of the spine he was operating on, he suddenly starts launching questions at me about god-only-knows what branch of whatever T3 nerve etc etc… and after like the fifth “I don’t know,” he literally says “Jesus, the fuck do you know?” Luckily I have a thick skin and was mostly laughing at the ridiculousness in my head lol, but at that point, all the scrub nurses immediately launched at him like a pack of hyenas, explaining to him (again) that I was just a junior in HS, not a medical student lol. As per usual, the scrub nurses saved the day.

      melina ugh while that’s true to a degree, trauma surgeons have to think faster than a lot of specialities, especially ones who almost always have scheduled cases. with trauma, you go from not knowing what you’re working with to operating in minutes. that’s usually very different than specialities such as general or peds. while those specialities are very difficult in their own respects, they aren’t all about quick, high-stake thinking, most of the time.

    I remember watching this video 2 years ago when I was a MS4, now I'm a neurosurgery resident! Hope to do a cerebrovascular fellowship afterwards.
    Here some tips to whoever is interested:
    1) If you're a high schooler, go to the best possible college you can. This will help you get into an elite medical school
    2) A lot of neurosurgeons come from top 20 medical schools, work hard in college and get into a great med School. Becoming a neurosurgeon will be significantly harder if you go DO or lower tier MD.
    3) You need to do well in medical school, but by that point the pressure isn't as high. You don't need to be 1st in your class, but ace the Step 1/Step 2. Get involved with neurosurgical research.
    4) Don't force a circular peg into a square hole, surgery isn't for most and you'll realize that by the time you're doing rotations as a MS3.

    I recommend the memoir Brain Matters: Adventures of a Brain Surgeon by Katrina Firlik for an insiders view on the field of neurosurgery.

    I love the brain so much, but I would be so unhappy with the unpredictable and long hours. It breaks my heart though because neurosurgery (particularly emergent and functional) is absolutely fascinating to me

    I'm just in a nurse who works in neurosurgery who is watching this to satisfy their curiosity of the field but this is for sure the medical field I have respected the most and feel they do the most for their patients. Where I work has quite a few female residents as well. Probably won't do as much as to actually go for it though 😅. I will keep to nursing I think.
