The surgical criteria is organ/system specific and so relative. Of course there are some conditions which cannot be treated without surgery like certain emergency pathologies, (i.e. appendicitis, it has previously been treated conservatively [with antibiotics] resulting in high rates of relapse), there are other conditions like certain spine pathologies which not necessarily need surgical treatment as Dr Webb points, in medicine decisions are based on a mix between strong evidence (which in turn results in the making of scales and decision orienting algorithms) and the own medical criterion of each doctor and specialist based on professional experience (empiricism).
Certainly there are unethical professionals in medicine as in any profession and job, but generally decision making in medicine is much more complex than non-healthcare professional population can fully understand and thus they should avoid approving or deprecating the medical criteria. When in doubt and health is not acutely compromised, one can always reach out for different medical opinions.

Both my neurosurgeons were cautious about surgery. First said put it off as long as you can and exercise to keep your core tight, holding the broken bones in place as best you can. Second met with me 3 times to go over risks and possible outcomes before I booked the surgery, that was 8.5 years later.
I'm in much less pain now and have better range of motion than I have for 2 years.

Yeah, it is pretty obvious that this is just temporary relief. It sure works, but without muscle training it won't have any lasting effect. People having back problems can truly benefit from a tailored muscle training plan by a professional trainer, and sleeping on their back on a firm bed. I know that it can be quite painful and very hard for such people to sleep on their back on a firm bed, and even more painful to push on with muscle training, but sometimes you need to have perseverence to fix a problem. These 'adjustments' aren't really a solution. And yeah, where and how you sleep is very very important. You spend 1/3rd of your life on a bed, it has tremendous effect on your posture and muscles, so you better make it count.

Nuts - there is plenty of ego in his presentations. The fact he is on TH-cam says a lot in itself. Do not mistake a soft sell, and false modesty, as a proof of medical competence. I sought multiple second opinions, and believe, on balance, he is gravely mistaken. The fact he comes off as a nice guy (he really does) does not mean he is medically correct or competent. As for ego, make no mistake - he holds represents himself as an expert we should trust - and there is plenty of ego (as well as confidence) in that! Truly modest doctors do not have TH-cam channels to promote their business. At least that is just MY take. Your mileage may vary. TT

I am also a spine surgeon. The statement "surgery is always a last resort" is fundamentally wrong.
After analysing thousands of cases worldwide and discussing in hundreds of fora, analysing publications & seeking expert opinions, we formulate broad guidelines for treating various spine conditions scientifically.
Some conditions merit definitive surgery, for some there are relative indications(need) and for others, operative procedures are not recommended. And the guidelines are always flexible and open to correction to account for individual patient's needs and variations.
So there is no question of "last resort"
Anybody can talk. But to do real and proper spine surgery by following proven guidelines, it takes years of hard training and extensive surgical exposure

But they won't do this. (I had many adjustments and it did absolutely nothing for my high blood pressure, complete and utter waste of money and also did nothing for my lower back pain) Chiropractors are on par with massage therapists, that is all, maybe it is relaxing to some people similar to a massage, nothing more. I opted for the ' no ' surgery and after many decades of limping through life I got a thorough adjustment by a man that was not even professional chiropctor.

A Y-strap caused my mom to have a subarachnoid hemorrhage 6 hours after she got it done. It was a very well known and successful chiropractor. She almost didn’t make it. 1/3 of people with subarachnoid hemorrhages die, another 1/3 will survive with permanent brain damage, and the last 1/3 will survive with little to some brain activity damage. This type of brain bleed has occurred many times to patients after a Y strap or neck adjustment. Please, be careful and consider carefully what risks you’re willing to take.
Edit: I didn’t expect all the responses to my comment, but I’m glad the info is reaching people! My mom made a full physical recovery (she’s only 46) but has lasting brain damage with effects to motor control, memory, and balance. Around 30-40% of people with this type of brain hemorrhage die. She was in the hospital for months when this happened and my family and I were too focused on caring for her instead of starting a lawsuit - my dad requested a refund for what she paid that day, and told the chiropractor what happened. The chiropractor did feel bad, but said he knew it was a risk (though a small percentage) and knew others that had brain bleeds after neck adjustments. If there’s any message to take away from this situation, I’m not saying to ban chiropractic practices entirely if it’s helpful to you, but please do not ever let someone manipulate your neck or put you in a position that compromises your spinal cord.

I have watched a series following one gentleman through his recovery. He has had a lot of hard knocks in his life, which led him to being unable to move his head even slightly for something like 20 years. He was in a very bad way, but his chiropractor (the one in the video with natural wood baseboards, white painted brick and right hand door), was very careful with him. He spent about half of the session attacking his inflammation with a massage gun. The other half was more traditional adjustments. He only used the y strap 2-3 times and is constantly checking up on them and how they feel. His TH-cam videos also allow him to offer his services for free to those who needed them the most. the way he moves around the patient, handles his tools and talks to the patient, he clearly knows his craft and cares deeply about the well being of the patient. My point is, he has done a world of good for his patients, and the y strap is another tool for helping patients. It may not be right foe some as you say, but others, it can do a world of good.

As a chiropractor for over 28 years I've used many different techniques to adjust the spine. I don't use this one because I think it's too harsh and is by no means "gentle". By the way, we call them "adjustments" because the word "manipulation" has too much of a negative connotation. I do applaud this doctor's objectivity. He's far more open minded than most.

    *Disclaimer- Chiropractor here
This Doc is an incredibly well balanced observer. If I ever required a back surgery, he would be the guy I choose. You are a credit to your profession and yourself. Please keep doing the good work. Your knowledge and expertise are above and beyond.
As a chiropractor, I get asked weekly on whether I perform the Y strap adjustment from patients who have been spending a little too much time on TikTok and youtube. My answer is always: "I wouldn't perform it on my Mum or my wife, put it that way". There are several reasons for this:
    1) I believe it is excessively forceful. It is not a high velocity, low amplitude adjustment. the stress it places on the cervical tissues in my opinion are too great.
    2) This adjustment is not specific. An adjustment should be very controlled. It should be targeted to the specific joint requiring treatment, with a specific amplitude and velocity. This adjustment is a hail mary. You have zero control over what is actually adjusted and by how much. This would be the equivalent of Dr Webb choosing a segment to operate on by Eenie Meenie, Miney Mo.
    3) I agree with the Doc here- it is a viral trend at most, and should not be common place in any DC's toolkit of techniques.
Yes, there is no evidence to support this, but here's my opinion on research and manual therapy (including chiro, physio, massage etc).
4a) It is near on impossible to deliver placebo manual therapy. I can't give a sham adjustment, or sham soft tissue therapy without the patient and myself knowing. This makes a triple blinded
RCT not possible. As a result, our quality of evidence will always be low.
4b) Funding. The government won't fund manual therapy research- and lets face it, there is far more relevant research to be done- cancers, diabetes, heart disease, covid etc. This is where the
time and $ should go anyway. There are no corporation i.e drug companies who will fund the research. When a pharma company puts a drug to trial, and it is a success, they make a lot of
money selling said drug. They have a product at the end that returns profit. These companies have nothing to sell with manual therapy- it is not an investment for them. They will not
benefit in any way, shape or form. The only people who would fund research, is the manual therapists themselves- and this just doesn't get done, likely because it is very difficult to study
and not financially rewarding. i.e wage in private practice would be 4x+ that of a researcher. And lets face it, we become practitioners to help people, not sit in a lab. As a result, the
quantity of evidence will always be low.
I find gentle cervical traction using a towel (slow and steady, no manipulation) gives great relief in a non-invasive manner.

All the patients in these videos looked young and healthy with relatively mobile necks so i feel there were probably safer alternative adjustments that could have been used. I feel this kind of video (meaning the Y adjustment videos, not Dr webbs video) on the internet are just for the chiropractors own benefit/marketing/ego and have less to do with the patients health. I should add i haven't seen any research on the efficacy or safety of this adjustment nor have i spoken to any patients or chiropractors who have received/used this adjustment who knows, maybe its gold for some neck conditions.

I have some issues with my connective tissues (autoimmune) and my joints tend to displace easier than your average patient. I see my chiropractor weekly and we use a traction manipulation only when I start to have numbing and tingling in my extremities. Traction works every time for that specific issue, but my chiropractor doesn't recommend it to every patient, nor do we do it more than once every couple of months because he feels it may be too aggressive on my connective tissues (obviously that reasoning is case specific).
I am one of the lucky ones in the USA where my chiropractor and MD actually work hand in hand to treat me and each other respects the other professional. My chiropractor has actually caught on to more autoimmune flare-ups/attacks than my MD has (probably because he has had the ability to see me more often). I am grateful to both practices.

Why did you choose to be a chiropractor? Are you aware that its a form of alternative medicine? (AKA medicine that wasn't proved to work, AKA not real medicine)
If so, are you aware that chiropractic practice started as a way to connect the human soul to god (based in vitalism)?
If you're a chiropractor who does so because you want to help others, then I really suggest looking into Myles Power's TH-cam channel, and educate yourself about the real dangers of the practice.
If all you really wanna do is help people, then you'll have no problem dropping the shithole that is chiropractice and going into a scientifically proved medical field (AKA real medicine).
If you only do so because you want to see yourself as someone with "hideen knowledge" or something (if it's just an ego thing) and you're OK with this dangerous practice, then the only thing I can tell you is to check whether you have any sociopathic tendencies (although I want to believe that the majority of chiropractors are just well meaning uninformed people who were tricked into this cult).
Sorry if I'm sounding mean, it's just that I have family members who were tricked into believing that chiropractice is real which just ticks me the wrong way.

If I'm able to help some in a more GENTLE way, I will always choose a safer and more gentle method. I have found the stuff you see on TH-cam is more for entertainment purposes, rather than realistic for the average patient. I've been in practice for 24 years and regularly get referrals from the medical community because they know I will "not rip their head off" as one medical doctor said to me. Stop trying to scare people. I guarantee you that any procedure you ever preformed in your entire medical career had extremely rare potential side effects, adverse reactions, or dangers. 10’s of millions of people have this done every day and there are a small handful of cases where bad things happened. You have a better chance of getting hurt from taking regular every day medication than you do with something like this. If it was really as dangerous as you are making it out to be then these people wouldn’t have medical licenses. They would’ve been sued out of practice snd professions ages ago. If the idea of it scares you then fine. Nobody is forcing you to do it. But it does help a lot of people and there is no reason to make others afraid of it. I never went to one until I hurt my neck and my back and my mom talked me into going. Probably one of the most relaxing experiences of my life. The potential danger you are listing comes from violent movements when your body is tensed up mixed with stopping in your tracks at 60mph. Where as this doesn’t match the intensity of a car wreck in the least bit. Chiropractors can feel the muscles in your body and will make sure you are totally relaxed and no tensed up. Again if you don’t like it thats fine but it is less dangerous than many things you did in tour career

Basically, for anyone who’s curious as to why anyone would willingly have this stuff done, These adjustments are for people with pain due to joints and muscles that can’t afford to have anything done surgically.
I was born with EDS so my weak ligaments cause my spine and all my limb joints to not always sit in their proper alignment. The body’s response to this is to use your muscles to PULL those things back into their correct spot. If you’ve ever woken up with a sore neck after sleeping in an unusual position, you’re familiar with this concept. Because the muscles are straining to pull on your bones they become tense and painful. Anti-inflammatory drugs with help this, But to cure the problem, you have to address the cause. Chiropractors reposition the bones so that your muscles don’t have to. Once the muscles no longer need to strain they can relax and calm down.
Fun story, EDS has caused me to pop several ribs out of place, MULTIPLE TIMES. Imagine laying on the couch sobbing because even the shallowest breaths cause excruciating pain in your rib, cuz that’s what it was like. I tolerated 3 days of this before seeing my first ever chiropractor. My rib felt better within 30 minutes of leaving so i can attest to the fact that at least some of these people know what they are doing, and if your in pain and drugs aren’t helping to dull it, it’s worth a try.

I would willingly be ring dingered (y strapped) because the extension is the one type of adjustment i can't do to myself. I can get the rotational ones and the "shift" ones, but I can't create extension for myself. I need someone to play me like an accordian.

Nice to see a fellow zebra! 😄
I have combo type 4 and 3 EDS, along with several other conditions that are associated with Ehlers Danlos Syndrome. I have at LEAST 2 daily joint dislocations (not to mention the countless hyperextensions & subluxations) due to the severity of my rare combo type of EDS. This affects every joint in my body, and it's to the point that at 25yrs old, all but 1 of my joints are now bone-on-bone due to the daily damage & the connective tissue is so naturally stretched that sleeping for more than 3hrs at a time causes my body to relax too much to where I wake up to dislocations of at least 1 dislocation of either my shoulders, hips, knees, elbows, ribs, or ankles; this of course is excluding all the hyperextensions/subluxations/dislocations that are caused by standing, walking, putting away groceries, sneezing/coughing, getting in and out of my power wheelchair, brushing my hair, etc. Needless to say the chronic pain is more than most can imagine, even with pain management medication and treatments. I understand the desperate want to try anything just to get some pain relief, no matter how temporary it may be.
I'm glad that you are able to get some relief through your chiropractic care! I used to get weekly chiropractic care for over 2 yrs (recommended by my pain management specialist at the time), and it unfortunately ended up doing more damage to my joints in the long run due to the extra manipulation of my joints. I STRONGLY suggest that anyone with hyper mobility issues see their specialists before any chiropractic care. Some EDS'ers find relief with it (generally those with type 1 and 2), however most others with the more extreme hypermobile types of EDS (like myself) found that it worsen their pain and joint conditions either in the short or long term. The worsening of the joint conditions is at NO fault of the chiropractors-- they genuinely help most by alleviating joint/back pain and are great at what they do-- it's just because our bodies aren't built like "normal" people's bodies due to the connective tissue. Every case of EDS differs from each other, even if it's the same type of EDS, which makes it difficult for doctors to find a universal way to manage pain for us. So to any zebras out there that are interested in starting chiropractic care or are referred to a chiropractor by a doctor-- please do research and talk to several others with EDS too, specifically those who share the same type/severity of your condition, to get their experience with chiropractic care to make an informed decision to hopefully prevent any worsening of your condition. I hope that whoever reads this has a wonderful day and is as pain free as possible!💜

@@parkercrossland410 you can get things to provide traction on your neck like that. You cant get it as much as the ystrap but i have one. It's kinda like an inflatable neck pillow
