Why the Ears Heal Slowly after a Facelift and How to Improve Healing in this Area

  • A woman had a mini facelift 5 weeks ago but after that, but she has scabs on her ears that are not fully healing. She wants to know how to help heal the scabs because it has gotten worse despite the use of antibiotic creams. She also wants to know if she has pixie ears.
    Dr. Amiya Prasad, an oculofacial plastic surgeon, explains that in face lifting surgery, the area which has the least amount of oxygen or blood supply is the very edge of the skin flap by the ears. Even in patients who are non-smokers, they can also have a compromise in the oxygenation of these tissues that can affect the integrity of the skin. This may lead to necrosis or the death of the tissue if the area turns black.
    In this woman's case, Dr. Prasad says that she is dealing with a breakdown of the skin resulting in scabbing and bleeding. It's very critical at this period that she prevents any bacterial infection because she has an open wound. Dr. Prasad recommends that she washes her hands frequently. In his practice, he uses a material called extracellular matrix (ECM) which he uses in incisions to help with the healing process of the skin. It heals by duplication of the normal cells so that the incision lines heal beautifully.
    Pixie ear by definition is an earlobe that tends to be pulled downward because it looks like it has a little point. From the photo the woman submitted of her left ear which looks rounded, Dr. Prasad can't say that she has a pixie ear. He advises that she wait for about 6 months before planning any type of revision work or other cosmetic intervention until she can see the changes after everything settles.

    I don’t remember her saying anything about smoking. She has an infection! I just had a full face neck lift and my ears never looked like that! She should be on antibiotics I can see the infection.

    Can you tel me please what is the consequence of removing the stitches from the wound which is not healing properly? Would the wound just open up more under influence of inflammation? I am due for removing stitches on 4-3-2023 but they are not really improving as you can see

      Hi N B,
      This is something you should be asking your doctor who stitched you as they know details about your situation that cannot be seen in pictures. I hope this clarifies things for you.

      Hi Renee Kerner,
      Yup, I need a haircut in a lot of my videos because my hair grows very fast. My hair is really long in the videos when I announced temporary closure of the practice due to quarantine laws, and even longer when the practice reopened a few months later. They are on my channel in case you are curious.

    Thank you so much for posting this video. I suffered a pretty painful hematoma after a deep plane facelift which then led to the skin on the back of both my ears coming off. It was very red and it had very tiny white pimples then the top of it started to turn very dark red which I think may have been the beginning of necrosis.
    I’m now beginning with the early stages of fibrosis and my doctor has given me one session of ultrasound and I am to take another session after two weeks.
    I am a non-smoker and I am not around people who smoke.

    If someone is smoker , how long does he needs to quite smooking to prep for surgery
    And is it same for diabetics people??

    • @AmiyaPrasadMD
      @AmiyaPrasadMD  10 หลายเดือนก่อน

      Hi @gyaniadmi2347,
      You should quit smoking as soon as possible because because reduced tissue quality due to smoking can affect your surgery, and the toxicity within your body from smoking affects surgical healing. I hope this clarifies things for you.
