I think it depends on what type of exterminatus is being used. If memory serves, there's a part in some novel or short story where a commander on a ship in orbit accidentally broadcasts to a broader vox channel than intended that they're going to exterminatus the planet and they hear a deafening wave of lasgun fire come back on the other end. The commander asks something like "What weas that? Are you under attack?" and the Commissar planetside with the Imperial Guard just goes "No, those were the sui***es."
