    If you have a disc herniation and it's causing sciatica, a microdiscectomy is an option that will give you almost instant relief, however, this minimally invasive surgery only addresses a portion of the problem.
    A herniated disc is weakest at its posterior portion and in order to avoid the disc from herniating again, you have to strengthen that weak point. In fact, it is very possible that you can re-herniate that same disc within 3 months after your sciatica surgery.
    painfreeinstitute.net
    It's very important to due your homework before you decide to have a microdiscectomy because 90% of people with herniated discs DO NOT require surgery and can heal conservatively through exercises.
    Exercises that are recommended for herniated discs focus on naturally removing pressure off the lumbar spine and sciatic nerve through lumbar extension.
    At the PainFree Institute, we teach you how to do these exercises for herniated discs and sciatica so that you can strengthen your back, heal naturally, and save THOUSANDS of dollars is risky back surgery and epidural injections.
    There are so many options when you're looking for a sciatica treatment and sciatic pain relief due to a ruptured disc and its always best to get a second opinion.
    Another option before surgery would be disc decompression therapy. To learn more about it go watch this video right now • Inversion Table & Spin...

    Yes, together with a physical therapist and a chiropractor, I've been trying to get my disc to heal naturally for more then over a year now. A year in pain. Pain which is unbearable at best. It has gotten worse and worse. I can hardly walk. I can no longer sit. I take a lot of pain medication which messes with some of my favorite body functions.
    Sometimes the body cannot heal on its own. And sometimes exercising only aggravates the issue. I swear that the "therapy" only made things worse. I was in a lot of pain during the exercises. My body was warning me that it was not good. Listen to your body!
    I can't wait to have my surgery.

      If you've been in treatment for a year and it has not improved then maybe surgery is a good option for you. All cases are different and the majority of people can get well without surgery but sometimes it may be necessary. Best wishes and a speedy recovery.

    I had a microdiscectomy 4 years ago. Could not walk before. Had it done and I've been great ever since. Doesn't cost a penny in the UK. NHS. I'd recommend it to anyone. Off work for 2 weeks. Recover and start Yoga. I play golf, mountain bike. Keep active and stretch and you will be fine. Best thing I ever did. Had 2 epidural injections that did nothing that's how bad my pain was.

    I've done 3 different 6 months of physical therapy, 7 steroid injections, chiropractic sessions, muscle strengthening sessions, and pain medications. All of this has done nothing for my L5 S1 area rupture. I am scheduled for a Microdiscectomy Surgery in 4 days. The Doctors and I are worried about permanent nerve damage due to the numbness in my right leg and foot. I'm writing this for others to know that it can be dangerous to wait so long and sometimes surgery is the only answer. Listen to your body, it will tell you what you should do.

    I have done in Italy and my life is paradise after 15 years of physiotherapy, pills and other stupid things that did not work. Only with surgery I saved my life and in Italy health service is free, so I did not pay a penny. Yoga helps but only micro-discectomy has eliminated 99% of the sciatica pain and back that forced me to lose 4 jobs in few years. So my advice is get surgery. I did 6 months physio to recover after the surgery. Out of work 30 days, but now I got back my normal life, work full time and never had any problem or painever.

    Dr. Guevara, I had no prior back, sciatica, or hip pain. My pain started in my left groin. I couldn't go up the stairs, the pain would increase. I went to the ER and was told I pulled a groin muscle. Pain meds were prescribed, and I was told eventually it will go away. After about a couple of weeks, I was experience the same pain going down the stairs. Couldn't take it anymore. I went to urgent care for. a second opinion. Long story short, was diagnosis with AVN, rarely caused by my Hypothyroidism. Total Hip Replacement was done, this year Feb. 16th, went to rehab for a month. Had hip, incision pain and tenderness. Also the same groin pain. Now I have lower back and sciatica like pain and pressure as well. Two MRI's were done. Hip MRI nothing showing a reason for the continuing pain. THR looked good. No tears and no detachment. The second MRI on the lower back spine, showed a herniated disc. HOW? is this possible? The first MRI diagnosing me with AVN states, no visual problems in my spine. Is it possible the THR procedure could have injuried my back? This other question is very important. This could answer everything. Can a Spine Specialist, tell how long I Had the herniated disc? Please reply sir, the procedure you just described is something I was looking into. At your convenance let me know. Thank you.

    • @drjoseguevara
      @drjoseguevara  7 ปีที่แล้ว

      The MRI's are usually per section. If an MRI of the hip was done to diagnose the AVN, that MRI wouldn't show the lumbar spine unless you had one done for that area also. I don't believe the herniated disc was caused by your hip procedure. As far as your last question, it's very hard to tell when a herniated disc occurred. They're usually discovered when a person complains of pain.

    Had my surgery in 2018 and I actually got worse on my lower back L4-L5. I’m 23 now and the pain has gotten worse each year. I go to my chiropractor monthly and it does relieve some pain but only for that moment. I’m scared to get to epidural injection because Ik it just numbs the pain. Any suggestions I’m scared and feel like I can’t engage in social life because of the pain.

    Re-herniation at 26 months post laminectomy. (Similar to microdiscectomy). This time on the other side of the same disc. He’s basically right. When the pain is gone you go back normal activity or even greater activity thinking all is well...... but that is not the case. ALWAYS gotta consider yourself weak and at risk.... don’t push it.

    • @drjoseguevara
      @drjoseguevara  4 ปีที่แล้ว +1

      The original herniation is not fixed, they just remove the portion pressing on nerves so yes you're still at risk because the disc is weak. You have to make strengthening your back and practicing proper ergonomics a lifestyle.

      @NeoTyhaich 3 ปีที่แล้ว

    My doctor just suggested I get a microdiscectomy because the bulge in my disc has gotten to be too bad. The pain is unbearable most times and very uncomfortable at others. If I end up having to get the surgery, if I follow up with the exercises you mentioned that address the weakness on the other side of the disc, will that still be effective and better my chances of not having reoccurring disc issues?

    • @drjoseguevara
      @drjoseguevara  7 ปีที่แล้ว +2

      Yes, even after surgery you still have to strengthen your low back to prevent re-herniations

    Q: Does the disc tissue regenerate after surgery, causing the disc to re-herniate on that weakness you mention in the video? Or is re-herniation caused simply by the remaining disc tissue protruding again through said weakness?

    • @drjoseguevara
      @drjoseguevara  6 ปีที่แล้ว

      The disc doesn't regenerate. The surgery is just to remove the herniated portion so the disc still has a weak outer border. Improper movements after the surgery can cause the disc to re-herniate.

    Hi Doc. I have been experiencing sciatica on my right leg for more than 2 years due to work injury. According to the MRI, my L5-S1 disc has 3mm thickness. The worst pain is when I put pressure on my right lower back. I avoid raising up my right leg, putting pressure on it when sitting, walking, and sleeping. I am afraid to get spine surgery, but I feel that my condition would eventually lead to surgery. Please be advised. Thank you.

    • @drjoseguevara
      @drjoseguevara  7 ปีที่แล้ว

      I would suggest trying exercises first. Most people with a herniated disc do not require surgery. If they don't help you out then surgery may be an option. I have several videos with exercises for disc injuries on my blog painfreeinstitute.net

  • @tractorback76
    @tractorback76 2 ปีที่แล้ว

    People bag on the surgery but once it's blown out what's the difference, other than cost and risk of rooting around in there (and anesthesia)? It's not like tolerating or waiting on the herniation to be reabsorbed or self corrected makes the disc any less susceptible to re-herniation. You've already demolished the disc.

    I had an MRI that showed a "significant " herniation to L5S1 that was compressing sciatic nerve,I had a week in hospital and it's now been 8 weeks with the pain,it's eased but still very uncomfortable,is there a chance tge disc has healed but the nerve is still badly compressed?

    • @drjoseguevara
      @drjoseguevara  6 ปีที่แล้ว

      It's possible. A disc can heal over time but along with a disc herniation there's also inflammation around that area so that can contribute to the nerve compression as well.

    Dr. Jose Guevara I been dealing with a herinated disc L5/S1 for 8th months. I did two Epidural Shots and some of the swelling went down. I'm still swollen on the right side and muscle knots by the si joints and the top of the buttock. I can tolerate the pain in the back but the leg pain is something else. I can't extend my right leg but I get the best Relief when laying down. I get the really bad pain if I sit or press on the trigger point on the side of my hip. I feel if the swelling goes down I'll be in better shape...

    • @drjoseguevara

      See if you can do some sort of decompression. You can do so using an inversion table that you can buy and use from home. Also try getting in a pool. The pool takes away gravity so you feel less pressure on your back and joints and it allows you to move better so you can start exercising slowly

    • @drjoseguevara

      I did mention in the video that people feel immediately relief after a microdiscectomy because the herniated portion pressing on a nerve is removed, so yes it's successful in relieving pain but that doesn't mean the source of the source of the problem (the disc) is better.
      You can avoid re-herniations by getting your back strong through exercises and lifting objects without bending from the waist.

I've been doing all the rehabilitation exercises and also started swimming, but I only need to sneeze in order to get in a crisis and as I said before, the pain is only getting worse. I'm on a crisis right now and I can barley walk! It's affecting to much of my life and I can't manage this pain without taking anti-inflammatory. Since I don't want to be using it for the rest of my life, or at least for a longer period, I'm really considering a microdiscectomy.
    It would be great to get some opinions!
    Thanks!

      If you've tried everything for the past 8 months without any improvement than a microdiscectomy may be the next step. With time and rehabilitation you can rehab the disc after the microdiscectomy.

    What about if you have symptoms of the herniated disc pressing on the nerve such as severe foot weakness, that impairs you from walking unless you use a cane, should you still avoid surgery?

      Depends on how severe the herniation is. If you've tried conservative treatments without any relief, then surgery would be the next option.

    I think I should have seen this before I did surgery. It didn't cost money because I had insurance but I got another pain again. I had a surgery in October because I couldn't stand up without pain but, unlike what doctor told me, I had pain for a few months after. But the pain was a different one. Didn't fee like sciatica, I felt sore in my hip. Now five moths later, just when I thought I no longer have any pain and just working on stretching, I have sciatica again.....Now I don't want to have any surgery, just plan to lie in bed for a week or a few weeks if I need.
    Would this work and heal naturally?? Physiotherapy seems to add the pain so really nothing else to do. :(

    I thought à microdisectomies actually seal up or ''stich'' the annulus fibrosis so the disc won't heal again...at least this is what I heard one doctor say in an interview.
    By the way I'm 23 and having sciatica for over a year and a half now despite dong exercieses. Standing/walking is the worst..starting to consider surgery....

    • @drjoseguevara
      @drjoseguevara  8 ปีที่แล้ว

      No they don't stitch up the disc. All they do is remove the herniated portion of the disc. This is why many people end up having surgery again. I get emails every day of people that go through surgery and don't get well. Many people think surgery is the answer to everything but that is not always the case. It's best to follow a treatment plan that goes through different phases of healing and strengthens your body to prevent injuries in the future.

    Hey, I had an on the job injury that caused me to herniate two discs in my lower back. The pain is severe in my back, and although it doesn’t cause pain in my legs, it does cause weakness, and makes me stumble and lose my balance ever since the accident. My workers comp has paid for an orthopedic surgeon to evaluate me and he said you do not perform a microdiscectomy unless there is significant leg pain radiating downwards, and thus told me I was not a candidate for surgery, is this true? He was very rude and accused me of being in it for the money, told me if I stopped being fat the pain would go away, I heard him telling his staff to decline certain patients because they’re annoying, despite decades of experience he has a poor reputation, and even in his lobby I heard his patients saying procedures they have gotten from him haven’t worked out, so I’m asking because I do not truth this man, and I have exhausted every other option, injections, chiropractor, physical therapy, etc, still in severe pain, even sitting hurts and it’s been 10 months since the accident.

    • @DatsDoodoBaby

      Just to clarify I know weight can contribute to back pain, but I was rammed into from behind by a vehicle traveling about 70 MPH, the herniation was caused from the trauma of that hit, and I had no back pain in the past.

    • @drjoseguevara
      @drjoseguevara  6 ปีที่แล้ว

      I would get a second opinion from someone else especially if that doctor has a bad reputation. If you have leg weakness and you're stumbling, it seems like you have pinched nerves from the herniation and if you haven't had any positive results in 10 months maybe a discectomy may help but that's up to the surgeons to determine.

Because the doctors didn't tell me how it was caused I'm still attempting stretches that could potentially be making my sciatica worse. I've been put on naproxen, gabapentin and now been referred to see a practitioner and the process of all this had taken over a year! Why is it so difficult for doctors to see the amount of pain? I'm 19 years old and can't even walk because of sciatica. My football career has probably been taken away forever and it's honestly heartbreaking.

      You need to go to a doctor who can find the cause of your sciatica. If you're 19 years old, it's most likely due a herniated disc in the low back but you need to get an MRI done to get a definite answer

    Hi I just turned 19, hurt my back in July of 2017, got a MRI that showed I have 2 bulging discs (l4,l5)and was experiencing sciatic pain on right leg and butt. I began doing physical therapy for a month and that didn’t really help. So I decided to get an epidural injection and with it, the sciatic pain in my leg and butt went away. BUT when i sleep I still get sharp pains in my back, now have tingling in my foot, and still can’t raise my right leg without pain in my back. Will microdisectomy fix the tingling and will it allow me to raise my leg???
    PLEASE help!!!

    • @drjoseguevara
      @drjoseguevara  6 ปีที่แล้ว

      If the physical therapy didn't help and the injection didn't completely get rid of the pain, then a microdiscectomy would be the next step. You'd have to talk to a surgeon to see if you would be a good candidate for the surgery or not.
      Another option would be to look into decompression therapy like I talk about in this video th-cam.com/video/R2HnwVNJdVs/w-d-xo.html

      Sorry I forgot to mention that we also tried spinal decompression, which helped for the first 20/30 minutes, then I would be back to the same pain...
      Thanks for the reply!!

    • @drjoseguevara
      @drjoseguevara  4 ปีที่แล้ว +1

      Sorry to hear. I only recommend surgery if you've tried everything possible and have not gotten results and if the pain is so debilitating that you can't do your daily activities.
      For some people surgery may be the only option but that's only a small percentage of people. A lot of people walk around with disc herniations that don't require surgery.

      @@drjoseguevara Thanks for the reply, wasn't expecting that. At the time I had an m.r.i and it showed 3 herniated discs. Doctor sent me to a spine consultant who recommended surgery. Now 6 years on and I have 6 herniated discs, and I'm in so much pain all the time, have been since the op but it gets worse everyday. Noone wants to risk another op they're worried about making it worse. They just keep giving me all kinds of pills that do nothing. I've become housebound, can only leave in a wheelchair, and gained 4 stone from being disabled which has made it all worse. Sorry, I know you probably don't have any answers for me, just don't know what to do. The doctor just tell's me I'm one of the unlucky one's but I feel my lifes over since the operation, I can't go on in this much pain everyday

    I've had this done twice. I'm in just as much pain now as I ever was. I had my surgeries one month apart. That was in 2008. My drs have had me on strong pain killers since. I HATE it. I'm talking to my dr within the next week to get off this crap medication. The dr that originally did my surgeries said it usually takes three times before it works. I said that sounds like bs and switched drs.

    For real Sciaitc PAIN not the nagging oh that hurts when I bend, but for someone that has been living with a ruptured disc, and pain so bad you can not walk or sleep, there is no NATURAL WAY to heal it. Yoga is not going to work, tumeric, lots of water, no amount of stretching or inversion or disc decompression. Some people need relief I pray you never get real sciatic pain because you will rethink your whole idea on this. I had sciatic pain for 4 years tried every means to fix it naturally. It didn't get any better and no it did not get any worse, but I could sleep and work and do things I normally did. Then my back went out and I got the "REAL" sciatic pain. It all comes down to what type of pain level you are at. If you are on a 0-10 scale I say anything over 6 means you are not sleeping. A 9 means you want to blow your brains out. I was at a 9, if you are at a 7 or higher I say go for the natural but you can only live like this a week or so before you need very strong pain meds which in my case did not work at all. I have a high pain tolerance, I have had torn ACL's in both legs, broken arms, broken anlkes, torn Ligaments, torn Hamstring, nothing I have ever experienced compares to the sciatic pain I had. Finally had Microdiscectomy and right out of surgery I could walk again and was pain free, Off all meds by that night... taking nothing now for pain and 2nd week of post opt. Natural healing sounds great but that is like saying for a torn ACL you can naturally heal it when without surgery you will never fix it. Don't put off surgery if you need it. That is all I am saying. See the posts below and know surgery might be your only fix...

    All respect to your little video Doctor, it is almost saying to somebody of my stature, your just not smart enough to be healthy, if drawing a Pic looking like a light bulb with the cord being disconnected has any value to it is almost down right insulting. Sorry but the video and chalk Board thing cracks me up, i can draw a resistor out on a chalk Board in detail showing exactly how it can be used in not only Automotive purposes, though many other operations, so i should make a video naming it 100 reasons why Doctors have to go get a license to Practice, because that is all it is, Practice. I sure wish somebody would pay me to just Practice on their Car and then tell them they just can't drive.

      Most people don't comprehend medical terminology so I make the videos very simple so people can understand what's going on in their body. In no way are the videos meant to insult anyone. They are here for educational purposes.

    Minimally invasive?? You kidding?? Cutting into your body and Drilling and sucking out half or all of your disc is not minimally invasive! For me, these yokos didn't remove my disc, they left all of the shards and pieces of my disc wherever in landed inside of my body where the surgeon drilled. These yokos don't tell you that you are left with scar tissue that only gets worse and causes even more pain. And on top of that, ARTHRITIS, they don't tell you that this surgery will give you ARTHRITIS on your spine which, gets worse and causes even more pain, ha. DONT DO IT. A DECOMPRESSION TABLE snaps your discs back in place like a gum band. THESE YOKOS WONT TELL YOU ABOUT IT BECAUSE OF THE "MONEY" THAT THEY MAKE ON YOU. These surgery's are unnecessary and these yokos know it! Jesus fn crise.

      Haha yes it's totally true! I am scheduled to have this surgery (L5 S1 herniation) on 30th December. After trying 5 months of physical therapy (also free here in UK) and several sessions of accupuncture, I have now been advised by physio that by now as there isn't significant improvement then surgery really is the best option. Not looking forward to it but I'm struggling to stand up straight, can't walk far without crutches and that's with taking gabapentin and naproxen every day. Thank God for the NHS and free access to health care.
