    ➡️ Question #1
A 74 kg male patient is intubated and receiving volume-controlled A/C ventilation with the following settings: FiO2 of 50%, set rate of 15 breaths/min, and a set tidal volume of 550 mL. The patient’s total respiratory rate is 29 breaths/min and the following ABG results were obtained:
    pH 7.53
    PaCO2 27 torr
    HCO3 23 mEq/L
    BE -2
    PaO2 82 torr
    SaO2 97%
Which of the following would you recommend?
    A. Increase the FiO2
    B. Increase the set rate
    C. Increase the set tidal volume
    D. Add mechanical deadspace
    ➡️ Question #2
A 52 kg female patient is receiving volume control A/C mechanical ventilation with a tidal volume of 400 mL, a set rate of 10/min, and an FiO2 of 35%. Her blood gas results are as follows:
    pH 7.31
    PaCO2 49 torr
    HCO3 24 mEq/L
    BE -2 mEq/L
    PaO2 84 torr
    SpO2 95%
Based on the given information, which of the following changes is appropriate at this time?
    A. Increase the FiO2
    B. Increase the tidal volume
    C. Increase the set rate

One of the problems I have on vent setting adjustments is learning to go by the waveforms. Obviously, I have the waveforms in textbooks and they are simple enough but rarely do I see waveforms that simple when I'm actually on the job. Can you post a video of interpreting dyssynchrony or discomfort based solely on waveforms?

Hello Coach .. yesterday we had a neonate patient the ABG result was acute respiratory acidosis .. the Dr. shift the mode to P-SIMV from P-A/C but the actual rate of Patient increased to 65bpm above and the Spo2 fluctuates to 75-80 I suggest to shift back the patient to P-A/C with the settings of PIP-18 rate of 35, I.time of 0.50, PEEP 4, fio2 60% but the Spo2 of the patient still is not good .. what should be the correct adjustment of the settings? I am not expert in managing neonate it is really my struggle .. I hope you could help me on this ., Thank you !
