it's interesting how, in the primary timeline, she goes from "kaname madoka" to "madoka!" as she gets more stressed, but even when she's superficially back as her early-timeline transfer student self (who *never* says madoka, just "kaname-san") in rebellion, she still only calls madoka by her given name. post-apotheosis, however, she goes through an erratic mix of "kaname-san", "kaname madoka", and "madoka-sans", sort of vacillating wildly between levels of formality and familiarity. it's neat.
