I swapped to an 8 3/4 out of a mid 70's pick-up. You could also re-drill the pattern in the axles. Unfortunately I don't have an easy solution for the rear.

Not really that will not help the back
My 83 d150 has the 5x4.5 you can get alot of different rims in that pattern but the issue is the center bore is really large

Same for the back rotors?
No I swapped the rearend with an older 8 3/4 for the rear.

The rotor with the small bolt pattern is off an 80-84 D150.
