Q:02 Difference Between Cortisone as Hydrocortisone. Ans: 1) Cortisone is naturally occurring glucocorticoid. Position of ketone group in Cortisol is at C3, 11 and 22 C, means contain three ketone group in it's chemical structure. And 2 hydroxyl group Present at carbon 17 and 21. 2) Hydrocortisone is reduced form of Cortisone because Keto group at C-11 position is converted into Hydroxyl group then hydrocortisone is formed, position of ketone group is at C-11. Biologically activity of hydrocortisone is greater than cortisone. Q:03 Synthesization of Prednisone and Prednisolone. Ans : A) In Cortisol's chemical structure C1 and C2 double bond potency increases 4 times and directly increases the Glucocorticoid activity through this C-11 keto group is converted into Prednisone. So Presence of additional double bond and additional unsaturation between C1 and C2 synthesizes the Prednisone. B) Hydrocortisone ( Natural C-11 hydroxy group ) is converted into Prednisolone. Ans:1 G.C involves in metabolism of carbohydrates, proteins, fats b/c they are mainly released in response of hypoglycemia, which acts as a stimuli for the released of G.C. They have an also important role in the inflammation and immunity responses inside the body. Also known as stress hormone. Ans:2 In cortisone ketone group is present at C-11 and in hydrocortisone hydroxyl group present at C-11. In cortisone 3 keto group is present at C-3,11,20 and 2 hydroxyl group present at C-17,21. In hydrocortisone 2 ketone group are present at C-3,20 and 3 hydroxyl group present at C-11,17,21. Ans:3 In G.C un saturation is present at the 4th,5th C-atom but we synthesize new derivative with maximum G.C activity in. Comparison to naturally occurring, so we introduced new un saturation at C-1,2 which increases potency of G.C 4 times in comparison to naturally occurring G.C. Cortisone can be converted into prednisone and additional un saturation of C-1,2 of hydrocortisone can be converted into prednisolone. Ans:4 Betamethasone is derivative of hydrocortisone. In betamethasone structure there is methyl group CH3 at C-16 position and Fluorine (F) at C-9 position while in beclomethasone methyl CH3 group is same at C-16 position but at C-9 Chlorine (cl) group is attached. Ans:5 During synthesis of different types of new derivative halogenation reaction was done at the different position in chemical structure of G.C, like it was done at C-6 but halogenation at C-6 produces G.C or M.C with lesser biological activity but when halogenation like (F) atom introduce or substituted at C-9 then new derivative form c/d fludrocortisone and its activity is 800 times more M.C and 11 times more G.C activity then previous. Fludrocortisone contain additional F atom which is halogen at C-9 and due to this F or halogen at its activity is increase and this cause inhibition of metabolism of or reconversion of hydrocortisone into cortisone b/c hydrocortisone & cortisone are interconvertable to one another due to metabolism reactions.

1.G.C are used to regulate the proteins, fats, and carbohydrates metabolism and mainly released in response of hypoglycemia .G.C are involved in immunity response and inflammation , also in the operation of processes that enable the body to resist stress and infection so that's why they are called stress hormone.
2. Cortisone contain keto group at Carbon no .3, 11, 20 and hydroxyl group at carbon no.17 ,21while hydrocortisone contain hydroxyl group at carbon no. 11, 17, 21 and keto group at carbon no. 3 ,20.
3. when unsaturation occur at carbon.no 1,2 the potency increases 4 times and thus G.C activity become increased and here is formation of new synthetic derivative is taking place. When cortisone naturally occurring carbon 11 keto group is converted into prednisone because of additional presence of double bond at carbon no. 1,2 .
when hydrocortisone naturally occurring carbon 11 hydroxyl group is converted into predisolone due to additionally presence of double bond at carbon no. 1 and 2nd .
4.Betamethasone is derivative of hydro cortisone .In betamethasone the methyl group or OH group is substituted at carbon no .16 and fluorine is attached with carbon no.9 position. This substitution can enhance the G.C activity.
Beclomethasone the methyl group is present at same carbon no. 16 but at carbon no. 9 the chlorine atom is substituted so this derivative increase the G.C activity, lipophilicity, bronchial tissue absorption and duration of action.
5.when halogenation occur at carbon no .6 it causes less G.C and M.C activity but when halogenation occur at carbon no .9 or when fluorine is attached at carbon no.9 it increases 800 times M.C and 11 times G.C activity so here a new derivative is formed by this halogenation called fludrocortisone.once the fludrocortisone is made the interconversion of OH group into keto group is inhibited because the hydrocortisone and cortisone are interconvertable with each other.

Then Unsaturation occurs at C1=C2 . All is the great role of additional double bonds between C1 and C2.

During synthesis of different types of new derivative halogenation reaction was done at the different position in chemical structure of G.C, like it was done at C-6 but halogenation at C-6 produces G.C or M.C with lesser biological activity but when halogenation like (F) atom introduce or substituted at C-9 then new derivative form c/d fludrocortisone and its activity is 800 times more M.C and 11 times more G.C activity then previous.

In GC saturation is present at the 4th ,5th atom of carbon. However, for synthesizing new derivatives with maximum GC activity in comparison to naturally occurring, we introduce new unsaturation at C1,2 which increases potency of GC 4 folds. Cortisone can be converted into prednisone by this way and additional unsaturation at C1,2 of hydrocortisone can be converted into prednisolone.
