They're allotted only a certain amount of time because it's very expensive to have all those junk haulers and their trucks there, plus all the cleaners. They show a lot of "volunteers," but there are many paid people who are doing the actual work so that it gets done in a timely manner, and within budget. Matt Paxton, Dr. Thompson, Dorothy Breninger---they aren't working the show for free.

Because it's a TV show and they need to add some kind of artificial drama
Imagine if they had a lot of pre psychological care and then slowly ease into the process and then the after care to rlly avoid relapse

There are a lot of very positive comments here about how Dennis has magnanimously addressed his hoarding for the sake of his sick wife. This video has been produced to appear as though this is the case, but if you pay attention to the video you can see this is entirely false. He was forced to clear out his hoard because his home was non-compliant and he was receiving fines from the city for his hoard (they mention this three times in the video - at 4:58 - non-compliance, 8:16 - fines and 8:22 - "the county is going to work with them"). He had no choice but to do it or lose his home. That's why there was a deadline on the clean-out - the city would not have given him unlimited time to sort out his house, so they had to work to a timetable. Its also why they attacked the outside of the home simultaneously with the inside. If the idea was to make the house safe for Judith, the interior would have been the priority and the junk outside could have been left till later. They were forced to clean up the outside because the city required it.
Though they don't show it in this mini-episode, in the full episode they explain that the reason why they threw out the furniture and got them a new bed and couch is because the existing furniture was too disgusting to keep. Also, they didn't show Dennis' objections to the removal of his junk during the process of clean-up.
Poor Judith was living in cluttered squalor. I have no doubt that Dennis loves his wife, but its inexcusable that he subjected her to such unhealthy, dangerous conditions when she was already so frail and unwell.
Though his wife is suffering from advanced Parkinson's, you can see that Dennis' illness far, far predates her own (it takes decades to accumulate >500 tons of junk). The fact that Dennis subjected his vulnerable, disabled wife to these unsafe and unhygienic conditions is very sad. Without considerable psychiatric help, Dennis will begin to rapidly reaccumulate his hoard.

Thanks for providing context for those of us unable to watch the whole show - very canny editing indeed. So true about the re-accumulation, as well; he basically admits as much at the end when he says he won't let it get as bad as it _was_ - it's like: 'no Dennis, it's not supposed to get bad _at all_ now that they've cleared it ! ' .
