    I think he “suspected” SARS just to sound important. He did put his best “I’m a doctor, look how awesome I am” face when he said that. That is 100% something JD would do to impress a girl.

      Very true! I’m a huge fan of scrubs and there are some pretty good episodes that are worth reviewing and to touch up of different topics I think Dr. Mike would like to review. One of my favorites: DOCTOR KEVIN CASEY aka Michael J Fox and the OCD :)

    Fun fact if you didn't know: Zach Braff who plays JD grew up in South Orange, NJ and in high school was a cadet at the South Orange Rescue Squad. My dad trained Zach to be an EMT and was his actual first aid and CPR instructor. There's a Scrubs poster in the South Orange Rescue Squad signed by Zach that says, "Thanks for teaching me all I know about the medical field." I know being an EMT is not the same as being a doctor but I still thought it would be fun to know!

    imagine: theres an emergency on a plane and they say "is there a doctor on board?" and someone stands up and is like "im a doctor but im a dermatologist" and the flight attendant goes "sit back down youre not a real doctor" and then another passenger goes "i watch Greys Anatomy" and the flight attendant goes "quickly come over this mans really sick" and they dermatologist is just sat there like 😳

    "Why did he say SARS?" - Its just because he wanetd to impress his new girlfriend with his medical stuff, by saying it could be something very bad he makes his job seem more dangerous. Also the lockdown with the siren was a fantasy of his (he always 'flashes' out of the fantasies with the white and the woosh)

    The beauty about Scrubs is that when they want to make things funny, they discard realism, but when they want to have more serious content they become more realistic, at least medically. Lol

    The idea behind JD asking if he's been to Honk Kong after looking at only 3 simple symptoms and concluding that the pacient has SARS, it is exactly JD imitating the "genius doctors" of TV shows trying to impress the girl. The whole part of the alarm and iron gates closing are poetic exaggerations of the protagonist's imagination.

      Also agreed, love him unironically breaking down intentionally dramatized scenes😂 legal eagle does a similar thing where he gives grades based on legal realism to fictional shows. Did one about willy wonka once. But I didn’t watch Charlie and the chocolate factory and think “wow this wonka guy is really violating a lot of labor and child endangerment laws💀”. So it’s not like people take these seriously...I hope. Wired also has a series that does the same too, where professionals react to scenes about their professions. Love the ones with the trauma surgeon..Annie? I think?.
      I may have an addiction to professional react videos🥴....and reddit text to speech videos for some reason. That last part was unrelated.

      It cut out but they asked if the patient had been to China (specifically Hong Kong) where SARS originated. The reason everyone thought the doctor was stupid is you find out the patient went you Hong Kong 20 years ago.

      Not terribly long ago there was an article about cabin crew asking for a dr on a flight. One passenger stood up, she is an OB/GYN (from Houston? Can't recall the departure/arrival of the flight) and a black woman. They told her that they needed an actual dr, then there was something about her providing ID proving she's a dr. Then when a white guy offers his service (he works in the medical field but is NOT an MD) the crew never asked for proof from him, and he's who the crew chose to assist the passenger.

      I dont know about the US, but in Germany all doctors know the basic stuff. His example with the allergic shock was really bad because thats a thing a dermatologist can encounter regulary because they also do test which can lead to a allergic shock where epipen is needed.

      @@AshesAshes44 Exactly that. Also the fact that Dr. Mike doesn't watch these shows in canonical order. He typically watches just the Pilot of some series, or select episodes that have been heavily recommended based on its content. It isn't always a big deal. Scrubs specifically however sometimes has sub plots / story lines that get put down and picked up again often. Missing 1 episode never did any harm, but missing 2-3 episodes, and I was completely lost.

    There’s now a Scrubs podcast that I listen to. They were actually awarded by AMA with having most accurate medical show at that time. Beating ER and other shows. Many medical students would tell them certain medical jargon they already knew was because of Scrubs.

    That is so amazing to hear about your chief of medicine. Having worked in the medical field there are a lot of egos out there and sometimes docs can get too big from the britches as much dad would say. I respect all doctors for what they do without them our healthcare system would collapse but, there are docs out there who let it go to their head and it can really be off putting for patients. When docs are humble and don’t just give orders to the people below them allows patients to connect with their docs and be more receptive to treatment plans. I’m so glad he is teaching doctors which is even better because he is helping create more docs like him which is what we need.
    Love you Dr. Mike! Please connect with Jessica Kent and/or Christina Randle to discuss medical care in the prison system, I would love to see your take on it, especially for pregnant women and how they are treated throughout their pregnancy and delivery.

    I always assumed this episode was set during the SARS outbreaks, so that’s why it would be immediately on their mind. Same thing with people now and covid... if you have a fever, you’re immediately quarantined.

      Alex, great response, you are exactly right. At my hospital, if you have body aches you get covid isolation. SARS was a hot topic at the time, so to call what JD does in this scene ridiculous and what we are doing now as normal precautions is pretty arrogant.

      It would still be a little odd as the show is set in the US. He asked the patient if he had been to Hong Kong, where there was a lot of SARS, but never waited for the answer. Good to be cautious but not exactly comparable with COVID which is incredibly prevalent across the globe, particularly the US.
      I think you're right that that's where the show was coming from though, not to mention it's not actually meant to be taken very seriously!

      @@caitie226 I read about a small story at army base (tiny one) where a junior officer was reading some paper and some one reported a strange smell whit description.
      the officer read that part out somewhat load to a collage off his the thing was there is a list of action anyone at the base can take for specific stuff.
      the officer then mumbled something in could be X whereby he said X a bit to load and a cadet that was walking by panic and did the action for X.
      que the base going into panic mode.
      I'm going to say probably a chemical attack. when in reality it was someone lunch or something equal stupid.

    Not sure Dr. Mike really *gets* Scrubs lol. Some of those scenes were within JD's imagination. And of course JD's isn't doing chest compressions properly on a guy who isn't actually having a heart attack. You want one of the episodes that is more diagnosis-heavy. This one is just a bottle episode that's mostly about the character interactions.

He had no idea who Kylie was. Thought maybe she was part of the staff 1:40. While they didn't introduce her here, this episode is all about J.D. attempting to show off his work and the hospital life to her. Haven't seen Mike do one of these in a bit, but the gaps in context appears obvious.

    The show gets sillier as they move along, Some info about this episode:
    The girl with him is a girl that he was trying to impress
    The caging scene is a daydream which is common in the show. Also this was filmed when the first SARS was still being studied and the sudden diagnosis was done to move along the story. Great reaction video, I hope you keep watching the show

      Kind of pissed me off he had his ass in a twist about JD mentioning SARS. The episode came out in 2005 and was right after China just had a huge SARS outbreak. Even today you still get asked if you had exposure to Covid 19 and it's 2024. Dr. Mike is an idiot.

    Aren’t chest compressions usually bad on tv? (At least when you can see the patient onscreen) I heard that there was too much risk of hurting the actor, up to and including breaking a rib.

      Most of the decent ones are actually recent & from big budget movies where they could digitally replace a dummy’s face with the actor’s... or shot in such a way that you can’t see the victim (the Buffy episode).

    Hey Dr Mike! Cabin crew here - We do have EpiPen onboard in the EMK (emergency medical kit). Most airlines do these days. And we are trained to administer it if necessary, following recommendations of the MedLink (or other medical support available, depending on the airline). Love your videos!

    Having lost my mom to COVID I really appreciate all you do. She worked in radiology her whole life until severe COPD sidelined her, and then we got really unlucky with COVID when she was already really weak. I really appreciate everything people like you tried to do for her thanks a lot.

    Pathologist speaking here: Dr mike we are trained in emergency situations- not as broad as you but we do human anatomy and we know protocol and how to handle certain situations

      It's still called practicing medicine, emergency or otherwise.. if you haven't done it ever in practice (or barely ever) or... it's something you haven't done for many years, you don't think you'd be more of a detriment to the patients health than a help to it? Outside of bls situations.

      @@liquidraver102 Continuing education is a real thing that many professionals do.
      In my experience of working with doctors (to keep me alive, usually), I have seen my surgical oncologist spend time in endocrinology learning some very important things that he then came back to share with his other patients (it was a several year switch. It's not like he took a weekend and got up to date.) I have also seen other doctors travel around to different clinics in the same medical center, and quite a few that cross train in any number of interesting fields.
      I have been assured that, while renewing your license to practice CPR, or any number of other emergency events, can be a yearly or otherwise scheduled thing, having an idea of what to do and being willing to do that thing is more important.
      It's also called "practicing law" or "practicing religion"; does that mean, in your mindset, that lawyers and priests/ministers/brethren/etc should not offer surface advice that is evident to anyone who has gone through the basic and initial training cycles in those specializations?
      Just because it's been a few years since I finished with my museum sciences classes doesn't mean I stopped learning about all the nifty things I can do to help people learn more about the world around them. It means that, while I may be continuing to keep myself well-informed about changes in my field, I can still make specialized displays and teach people about the world.
      I would be more likely to trust a pathologist than a random person on the street, or even a medical tech/First Responder, if I needed emergency care. While the tech/Responder may have more up-to-date paperwork in the form of certificates and appropriate check boxes in their CV, the doc, no matter their specialization, did a cubic butt-tonne of study into how things in human anatomy work together. Heck, it's more likely that a pathologist knows what some of the more serious issues in my local area would be, and would also know how best to fix them. Identifying Gram Negative bacteria, parasites, and disease is what that particular group of medical professionals does well.
      I mean, I have had fun with most of the pathologists I've known, and have learned amazing things from them.
      TL:DR? - No matter what the specialized field, doctors are more likely to understand what they feel comfortable doing and make whatever adjustments needed to save a life. I would not assume a random dude, or MedTech/First Responder would be able to say the same.

      @@goldengryphon Intermittent continuing education of a specialty vs continued practice are two very different things. Doing an ER fellowship is not the same thing as doing a one-week refresher course once every year or 3 or ever. Medical professionals will of course do CE's and stay current with BLS, because they have to, but they aren't going to have a significant level proficiency, as a provider who works in the area would. I wouldn't want a doctor who has been working in a specialist clinic for years to work on me outside of their specialty regardless of whether or not they've had some CE credits in that area. To add to the inexperience, if they haven't dealt with prehospital medicine in practice and only theoretically, their adrenaline is going to be coursing which could hinder a clear thought process and further risk patient safety. Returning to the emergency on a plane analogy, i'd rather have a paramedic than a doctor as they deal with these scenarios on the daily in a prehospital setting.

      @@liquidraver102 Stuck on a plane? I can see that. Paramedics are taught to think outside the box and to stabilize patients. So, for an emergency at many thousand feet in the air, yeah. I can understand that a paramedic stabilizing a patient would most of the time be preferable until more/different healthcare is available.
      In almost any other situation, where you are in an health related emergency situation, I would prefer a doctor who doesn't have a "stabilize the patient" mindset, and might be more inclined/better able to get to the root of the issue.
      I have been in a situation where the information I had just talked over with a doctor made that doctor a better candidate for treating me for a self-limiting condition, but I was put under the care of two paramedics who did not have any information about me. So, no. I can't agree that emergency situations require emergency specialists. Even once they had the information, my case was not handled well or properly.
      In the case of my oncologist and his several year exploration of the endocrine specialty, he is now a better doctor for the CE he picked up, while training under a specialist.
      I don't think you are understanding the difference between what I have seen doctors in specialty fields do (take long several year periods to focus on a different aspect of their over-arching specialty), and the idea that you can go for a weekend or week-long"refresher course" for some aspect of their job or to get certification in something.
      I agree. "Refresher courses" are not good enough. That said, a steady rotation of job skills (as you may find in rotating through different specialties and First Contact/First Responder situations) may be enough to keep the information fresh in a given person's mind. Let's say, everyone having to do a set number of days or hours in an ER, or Pediatric Ward, or hospital sponsored walk-in clinic.
      I am familiar with the 'Hammer' docs who tend to believe that every problem they run into is a 'nail'. That is a sign, to me at least, that they are inflexible in their options and rather fixed on their specialty. Those guys may be awesome in their fields, but they aren't the people I prefer to see.
      I guess, as the vast majority of the specialists I see are involved in expanding their understanding of medicine, and tend to be active in programs that force them to regularly deal with "pre-hospital" situations (Doctors Without Borders being one of them), I don't see the same limitations for CE as it involves my doctors and those medical professionals I work with. Maybe you are right about the majority of doctors being fixed on their specialties and otherwise inflexible. As I am able to do a lot more fine-tuning of the doctors I work with, I limit my exposure to single-minded problem solvers.

      @@goldengryphon to me, this entire conversation is a moot point. It's not about the title of the doctor. It's about who that doctor is. Not everybody is like you (and kudos to you), and willing to research and continue their education past what is necessity. I spent 15 years going from doctor to doctor, all of which blamed me for an illness that had nothing to do with me or my actions,(You're too young to be sick, You must be depressed and that's what's causing your pain, you're fat and lazy and that's why you're miserable, You're a drug seeker, it's all in your head) None of these physicians, regardless of their specialty, listened to me or spent any time outside of the 15-minute appointment that they deemed appropriate to attempt to help me by researching What could be causing all my issues. For seven of those years I told them exactly what my diagnosis was and they refused to even test me or find out who would be willing to test me. I would sooner trust my mom to diagnose me than most doctors. It all comes down to the commitment of that doctor or that EMT to do everything they can to help their patient and understand as much as they can to help future patients. And sadly, I find it to be a very rare person who cares more about the patient than their ego.

    The snacks thing is real. My mum is a nurse and whenever she works night shift there's always food. Most of the time I make her food to take to work. And whenever I've been in hospital at night time I always see the doctors and nurses running around trying to sneak something to eat in between patients. Makes me appreciate what you guys do even more.

      Sounds tricky given the symptoms vary so much from person to person. It can be hard to talk about symptoms without making one group feel isolated from the discussion. Worth discussing but it needs to be done with care.

      Stettafire i think people needs to be taught of how the condition varies from person to person since for example some people say that i fake my autism when mine is more high functioning than compared to my cousin which his autism is very severe and disabling. autism does need covering but does need to be handled with care, but we need to educate more people especially since it is becoming more of a well known condition over this past decade

    Fun Fact: the cake that I believe you called a “Philly Fluff”(Don’t shoot me if I got it wrong).
    There’s actually a cake that I believe is the same thing that was pictured in this video which is a German cake and we call it “Marmorkuchen” which translates to “Marble Cake”.
    The Marmorkuchen is usually made in a different cake tin with far more marbling but I always use the same cake tin that the Philly Fluff was in 😊
