    pilots work like shirt worker but even worse. Their schedules are all over the place and never the same. It's an accident waiting to happen, also, it's going to take a year to invstigate that accident in SF. I guess it's a hourly wage being an investigator.

    Fatigue is an issue but not the primary problem in this case. The problem was ILS approach verse VFR approach. An ILS will put the aircraft on the appropriate runway, not the taxiway. VFR approach being "visual" will always be at risk of incorrect alignment. The primary issue is procedural, not the consequences of fatigue alone. This is why they have two pilots and an auto system with back-ups in place. This is typical media driven paranoia.

      Fatigue can be a factor in said incident. However, the current rules in Canada are alarming. Even in the US, the fatigue rules still aren't perfect, so Canada being where they are is even worse.

    Why is Canada so far behind? Pilots and the air traffic controllers too are working long hours and have experienced fatigue. Bring change Lord to our Canadian rules. Safety is something we must bring to the forefront. We don't have to have an event to happen in our nation to change the rules. Bring us up to date in Canada in line with scientific data, international standards and best practices. Thank You Lord for all the harm that has been averted, and we ask for continued help from You.

    12 hours more ,over other countries Max hrs/28 days.. so tack another 10 minimum on there realistically.. I soon will be deciding where to take piloting. Canada is becoming less and less appealing. Less wage, more demanding flexibility, terrible weather.. also training costs are increasing.. I wonder why theres a shortage...

    At 2355:46 PDT, when ACA759 was about 0.7 mile from the landing threshold and about 300 ft above ground level (agl), the flight crew contacted the ATC tower, mentioned seeing lights on the runway, and requested confirmation that the flight was cleared to land.

    I'm not discounting the issue of pilot fatigue, but it seems an improvement in airport ground lighting is indicated. Anytime there are runways and taxiways in parallel and close together the end of the taxiway should be clearly and distinctively lit, such as with a set of lighted red crosses across the end of the taxiway. In the San Francisco incident the pilot landing at night simply appears to have gotten confused and mistook the taxiway for the runway.

      Or you can take the time to learn differences between runway markings and taxiway markings. For example, runway lights are white, taxiways lights are blue. Runways also have a line of green lights that indicate the beginning of the runway.

      Don't you think these Canadian pilots already knew all that? The point is that in this particular instance some set of circumstances came together to cause the mistake. Somehow the existing set of visual clues were misinterpreted. As such the more definitive you make the set of visual clues the less likely it is that a similar mistake will happen in the future.

      Just look at a photo of San Francisco runways 28L and 28R. Taxiway Charlie, (the one 759 lined up for) is scarcely lit, and on that night the taxi lights of the planes lined up were brighter than the taxiway lights. Meanwhile, the runways (27L and 28R) are clearly visible, twice as wide, twice as bright, and with PAPI lights, End Indicator lights, standard runway lights and Rabbit lights. How 759 made such a difficult mistake is unknown, but once they were lined up, little could make them change their minds about what they were lined up for.

      You will only see white light in the runway. Blue and green lights are for taxiways. Airplane waiting on the taxiways doesn’t have their white landing lights until ready to take off. Taxi lights are angled down to see the taxiway center line. The pilot already recognized that something doesn’t look right yet continued the approach. Fatigue was indeed a contributing factor to poor aeronautical decision making.

      Working at the airport, I can assure you that airport lighting (especially international airports recognized by ICAO) is standardized. Every airline pilot is used to the same lighting whether be in Toronto, SFO or Hong Kong. This is pilot error due to fatigue.

    To add! It’s December 2022 and 704 colleagues are still being flown up to 16 sectors a day! Implementation of all these new regs only coming into effect at the end of this year for them. Disgraceful!

    Maybe more pilots is the answer. I heard somewhere online that in the U.S, the military is hugging up all the good/experienced pilots, maybe that's happening in Canada too?

    It´s insane that MONEY IS THE ABSOLUTE GOAL DESPITE HUMAN LIVES. When I flew in Mexicana Airlines (ICAO Rules) in 1981, the MAXIMUM for JET PILOTS WAS 70 HOURS A MONTH, and for NON JET PROPULSION WAS 90 HOURS.

    The aircraft was attempting to land in the dark on Runway 28 Right. Nearby, Runway 28 LEFT was CLOSED and the runway lights were off. The pilots were confused and thought that Runway 28 Right was actually Runway 28 Left and the taxiway was Runway 28 Right. The pilots had not been informed that the lights were turned off on Runway 28 Left. Pilot fatigue was not a factor. The Canadian pilots' union keeps pushing this subject because they want to work fewer hours for the same money.

      John Ferguson whether or not fatigue played a role in this incident is not the point. There is demonstrable evidence that fatigue leads to delayed reaction time, and impaired judgement. It's true of airline pilots, drivers, physicians, and everyone else. It has been a proven factor in car and plane crashes, and in medical errors. I know if I arrive at the ER requiring emergency surgery, I hope I get a surgeon on her 8th hour of duty instead of her 16th. I sure as hell don't want a pilot approaching 24 hours without sleep.

    Once these planes hit cruising altitude. The pilots set it on auto pilot. These planes fly themselves. Pilots will take over if something mechanical happens, other then that, the pilots are probably getting it on with the hot stuartist on long flights.
