Like during access opening its commen due to fear of perforation complete deroofing isn't happen as a result of inadequate access dentinal map is not clearly visible. You can use a safe ended bur e.g Endo Z bur

CEJ is the most important landmark for depth of pulp chamber
So, mark cej depth on bur, and do t go deeper than that
take iopa to confirm
