    The challenge is finding someone who is this thorough without having to spend a fortune on wasted trips to various specialists that either want to load you up on meds or order a bunch of expensive imaging tests before they ask any questions or even touch you.

    Ive had a tumor (3 lobes over 10cm each) resection from my sciatic nerve. one lobe was im my sacral notch. I was in ontense pain for 15 years. No drugs were prescribed for me amd i suffeted silently. Just before sugery, I had refused to go to pain management clinic because i knew the lumps i had were the issue. The nurse practitioner and her LPN dumped me as a patuent for refusing pain management an hour away. It hurt to sit and driving was torture. It was my orthopoedic surgeon who worked on my knee previously who an ordered MRI amd referred me to a top tier orthopoedic oncologist. They actually listened and I thank God for practitioners that do good work. Bless you for helping people. Years ago, nobody would even look at you because you dont have insurance. I suffered for 15 years. I was my own advocate. Keep saving people from surgery.

      @ Valentina: I've lost almost all faith in allopathic medicine. I had an orthopedic dr tell me he can only work on 1 body part at a time. How does that old song go? The hip bone connected to the thigh bone the thigh bone connected top the...I had a bad knee. I asked the ortho to check my hip...nope. went to a chiropractor put my legs in the air AND said I think it's my hip, my foot was turned out. He said yep. Adjusted my hip and my knee pain went away. Go figger.

      I went to a foot doctor, major problems with both feet and he would only check one foot but then he wanted to make orthotics for both feet yet he hadn't even looked at the other foot; needless to say I never went back and never had them done by him, best orthotics I ever had done were by a PT.

      @@donnebonne after a bad fall I had to see a hand doctor but I also had a problem with that same shoulder from the same incident but no, I would have to go see someone that just worked with shoulders because he was only a hand doctor, unbelievable. Same thing with a well-known local clinic which will not evaluate your hip and your knee problem, you have to see a hip doctor or a knee doctor, talk about follow the money!

    The problem with these many specialists is that they only know the part they specialize in. So if someone comes in, all they can say is if it ISNT caused by their specialty. They know nothing else to have the proper person to send you to next. Meanwhile, you’re paying hundreds in copays to get nothing accomplished. This PT understands ALL anatomy. Which is how every practitioner should work.

    Being a retired RN, I have to say that the fact this gal was scheduled for back surgery is scary but should not be considered unreal. Sadly, it is very real! Plenty of less fortunate folks get the surgery, have no improvement or are worse. Each specialist knows a lot about their one speciality and little about anything else. This man doing this thorough exam has the tools/ knowledge to pinpoint the problem, causes and ways to manage it. Clearly, someone like him is the correct person to see.

      These loser drs refuse to thoroughly exam you I keep telling them my exact symptoms and they won’t listen. I’ve been disabled from this since 22 years I’m 44 now still no answers. I’ve been to at least 200 drs 40 MRIs of my hip I keep telling them it’s not my hip. I’ve had them tell me it’s time for psychological help and maybe I’m an addict. I suffered 17 years until I asked for pain meds I take 2 a day. My groin snaps 1000 times a day so does my ankle. My left buttock feels so tight and pinched I’ve literally become legally insane from the burning pain and tightness. Sitting is unbearable, laying, driving, carrying groceries or laundry ect. I know it’s 1 of 3 things either snapping hip syndrome, piriformis syndrome or SIJD. I know the only way I will get well is with an expert that cares to look at everything like this Dr in this video. These drs spend 2 mins with you and walk out they are all abusive, neglectful pieces of 💩 This dr in the video is doing like dr house does I don’t understand why drs won’t do what he’s doing. All they have to do is listening to the patient and it will lead them to the answer.

      You are just passed around and I am over 70 yrs now and low Leucocyte count, and am tired. They first called it Larger Granular Lymphocytic Leukemia" then when the White count went to 4000 they were not so concerned, But before it was in the 3000, and they were more concerned, when lower than 4000, not as concerned by fact can catch any infection easily. Plus, exhausted, and do have symptoms even though they may think you would as of yet. But also have a cyst on pancreas that has grown in last six months and they must test me again in 6 months. (If cyst bursts and keeps growing or any of these things, it could be bad ) If it gets too large, they will need to operate. But it may metastases and will be cancer. I do not want to spend my ending years having surgery all the time, This started when I was age 50. Had t get done after working at a few jobs and the one I had before i held for 20 yrs and some other places over the years, I worked, but after a while it was getting worse but nothing works although I keep having surgery as am searching for some kind of help , I must stay independent as no family close enough that can help, and have my husband but he is older than me, and he has some problems now too but nothing this way, It is terrible to have surgery, but you become desperate for answers as you cannot handle pain all the time and do not want to end up ni a home , if possibly , do not need to but I certainly cannot do all things I used do . We own home and must pay people if the day comes i am alone to snowblow or clean up leaves or mow lawns and so much more and really never dreamed his day would really come and not happy to think i am not still strong, When young, it looks so far away and we do not know what all will happen, and it is scary if we ever knew, sometimes,
      Good thing someone would know what can be done! It is confusing when you cannot tell where pain is because it radiates. But you certainly helped to try to pinpoint it out better! '' Thank you!''
      Poor Melanie must suffer with this pain, and it is hard. But also, it is great she now has good help, and this will make a difference. All body parts work together and if one thing is off often other things will go, so it Is good to take care of them, before they get to a Red flag point!

    75 people don't like??? Blimey! As someone who has just had hip revision surgery and gone through years of examinations and misdiagnosis after misdiagnosis, this has to be one of the most thorough exams I've seen. Anyone who comes to your clinic is seriously lucky!!! I'm in the UK else I'd be heading over right now! If only Doctors would learn that the body is not made up of separate parts that have no interaction or synthesis, we'd all be better off but, can't see that happening any day soon. What's the difference between God and a doctor? God knows he's not a doctor!!
    (That said there are tonnes of really excellent, caring, informed physicians out there, of course. But, in my experience too many treat you like a body part and don't listen properly)

      I disagree that God isn’t a doctor, he is in fact the Great Physician. The one who created our bodies knows everything. Sometimes we don’t get healed because we don’t ask, sometimes we ask but we don’t have faith. Perhaps we get our healing but sometimes the answer is no or wait. Do I think doctors are blessings sure some of them but we should always consult God first. He is all powerful and all knowing.

    This is amazing. I’m blown away. She has the exact same issues I do as well as being tossed around by specialists and having various injections. This has been happening for six long years. YOU are the only one to have done a thorough examination. I have NEVER had any doctor, surgeon, PT, etc do these tests. It’s absolutely absurd it’s been suggested she has surgery. I got the same suggestion! It’s disgusting how they don’t do the proper analysis and think that cutting people open is the answer. We need to clone you! I’d really come to see you for treatment if I could! Thank you so much! 👏🏼👏🏼👏🏼👏🏼

      It’s a very common differential diagnosis that PTs come across. All doctor of PTs should be able to diagnose this. Dunno why neurosurgeons can’t learn this- perhaps they cannot see non surgical fixes because of well, they are surgeons.

    Physical Therapists need to be an option to go to FIRST and not just a remedial limited visit option after a surgery. I feel the insurance industry really limits the ability to pursue physical therapy at all.

    This video pisses me off in so many ways. Especially that the patient was about to have back surgery!!! Kudos to this Dr. For caring enough for this woman to be thorough enough to find the problem causing the problem. We need more Doctors like this man. It would help so many people!! I’m having so many problems like this and still no answer. But no provider or doctor has done this thorough type of exam. Is there a part 2 to this? How is she doing now and what was her therapy to get better?

    I can't believe she is scheduled for back surgery after having this for only six months that is insane. I have no doubt that she is in constant pain but any good Doctor would explore alternative treatments and give them time to work before choosing to cut this woman open.     A few months ago I had a chiropractic exam that was decently thorough but mot like this. They just won’t listen or they just don’t have time to listen and do an exam like this. Very insightful.

    I’m a respiratory therapist for along time. The back surgeries that I have worked with has been more on the negative side than positive side of MDs. I agree that a physical therapist should always be first inline for patients to see. It’s always about the mighty high cost of $$ for the Mds and the hospitals to receive in there pockets.     I want to thank you so much for your videos. You helped me so much with my pain. I was freaked out and in a state of panic and didn’t know what to do. After watching your videos, I took your advice and followed your diagnostic stuff and realized that for me, it was mostly a posture issue. I started sleeping with a pillow between my knees and found perfect relief, and got a hold of the panic and feel so much better. My pain is almost gone, and I am paying attention to the way I sit and move, and things I do that aggravate my back. I stopped crossing my legs when I sit. I don’t sit sideways on my couch, and I pay attention to how I lay in bed. Thank you from the bottom of my heart. I could never tell you how much I appreciate all that you are doing.

    Thanks Doctor! I feel people can be confident seeing you. I ran through some of these movements and am still fairly confident I am dealing with an SI issue. I am a teacher and mine only worsened after extended sitting at desks or standing in one place. It actually improves after brisk walking and now three weeks later, it is not so much an issue as before.

    Good work distinguishing between back or hip issue. I would have liked to see you identify which particular muscles are causing her hip pain and recommendations for rehab. It would have been great to see identification as to whether it is Iliopsoas, SI joint instability, piriformis syndrome or some other structure within the hip. Can you make a video specific to muscle testing for the hip? Thank you

      Maria I agree with you completely and it's what I found lacking because we don't know the cause of the pain in the hip. Is it the piriformis muscle or sacrum instability or what else could it be? ONCE that is determined what's the solution ?
      mtaybe he plans to work with her and determine that but I wish she could do a sequel. WHAT is the rehabilitation that would be involved?

    I have been suffering with S1 joint , had injections to burn those nerves. Did not work.
    I’ve had 2 back surgeries and back infused with rods. This has been very helpful. I like the way you explain

    I went to the Dr yesterday for horrible sciatica pain going down the back of my leg. I self diagnosed thanks to the internet before by doing self test at home from physical therapy vidoes on TH-cam for piriformis syndrome. The Dr didn't do any physical exams at all just a few x rays and gave me a shot I believe it was a steroid injection in my but and told me I will need physical therapy. Wish you were close to me would come to you for sure!

    Brilliant video, well done, I recently saw an Orthopaedic surgeon for my lower back (hip) pain, no real physical examination, said I had week lower back muscles, showed me some exercises to do, gave me some pain blockers and sent me on my way. That was a week a go, been doing exercises, didn't take the pain blockers as I want to know if the exercises he told me to do are working. That said, I decided to squat yesterday, all the way down like some Asian people do when they eat, all of my pain was gone for the rest of the day, first time I had no pain in months, however this morning, it's back, the pinch in the right buttock, i.e. every time I walk, I am ok when I sit, but then the pain can move to the side of my right thigh. Was also told to do an MRI scan which would tell me for certain what I have, now that costs about a grand and I don't need it that much, suffice to say, if the squat stretched those muscles and the pain went yesterday, I think I am onto something, so will try that again another day. I believe it's the hip, but who knows, could be a perforated disc in he lower back. Growing old sure is fun.

      Sounds like ortho doc didn’t do a very thorough exam. Sorry to hear. Use movement as medicine (your squat to start for the next 4 days so long as it doesn’t feel yucky) optimize the dose, frequency, and timing and stay scientific.

    Great demo! Investigation / diagnostics of all parts in complex systems is always needed to find the source of the problem, weather it be a car issue, a computer system issue or the human body! From my past experience in the human body, our parts sometimes have an amazing way to heel themself, unless there is clear signs of damage.

    Wow oh wow! This is my chronic pain all the time. I go to my chiropractor which has been the only doctor who spends time to figuring it out . I drive for my job so I’m in that same position for hours. I’ve been going for the last 17 years. He has many modalities to work with in a bad flare up

    More items to consider:pain in one locstion may be referred from the actual source. For example, i had low back/SI joint pain, which turned into severe hip pain. After years of treatment by chiros and other doctors that recommened only chemical pain management or surgery, infounf a pyhsical therapist that actually csred about the situation and genuinely trying to fix it. After experimenting with differnet areas, she determined that all my issue were somwhere in my thoracic spine. Turns out, i had a diplaced rib that was so stuck it took weeks to gently free it up and get it moving again. Once she was able to free it, all my pain was resolved. The bottom line, i dealt with about 10 DPTs who just said , "ah, its an elbow problem" or "its because of your job"or "its your neck". Find a doctor, DPT, naturopath, etc, that is willing to find the root cause of the issue and correct that instead of treat symptoms and prescribe medication. If youre not improvimg, someyhing wlse needs to be explored. And only accept surgery on your spine as a last resort, like, your junk isnt working, you cant hold your bowels, or you cant walk.

    Hello, so glad I found your channel. I was beginning to lose some hope. I see my own doctor yearly, and if anything new occurs she’s the first to know. I’m a 66 year old teacher, and about five years ago my class went on a hiking trip to the Santa Monica Mountains. I prepared my class for a somewhat moderate hike. Well, it was a bit more challenging than the hikes I’m used to. It was up and down very small paths. Well, I was standing taking picture when I started the climb back down. My right leg felt very weak, and it just gave out on me. I fell several feet down, with scared students watching. Good thing parents were there, as they helped me up. I still had to hike the half mile back to the bus. It seemed to take all my energy just to return. I wasn’t hurt, since all my life I’ve been active in roller blading, runny, and hiking. I just felt very tired. Well, a year later, I was with my niece and we were jogging back to my car when I noticed a low barrier between the stores. She jump first, then I did. The odd thing was my right leg didn’t make it over the barrier. It wasn’t even that high. Must have been like just a foot high. Well, my time for retirement came up, and I noticed that the stairs leading up to my class were becoming more difficult to step up. That’s when I noticed I seemed to be having trouble with balance. It seemed harder for me to navigate a typical walk around the yard. Like I felt uncertain about my footing. I took it in stride since I was also known for being clumsy. The last thing my doctor said to me was it seems like it’s a sciatic problem. So I was given some pointers on exercising my legs. Yet, the problem never seemed to be with my back or my legs. It was with the walking and balance. I also noticed some tickling in my feet, both of them. I don’t drop things, nor are my arms affected. Now, I bought a cane in case I have to walk longer distances. I hear so much about it being my sciatic vs something like MS? This is the point where I’m at.       Physical Therapy & Johnson well you’ve got great content. Please forgive my forwardness, may I suggest you look into hiring a marketing person or company. You’ve got great presentation & content but you’re missing the huge audience that you deserve. I’m sure you’ve looked into it by now and considered the cost but I believe you’d make it back fairly soon. *I’m not promoting anyone.
      As most companies are suffering you maybe able to negotiate a better deal than previously or find a new freelancer? Stress and lack of activity is destroying our bodies and your services maybe more in need over the coming months.

    I have exactly what Melanie has!
    Pain in the left butt to the shin and around the ankle
    The pain is not constant but it does flare up often.
    While I can manage the butt, the pain in the shin and ankle becomes unbearable
    I have stopped stretching and as i am living in a condominium 17th floor, i am walking down the stairs and coming back up in the elevator at keasr once per day.
    I will try this and watch out for the outcome.

    So glad I found your video I have the exact same symptoms she does. The Motions you ran her leg through are exactly the same motions that cause me pain and in the same places as her. I have been struggling to figure out if this is sciatica a herniated disc or something actually in my hip or buttocks muscle. I've been dealing with this for three years and it has totally ruined my life and just like her it seemed to have occurred overnight. I to sleep on my sides mostly so I will start trying to sleep on my stomach and back. As far as Crossing one leg over the other, I could never do it with the left leg but no problem with the right. I'm sure mine has something to do with osteoarthritis since I have it all in my feet, hands wrist and my right knee. Anyways thanks for the info!

    Wow! I am so glad I saw this video! You have truly done a good exam,
    I have had several fusions on my back and a knee surgery and one hip surgery (right) and then the Dr said (Orthopedic) "are you ready for the left one now?" Well, it is osteoarthritis they say, and yet it is one surgery after another, and I am so tired of it. The first back surgery in another hospital went badly (On good Friday yet) and they did not have a Titanium rod and called a distant hospital t get it and I stayed under anesthesia most of day. They told my husband" If we touch a nerve, she could be paralyzed from the hips down" Well it ended up they put a Titanium cage in left side, and the rod in right side of back or spine, and had to get a 'full hard body brace' and not twist ,turn or bend for months, and the brace was placed on to go to bathroom and after awhile showers and believe me for someone always caring for others, it is not easy to have anyone take care of you!
    Then got MERSA STAPH infection and almost died! They admitted it. Well, anyway, did go for another surgery many years later, and was afraid, but now I have they say, the Left hip needing surgery and someday the right knee, with total knee replacement. You cannot have same orthopedic do all he surgeries as they all specialized in different parts of the body!
    I am in severe pain all the time and could just give up, but because I look normal, people do not understand. I find I bend forward and it is hard to stand straight, and pain wears me down.
    The funny part of this video is that this young lady , Melanie, had the EXACT same thing on the same side and my Primary said to me " Are you sure it is not your disks in your back?' Now this confused me, as they told me the 'left hip' had to be done, and I truly needed that surgery right after the but now I must do something, as it is 'sacrum Iliac pain and then down left side and right side and the left is worse, and stiff and cannot sit long, walk long or sleep long, 'And when I do, I keep pillow between knees. I guess I got out of this it probably is the Left hip. But you know it is terrible to have surgery all the time. Scary after that MERSA scare! They cannot promise you anything and if you die they wild say they are sorry to your family, and this is it. I did not sue the Orthopedic when I got the MERSA and he admitted I was just "Lucky and must have had an angle with me" but I was feeling sorry for "him" Never knew my life would be like this, and need relief and it never ends no matter how many surgeries it seems another thing will then happen. (I was so active most people could not keep up, and although I look alright, my one friend admitted " you are not the person you used to be." Now I am not able to keep up!
    One disc goes, and then it makes another one go, and one hip is done, but the other one needs it, and then one knee is done but the other one needs it! Now am having my feet done, as of' hammer toes' and not sure what else yet? But walking would help with feet in good shape as well. Have been confused if it is my left hip, back, or nerves pinched? Wehn my primary through in the spine it reminded me the other Orthopedic did mention another one was going buy fir got just which one. They would need to read the reports on this first it seems or do tests. Mostly is with MRI and X-rays.
    Wish I knew what you decided she would need to do for this pain (I know you mentioned the pillow between left but I do it and must already) and how much it would take to get the pain to be gone, or can anyone "ever totally be relieved of this pain, if not with surgery?'' Have never been examined like you did, with Melanie.
    Wish you were not so far from me, but the distance is a problem, for any help. Am in the Northeast U.S.

    I decided about 6 months ago that I needed to stop crossing my legs period... Habits are sooooooo hard to break. Much less sciatica pain! I have always slept with a pillow between my knees side lying. Out under my knees while back sleeping.. Learned that decades ago after a patient caused back injury. (Yup, retired RN)

Thank you both for publishing succinct explanations about proper diagnostic exams. Now we know when our healthcare is giving us our moneys worth. I disagree that arthritis is normal. I believe we should look hard at the diet when arthritis starts. Dr. Jerry Tennant covers this well talking about stomach acid. There you go I have returned the favor with more information.

    I can’t tell you how happy I am that I found your channel!! I’ve tried everything for 5 years, been to every doctor under the moon, chicropractors, all kind of specialists, tried all kind of treatments, surgeries.. This pain is ruining my life!! The newest diagnosis they are thinking off is bursitis.. I’m gonna watch all of these videos and hope I’ll be able to find out what’s wrong myself, be my own doctor and find a “cure” 😅 Thank you for helping all of us people with pain in our ass 😂

    As an LMT I have been working with a client who has sciatica issues on his left. Pain mostly feels like tight inner thigh/groin and lower back glute med muscle areas. What I have found after an assessment....and this could be her issue. His inner thigh muscles and glute medius compensate for his over stretched periformis. His bad hip and periformis has way more flexion then the good side. An overstretched periformis from her sleeping that way is causing the issue. Totally agree on the pillow in between the legs at night. His homework are exercises for shortening and strengthing the periformis. Something else to consider.

      He should really use that as his logo - shorten and strengthen because so many people, myself included, would rather hear "take this drug or do this surgery and then done". I watched this to the point of "she had no tingling", boy do I have tingling. I always had a weak core and a strong back, alternated with lifting and sitting, and of course not stretching enough. The kicker was his video on heat and cold. I put an ice pack on my lower back, and felt all the muscles down my leg unclench. So I'm on strengthening and my pain has already decreased by about a tenth. My problem is I may have pulled a muscle from overdoing it. You can definitely overdo strength training. But I definitely have a herniated disc, the pain at first was unreal. I'm still not sitting for very long, but am only two weeks in, and at least now I'm sleeping unlike the first week when I had no idea what was wrong with me. The big change is of course I really had slipped into some bad diet and lack of exercise habits and have to get back to getting healthy.

    Professionals that can so elegantly isolate pain causes for individuals are rare. I think the training just isn't geared toward helping a person help themselves (changing their habits to improve their life).
    In high school cross country, I developed severe hip pain. I went to the doc, a chiropractor, and a physical therapist-- none of them could help, just passed me on or gave me a one-size-fits-all treatment. On my coach's recommendation I went to the high school's sports therapist. He listened to me, compared my hips, and WATCHED ME RUN (none of the others had done that). Then he taped my foot before practice for a week, to change the way I rolled off my toes. That took care of it.

    Thank you Dr. Johnson for a very honest and thorough examination of Melanie. It is very refreshing. I am 75 year old woman who has been diagnosed 7 years ago with bone on bone ostheoarthritis on my right hip. Been told twice to get a hip replacement but I refused because I manage my situation well. I go to the gym before COVID started. But recently I am experiencing so much pain (not from the back) but on the lower thighs and legs. I went to a chiropractor for some relief but they said it will take time. I am waiting for my doctor's referral to a PT. I watch videos for exercise recommendations. I also go for accupuncture. I live in San Francisco. Appreciate your help. God bless you doctor.

    Brother i had a spinal fusion in 2019 and now im dealing with severe nerve pain in my left hip. This video is very helpful. I can sympathize with this young lady. The pain is twice as bad as when i had my spinal fusion. I can barely walk or dress myself. Acupuncture seems to make it worse. The stimulator gives some temporary relief. I cannot do any of these exercises. I'm not looking forward to a hip replacement but i think that will be the doctor's opinion.💪😑👍🙏,

    I have major spinal problems and the best I could get was out of a neurologist who did "group muscle testing" for instance, rather than muscle testing each individual muscle he would test hip flexors, hip extensors, etc. I was an OT for 35 years and I tested muscles individually when needed. And you're right - he never asked me to move an extremity or did he move it passively to test where the dysfunction was🤔

    Great video - a big thing that helped me in sorting out my issues is look at what u are doing when u are doing “nothing “ . Those passive exercises u do for a long time , a lot of driving ? Leaning on one leg ? Lay on one side when watching tv etc ? Could be the key to muscles getting used to one position and adapting

    I can’t believe it! My problem exactly!!!
    Have no idea which of the 3 is my problem. Been to my chiro 3 times. So far no improvement. He uses an activator as I’m old & have osteoporosis. Went through your motions. It’s my left side. When I bent to the left, I was in acute, major pain - mostly left butt & left side of calf. Have to strongly poke my left butt and walk which diminishes the pain. The calf pain is always there like a toothache. Walking is the fine, sitting with good posture is fine, sleeping is torture, especially turning over! I think my issue is SI joint/piriformis. “It” is totally unstable. No tingling or ankle issues.Guess I’ll try PT. I’m in FL. But thanks. Have a labels tear I my left hip but I dealt with that earlier this year. Took 3 months!
