I am an internist and my cousin is a dermatologist. I went to do a rotation with her during medical school (she entered medical school at age 26, I entered at age 38 and we are the same age so she was in practice for a decade before I started training). One patient of hers we saw had diffuse sun damage. I was studying cardiology at that time and I exclaimed when we left the room, this patient has SKIN FAILURE (thinking of a heart failure analogue). My cousin said "YEAH, EXACTLY - I am going to USE that!" Through the years we have thought of the outcome of excess sun exposure as skin failure. Today due to you two I now have a new word to describe the lifetime etiology. "Exposome." I am going to USE that. Thank you!

Maxfield and Shah to answer in more detail as they are the dermatologists, but what I consider to be symptoms of "skin failure" would be diffuse sun damage frequently leading to brown age spots (like seborrheic keratoses) , fragile skin of age with chronic breakdown - the actual medical term for this is "senile purpura", appearance of numerous skin cancers. It is one of those things where "you know it when you see it." This is the kind of patient who is frequently seen by dermatologists and tends to have had significant sun exposure over a lifetime.

Vichy probiotic fractions is great for oily skin. But for those who have dry to super dry skin, I think a more affordable option would be the La Roche-Posay Lipikar balm AP+, which also has prebiotic thermal spring water, niacinamide and vitreoscilla ferment.
I have both and they are not the same, the vichy is a serum and the probiotics are higher up in the ingredients list, I use it as a first step with great results.

Yeah, I heard about that a couple years ago and do it every once in a while. Maybe if I did it every time before bathing I'd see or notice a difference. Doesn't irritate or anything though.

There’s another certificated dermatologist on TH-cam (Dr. Dray) and from her videos she says that they are NOT effective in boosting collagen, there other ways to boost it

Actually this Vichy probiotic fractions would be good for all of this and also put your favorite cream on top. Tolerance Control cream from Avene is good but la Roche Posay makes a good one too called Toleriane Sensitive Cream.

I use only tretinoin at night. I apply all other skincare in the morning. My morning moisturizer does not contain niacinamide. I want to add niacinamide, so my question is, could I use this Vichy 89 prebiotic at night with tretinoin cream? If so, would I apply before or after the tretinoin?
I live in Canada and snowboard all season round, our winter is 6 months long, and pretty brutal. I use Dr Jart Ceramidin cream after the sunscreen. That cream is bullet prof from our weather.

Vichy 89 is theee BEST serum I've ever used! It actually reduced the chronic redness on my cheeks and chin due to sun damage.
Such an under-discussed part of many folks’ exposomes is chronic illness (especially autoimmune and immunological conditions)

(Moving to Scandinavia to Africa lvls without any sunscreen and My mom Even teased me Ford years that I burnt My face in cloudy Day 🙄...i still love her... ) at forties I still do Have terrible redness/couperosa, hyperpigmentation and Even rocacea around My nose and cheeks. Im trying to learn How to Fix it My own, 😄 Here in Finland its so hard to get doctor help problem wich is consider "Esthetic problem".. by The way, we Are not mist happily people in The world, we just lie to those studies...😄

I use only tretinoin at night. I apply all other skincare in the morning. My morning moisturizer does not contain niacinamide.
I want to add niacinamide, so my question is, could I use this Vichy 89 prebiotic at night with tretinoin cream? If so, would I apply before or after the tretinoin?
I live in Canada and snowboard all season round, our winter is 6 months long, and pretty brutal. I use Dr Jart Ceramidin cream after the sunscreen. That cream is bullet prof from our weather.
