I never actually watched the movie, but I remember going into a blockbuster with my mom and seeing that cover and having nightmares. I’m quite happy to know that a pathologic fear from my childhood stems from a Shane Dawson movie

Back in 2012 (about maybe November-December) the movie reached Russian Theatres, and I remember how I walked near the cinema, being a proud 9 year old dumbo, who just discovered creepypastas, and threw my sight on the covers, to see what's good in cinema. I think that the fact they got the whole cast to condition themselves to say "Smiley" without cracking a laugh or smiling is the scariest part of this film.
True ending: Pyro does a chad move and uses his kinks to defend himself, and we finally can watch his videos in peace, even though we now know his secret.

It's because he actually understands Trisha's BPD. You and most of the audience doesn't. it's not something she's able to control (there's no cure, only treatments that take years to show progress, which she's doing). So there's no point in Dr Drew lecturing her on her disease. It's like telling someone in a wheelchair how to walk. Ethan can change his behaivor, trisha will always have episodes like this. Not saying it's Ok, just that that's the reality.
