thyroid scan is functional nuclear medicine study used to assess the activity of the thyroid.
other names -Thyroid scan (Tc-99m) ,technetium scan ,Tc-99m pertechnetate thyroid scan, scintigraphy
Earlier thyroid scans were done with radioactive iodine. It is replaced by technetium (Tc- 99m) scan
Advantages - chaper, lower radiation dose , unlike iodine, pertechnetate anion is not incorporated into thyroid hormones., short half life 6 hrs so rapidly eliminated
it is done in nuclear medicine department of hospital and no hospitalisation is needed
No need to enter inside any machine
it is Not a primary investigation.
OTHER test- thyroid function tests (TSH, T4 and T3, thyroglobulin, TSI, TPOAb), ultrasound
    Indications
Size and Shape of Thyroid, To know Thyroid activities
    Differentiate from Hyperthyroidism and Hypothyroidism
    To know cause of Hyperthyroidism
Differentiate from benign and malignant nodule
    After thyroid surgery
    Metastatic of thyroid cancer
    Retrosternal goiter
    Contraindication
Pregnancy ,Breastfeeding women ,Allergy to Chemical, History of CT scan requiring IV iodine contrast medium in previous 6 to 8 week, medicines continued like lithium and amiodarone
    preparation before thyroid scan
low iodine salt, low iodine diet, avoid iodine supplements
stop medicines like levothyroxine, liothyronine, carbimazole and propylthiouracil, amiodarone, lithium
if you had (CT) scan IV iodine contrast than wait for at least 6-8 weeks.
check if pregnant or if you are breast-feeding.
if not sure do urine pregnancy test, beta hcg on the day of test
How does the test performed ?
fast for 4 hours prior to exam
    carry medical file and documents
    carry medicines
wear loose clothes and half sleeves/ sleeveless.
    to remove neck jewellery.
expect the whole process to take 45-60 minutes
A nuclear medicine specialist may wish to examine your neck
body position - lying down on a bed and the camera will be positioned very close over your head, but will not touch you.( gap of 20cm )
If u can't lie down than sitting can be advised.
sodium pertechnetate is injected intravenously into arm and wait for 15 mins
during this time it will concentrate in the thyroid gland.after that images of the thyroid are obtained with a scintillation camera . radionuclide emits gamma rays (photons) at a predictable rate.
The camera is set to detect a predetermined minimum number of photons.
Images are usually obtained from anterior, left anterior oblique and right anterior oblique projections Two anterior images are usually taken. camera may move slightly during this time. It is important you lie very still and try not to swallow during this time, as movement will blur the pictures.
    What is the radiation dose?
Radiation dose is measured in units of millisieverts (mSv). it is approximately 3.2 mSv.
no known associated risks. The dose of radiation you will be receiving is relatively small
    No hospital stay
After the scan they will be able to drive and resume a normal diet and activities.
    Increased uptake-- Graves disease,Hashitoxicosis
Normal uptakeHashimoto disease, following I-131 therapy
Decreased uptake--excess iodine intake, amiodarone, high iodine diet, iodinated contrast, subacure thyoiditis, Struma ovaii, Hashimoto disease
Abnormal site of uptake- ectopic thyroid, lingual thyroid, Retrosternal goiter
