    We also got taught the mnemonic: 'Vanish DDT' which stands for vertigo, ataxia, nystagmus, intention tremor, staccato speech, hypotonia, dysmetria, dysdiadochokinesia and titubation.

    I have seen a lot of people do that rapid alternating movement test, and this guy is, by a longshot, the fastest.

    Scanning speech, nystagmus (fast phase towards lesion), dysmetria (index to nose and finger to nose test), arm rebound test, dysdiadochokinesia, pendular knee jerk, titubation, cerebellar gait (broad stand, staggering, towards lesion). Romberg unrelated (just proprioception altered).

    I would like to thank this great Dr .. for sure he is great in teaching aswell ,, tomoroow am taking an osce exam just wanna refresh just I came across it
    first time to get all of cerebellar sign and symtpoms this way although I used to master it when I was student
    very will done
    he is so confident and relax and takes things by reasoning

    Reminds me of good old days during the Internal medicine rotation at Trichur Medical College, India where our brilliant but unassuming teachers demonstrated physical exam skills to us at the bedside every single day. Thank you so much, Dr. Verghese for bringing this art back to medicine. Is there a way we can extend this to pediatric practice?

    I really like this doctor's presentation.
    On the other hand . . . does every video like this need a musical soundtrack? Now I have to process not only speech, sights, the sounds of some of the movements, the implications and connections of what's being described, and some new relevant information-but also music? This is what my brain needs?

    Our baby at 2 years old is diagnosed of Cerebellar Ataxia. We found it very difficult to do the therapy considering his age. He used to be a very happy and energetic kid. Hope you can help us in his condition. A little advise and prayers will do.

    I believe the romberg test also checks the vestibular system and not only the posterior columns.

      The cerebellar atropy gets more and more worse with time. My dad is suffering from it since last eight years. Could you please suggest any medicine or treatment therapy to control it? Video on it would be very helpful for such patients worldwide.

    Very informative! My nursing assessment text says a positive Romberg test is seen with vestibular dysfunction and cerebellar disorders. It does not go into detail unfortunately.

    thank you for sharing you knowledge.....I would love your review of Dr. Titus Chiu's book and how you feel about post concussion syndromes. I had a head injury before I was married. It was not considered a problem until I began exhibiting acute mood changes and experienced seizure type acitivity.

    Ataxia can too often be linked only to the cerebellum and so the confusion is made. It is SENSORY ataxia (peripheral neuropathy) which can be indicate with a positive Romberg test... nice mention :)

    Nice video, thanks. I think it is better to change the depth of your finger in relation to the patient (near then far from the patient) when testing for dysmetria rather than changing the direction of your finger. The idea of finger-nose testing is to see if the patient is judging well the distance (depth) of the object (your finger), so in case of cerebellar disease, the patient will past-point, meaning he/she will past point the depth of your finger.

    I thought the fast beating of nystagmus is usually pointing toward non involved side, and slow beating phase is the side of lesion.

    This may be a bit off topic, but nevertheless has to do with neuroligical tests. I was diagnosed with moderate to severe cervical spinal stenosis based on MRI, I also have disc herniations, degenerative disc disease, and bone spurs. I went for a 2 cd opinion and when the neurologist did a series of tests, found that I have pronator drift - stroke was ruled out ( I have no symptoms, my BP is fine, etc.) but this scares me - could it be from the stenosis? I also have cervical instability. I am also being sent for a CT scan because the Dr. is very thourough. Should I be very worried? I am 59, in otherwise good health, very active, exercise, and eat nutritiously.

    The Romberg test went from having nothing to do with the cerebellum to having very little to do with the cerebellum in less than a minute. 4:00-4:36. Which is it? ;)
