A specific passage that inspired the song is: "How could a man lose as much of himself as I have and still live? If I'd read about a guy like me in the paper I wouldn't believe it, cos it's a million to one. But a million to ONE always leaves one. The song is about a soldier who lost all his limbs, his sight and his hearing when stepping on a landmine and all he can feel is pain. He simply wants to die. The video material is from a movie called "Johnny got his gun" and to my understanding, Metallica bought the rights to this movie just to be able to show the music video without having to pay royalties.
Kid grows up and goes to World War 1, loses all limbs, face and senses, is so, alive, but trapped in his brain, unable to communicate anything (can only move his neck unconsciously/uncontrollably). A mercyful nurse discovers, that there´s a spot at the center of his chest, where he feels when someone touches him, and so, a way for communication opens. She is able to start communicating with him (through Morse-code)... At the end of a conversation with her, and a doctor, his words are: "kill me...!!!", but Christian morale forbids them that, so they simply lock him up in a dark, windowless room in the hospital´s cellar (for cost-reasons), till he eventually somewhen dies, decades later, totally forgotten (of course, they always pump some nutrient sugar/vitamin-solution in his body, or else he´d starve).
