Are There Limits to Exercise or Activities After a PRP Treatment?

  • Hi there, it's Dr. Grant Pagdin. I wanted to address a question that we sometimes get asked after PRP treatments, and that is are there any limits to my exercise or my activities? For instance, if I had a treatment of PRP to the face or the scalp would it be safe to go out in the sun? And if I had an injection of PRP to a tendon or ligament or into a joint, what can I do in terms of exercise? So in terms of the cosmetic treatments for the face and the scalp, no restrictions going into the sun, the only restriction would be, we ask you not to wash the area for 24 hours. If you are going to be getting a little bit sweaty, just don't shampoo your hair or wash your face, just let the platelets sink in and do their job for 24 hours.
    With regard to the orthopedic injections, say for joints or for other soft tissues, there are some restrictions with exercise. I specifically ask my patients not to be doing high impact activities like running and jumping, or striking a ball with a bat, or hitting a golf ball with a golf club, or chopping wood. Those kinds of impact activities could interfere with the platelets a they are fairly delicate. The platelets can survive in the area that we inject them for up to seven days, so this restriction against high impact is really just for the target joint, and just for seven days after an injection of PRP.

      There’s a recent article in Joint Rehab suggesting that PRP can do exactly that: strengthen the capsule and supporting structures to help prevent recurrent dislocations. I’d like to see more long-term follow up data but it sounds like a reasonable approach. See:

    Hey Good morning,
    Hope all is well, I got prp to the scalp yesterday around 6pm and went to work the next day and sweat a little around the areas where I got the prp injected. My only concern is that because that happen will this make the prp less effective?

    Thanks Dr Grant, what about alcohol and caffeine? Specifically for the scalp. My doctor never mentioned anything besides not washing my hair, but I’d like to get a second opinion

      Alcohol may have a mild inhibitory effect so keep it to a minimum. Some providers recommend zero for 2-3 weeks after PRP or stem cells. I’m not aware of any influence of caffeine.

      Yes, if you had PRP to the shoulder you can do leg exercises with no restriction

    Hello doctor, I just had PRP for my l4 l5 disc bulge 3 weeks ago and my pain has gone like 60 percent but i feel tingling if i sit for long. After how long can I go for physio? And I would like to know if this back pain will be for lifetime and I'm 23 now.

      PRP can certainly heal your disc, provided it is concentrated enough: remember, not all PRP is created equal, and the disk is a very small space so the actual dose of platelets delivered is very important. For a disc, the PRP should be 10 to 15 times baseline. If you injection was three weeks ago, you should start physiotherapy now.

    After 3 years going to my GP for severe hip pain, going to physio and massage with little relief (and been on a nearly one-year waiting list for a hip replacement) I went to Dr Pagdin 6 weeks ago for PRP hip injection. There were good days and bad day for a couple weeks but now I feel 70-90% better. My neighbour who has witnessed my pain over the years came over today and I burst into jumping jacks to show him the improvement. THANKS Dr Pagdin!

      Yes, for certain conditions PRP can provide a complete “cure” - for instance, with tennis elbow we get excellent (permanent) results most of the time.

      No, most of it is getting injected just below the surface. We recommend not washing your hair for 24 hours, but you can’t help sweating a little!

    I partially tore the medial part of my plantar fascia in November. Didn't know it at the Tim, kept walking on it and finally went in for an MRI in March, 4 months later. I did prp and partial weight bearing a few weeks. My doctor never seen a tear before, but it's been 4 weeks now. Should I transition to a shoe now? my foot feels like jello when I tried to walk. how many prp would you recommend for this injury?

      For an injury like that, you may need 2-3 treatments of PRP at 4-week intervals. Work closely with a physiotherapist until you have full recovery

    Just had prp injection around 6 weeks ago on my elbow for the tennis elbow .. I have been doing cardio like 20 mins Eliptical and 10 mins running , is that ok ? .. how about push-ups ?

      Yes, at this point you can reintroduce push-ups and resistance exercise. Your elbow should be close to 100% better by now. Avoid any repetitive activity that causes pain

    Hello Doctor. I had PRP into my glutes tendons. it is the number nine after the procedure and I feel like pain is increasing every day is this normal?

      Yes, immediately after the injections it can be quite sore because we are sticking fluid into an area that is already “angry” causing the tissues to stretch a bit. Once the platelets start to go to work, the pain should settle, but usually still fairly sore for 2-3 weeks. Maximum benefit takes about 4-6 weeks in my experience, so be patient!

    Hello doctor I've had a PRP on the scalp today while I've been hearing on the internet to avoid caffeine my doctor never mentioned anything about it other than avoid washing it or consuming alcohol what's your opinion on the matter?

      If your arch is adequately supported in a boot, then cycling would be great. Are you wearing a custom orthotic?

    My partner had lower back prp injections month ago and is now supposed to do excercises, next doc visit is after another month. But the pain hasn't gone anywhere and now he cant even stretch, the pain is different now and feels like he will snap his back or loose feeling in his leg if he stretches a bit more. Is that common during recocery?

    Hello, I am after my 2nd PRP to the knee. I am a fitness guy, and doctor told me that after 3 days, i can exercise anything except legs. I have some pain after i did stretching including being on the knees, does it mean, healing process of the first 2 injections is lost, or It my knee is just irritated ? ... I am guessing if I can go to the gym and do the exercices while sitting or should I stop gym at all for whole healing process. Gym is very important to me, but dont want to destroy the healing process ... What do you recommend ?

      I recommend avoiding running and jumping for the first week after PRP, but I certainly encourage range of motion exercises and adding light resistance as comfort allows. If you are having pain, back off a bit.

      Yes and yes. Just avoid high-impact activities with your hand like golfing, hitting a baseball, or chopping wood.

      Expect to be sore for about 4-6 weeks total after PRP; ok to start doing some light resistance exercises now, just avoid activities that aggravate pain

      Stretching only for 2-3 weeks, then work with your Physio to introduce some gentle plyometrics (light jumping) before you transition to running at about 4-6 weeks

    I had prp in my foot for a 50 percent plantar fascia tear . I am unable to bear weight on it after 4 days. Does this seem normal? Do you recommend non weight bearing or boot ? How long after prp?

      Be very careful with the plantar fascia to allow it to heal: no barefoot walking!! Must have good arch support and start with partial weight-bearing only. Expect the PRP to take about 4-6 weeks for maximum benefit.

      Focus on range of motion first, then add light resistance. Suggest the supervision of a sports therapist to optimize your recovery. If the tear is small you can expect virtually 100% recovery

    Was going to start walking and swimming and lifting up until that 7th day and then start running.

      Walking and swimming are great but I would be cautious with resistance and running for 2-3 weeks. Recommend working with a physiotherapist and be careful not to overdo it!

      Best advice, Dan, is to work closely with a rehab specialist (Physio, kinesiology, Physiatry) to optimize your recovery. June 24 gives you 5 weeks which is realistic.

      We tried dry needling,
      Hypothetically tried Bpc-157/TB-500
      Tons of Vitamin C, Magnesium, Calcium, Collagen, Zinc, Vitamin B’s.
      Protein powders.
      Been rehabbing and resting for a month.
      Had a couple runs on the alter gravity.
      Felt slow but good recovery.
      I had gone to get PRP since I’m on a time crunch.
      Before PRP I did PEMF and piezo wave therapy.
      Going to start easing into running this Thursday and by the time June 24 comes around I should be good to go hopefully.

      Gentle range of motion is good, even if sore. I don’t recommend ice because it slows down the platelets. Heat would be OK. Be patient, as it takes a while before you start feeling better!

      @@mrtmonthego1182 don’t worry about the sweating. I am presuming you are having a series of treatments, not just one isolated treatment. We typically recommend a series of three approximately 4 weeks apart for best results.

      Different providers may have different recommendations, so please check with the physician injecting your PRP. I’m ok with range of motion and Physio as soon as comfort allows but don’t load with heavy weights in the first 2-3 weeks.

      @@doctor.dr.2512 PRP will help the symptoms, settle inflammation, and improve function but it will not “cure” chondromalacia 100%. You may need ongoing treatment with PRP every 8-12 months

      No, severe pain is extremely unusual after PRP. Most people use plain Tylenol for the first 24 hours and resume usual activity quite quickly

      yes, we often combine an exosome called A2M, which is quite prevalent in the plasma of the patient, which we would otherwise be discarding. We do not add any extra ingredients, except when we use PRP for penis or vagina injections, when I would activate the platelets with some calcium gluconate just prior to injection.
