I think a fun "realistic" add would be make it so if Mario is blocked from walking off screen or from going into the castle, the level doesn't get cleared for upload. Cause Mario doesn't really win if a sping behind the flagpole bops him into lava or a pit or leaves him trapped in spikes with laughter sound effects being triggered.

Goombas always fall off everything, so now we've given them a realistic brain so they never fall again! Also, if there's a stack of enemies that's taller than two blocks, the stack will collapse because the enemy at the bottom cannot sustain the whole stack's weight.
Ideas: 1. You can't stand on clouds, because, in real life, you fall through them. 2. There is blood when you kill an enemy. 3. The Koopa car runs on gasoline and you need a license to drive. 4. Fire Mario burns himself rather than enemies. 5. Lakitus have a maximum abount of spinies. 6. Koopas are slower because turtles are slow at speed. 7. The Moon does nothing. 8. There is no timer. 9. Lakitus fall through their clouds, also you cannot enter them. 10. The Super Bell doesn't work because cats cannot climb walls and dive.
Realistic: Whenever Mario jumps out of water or exits a water section, his jumping ability is more limited because wet clothes are much heavier, also he gets a bad rash on his back because he is still wearing his wet clothes, so you have to complete the level quickly after or else Mario dies of the rash’s severity.

According to conservation of momentum, kicking a shell at a pipe should cause the pipe to bend slightly and the shell to return slower. Also, shells should be subject to friction as they slide along the ground.
Now when you want to play a level, you must move Mario the entire way between the level you last played and the entrance to the new level. Also applies to Course Worlds, since going between long levels in just a second isn’t realistic
If a level ends with no obvious “exit” to go anywhere or in conditions that should kill the player, you have to either trek through the level backwards to an exit or switch characters to go anywhere else
The desert theme is now realistically sand, so when mario ground pounds, jumps and lands, or spins, he get sand in his eyes which covers your screen with it.

Some of my ideas: 1. You used to stomp on a koopa to remove it's shell in some game styles, but turtle's shells are part of their body. Now, if you try to remove it's shell, it'll either back into it's shell just like regular, or it will just die. And naturally spawned shell-less koopas now have gore coming out them, and they will die from blood loss and broken organs after 15 seconds. 2. Stomping on a Koopa Paratroopa (in super mario bros and super mario bros 3 style cuz they look like turtle-birds there) will make it get angry, angrily chirp at you, and crap all over you. This will stop after 20 seconds. 3. Every type of Koopa is a turtle! So now, the Bowser and Koopaling bossfights have the bosses do nothing other than walk around on 4 legs. 4. Any sort of winged thing will eventually run out of stamina, fly the the nearest surface, and recover their stamina before going back to what they're doing. 5. you now have to clean your Koopa car. Winged things poops on it like a bird? Thwomp gets dust on it? You run over an Ant trooper and now there's insect blood on your car? Well, too bad, gotta clean it!
We’ve noticed that jumping on lots of enemies give you lives so we’ve made it so instead jumping on enemies just makes Mario tired. Also 1up mushrooms don’t really do anything because we all know you can’t have more than one life in real life.

Boos and Peepas are ghosts, and in real life, ghosts cannot interreact with living beings. So, they should attack you at all, and it's okay to touch them, and you cannot kill them, and they can't kill you. Dry bones are Skeletons, and skeletons moving isn't realistic. So, They should just lie around dead. Fire bars and those fire engines produce fire underwater, and fire enemies like bowser breath fire underwater, and lava bubbles are okay underwater, which is very unrealistic. So, they should be just blocks when underwater, enemies like bowser cannot breathe fire underwater, and lava bubbles become stone or obsidian. Koopas are turtles, and turtles cannot have wings. So, Koopa's speed should be reduced, and restricted from wings.
Suggestion: you instantly die in lava levels due to the heat. And the higher up Mario is, the more frequently he has to stop to catch his breath, due to the thinner air.
I got an idea for the next update. When you first boot up Mario Maker 2, Mario will start pretty young. But every year, he gets older and older like on real life until the near future when Mario becomes so old that he'll pass away. Mario will act diffrently depending on which age Mario is.(e.g As a baby, Mario can only crawl)

At first the bro's movement is really slow, but with each throw it gets faster. After about 10 throws it can't throw any more hammers but it's faster.

About the Lakitu one : spinies' shells look hard, so if there would be any injuries, the worst one would've been a spike fall off.
It would probably depend on how well it can soak up the impact. If the soft body inside takes too much, that hard shell isn't doing jack.
