A mere thought from someone who has just finished SDRA2 recently. I reckon that chapter 3 title, "The girl who looked at the devil inside the abyss" directly refers to Otonokouji twin 's execution with the girl-Hibiki had her head decapitated and the devil-Kanade when the ground fell off respectively. Put it together, it informs us that Hibiki was actually alive a little bit after decapitated, witnessed her remaining body on the ground and Kanade's madness. A gruesome end for an innocent live like Hibiki.
Shinji died a hero, so there's that. But yeah, they all deserved better. My one consolation with Setsuka is that she likely never felt herself being stabbed, so she didn't suffer. She WAS stunned with a tazer after all. RIP Best Girl (and my personal favorite character in any media) Setsuka. You are missed dearly.

Not really, if you wanna know (Spoilers) In chapter 5, the victim was Teruya, the guy with the green hear and long scarf, and the killer was Mikado, the tophad guy with a half mask. Anyways, the culprit managed to frame the artist girl, and win the trail. Everyone alongside got excuted in the process, but they were saved

It was a simulation and in the real world every character is Alive excepted Mikado who was killed by Hashimoto (who logged out of the killing game stopping the 4 from getting executed). Iroha,Sora,Yuki and Yoruko survived the game but Sora stayed in the simulation after since she was like a remnant of Akane Taira. Mikado AI was shot by Tsurgui Kinjo. So Iroha,Yuki,Sora and Yoruko in the simulation but then every character besides Sora and Mikado in the real world.
