All majors ports and airports around the world are monitored for radio-active emission and there's even satelites looking for trace of radio-active isotopes, all of this to prevent this sneaking civilian-vector attack. However, a bomb tightly enclosed in a lead coffin could theorically still go unnoticed for a while, maybe until it's too late. And it still perfectly possible to unload the bomb from a submarine or a tanker at sea and sneak it from a private dock with the right equipment (nuclear weapons are heavy!).
It'd be hard to smuggle in parts but that's really the only way if it wasn't a country using a ship. But you can smuggle in small amounts of material and build the thing at home. it was proven possible

We have used depleted uranium shells in Iraq Afganstan and Yemen. The birth defect in hard hit areas of Iraq is more than 10,000 times the normal rate. Couldn't these acts of terror by the US lead to retribution perhaps the use of primitive dirt bombs in a manner similar to that proposed by the Secretary?

Japan pre-emptively attacked the US when the US wasn't even involved in the war. The plan to invade Japan also anticipated literally millions of casualties for both Japan and the Allies. I suspect you know nothing about history and are speaking from emotional motivations that are easy to destroy in reasoned debate.
