- Bimaxillary Protrusion Treated with Insignia® System Customized Brackets and Archwires
Abstract
Correction of moderate-to-severe bimaxillary protrusion is challenging, particularly without orthognathic surgery and/or temporary anchorage devices(TAD). This case presents a 4-first-premolar extraction treatment of a 12-year-old boy with a chief complaint of lip protrusion. The diagnostic evaluation revealed a convex profile, steep mandibular plane angle, bimaxillary flared incisors and protrusive lips, deep bite, and deep curve of Spee.
The orthodontic treatment was accomplished with a customized appliance (Insignia® system), including the self-ligating brackets and progressive archwires, constructed by reverse engineering from a 3D digital set-up. Extraction spaces were successfully closed in all four quadrants in 13 appointments over 19 months without using TAD supplemented anchorage. The facial profile and occlusion have been significantly improved.
The 3 keys to success of bimaxillary protrusion treatment with digitally engineered mechanics are precise estimation of anterior torque compensations, appropriate use of auxiliaries, and careful adherence to the recommended archwire sequence. With correct treatment planning, the outcomes are enhanced by minimal treatment adjustments, and decreased treatment time.

使用數位化矯正裝置成功治療雙顎前突有三個關鍵：1. 精準預測前牙矯正所需的torque補償，2. 在適當時機使用輔助裝置，3. 依循系統建議的矯正換線順序。在正確的治療計劃設定下，使用數位化矯正裝置能減少矯正過程中需要調整的次數，縮短整體治療時間，更有效率的達到理想治療效果。

i was little skeptical when i was told my 4 premolar teeth were to be extracted. this video helped me understand the why i need to undergo that procedure. its been a week since i had my teeth extracted. hoping for the best results.

Really hoping to get this done someday, most of my facial insecurities deal with the lower third of my face. The profile looks horrible and my philtrum is already longer than it needs to be with my small lips. I feel like this would be beyond helpful to my self-esteem

Hi everyone! My daughter's teeth moved into place so quickly after extractions. And she will take off her braces just before Christmas 🎄. I can't believe how quickly time passed by. I’ve had my braces for 6 months now and the gaps in my lower teeth are semi closed but the upper ones are still there but on their way to close too! The gaps don’t bother me tbh and they’re not noticeable unless you open your mouth wide open.

@@ashutoshsegekar8144 You can see at 8:50 his protrusion is fixed but at the same time his chin (and his facial profile in general) has shifted down a little bit. You are not supposed to have a long face while you are still young.
Unlike orthodontics, orthotropics is all about forward and upward growth, pushing your upper and lower jaws forward and upward.
Ailar Kma imagine the exhaustion in our jaw and chin when we can’t properly close our mouth. I think your face slimming down isn’t a bad thing.
I used to have an open bite (5mm) and most orthodontists had suggested extractions since I couldn't afford surgery. So I opted for a camouflage technique instead, osseous genioplasty to fix my protruding chin, 4 premolar extractions (wisdom teeth had already been extracted), lingual braces with tads for 2 years. I now have wire attachments behind my front teeth and wear a retainer for 12 hours a day and a tongue crib to better help me position my tongue. It has been worth it.
@@jazmin2111 it's a bit difficult to say! It has become a bit slimmer but that was from the genioplasty and masseter Botox injections but im not sure about the premolars themsleves. it loiks better now tho!

My smile is already fine but i am very insecure about my side profile and my overbite so i was planning to get braces but my new dentist (since the last dentist just scammed me) informed me that I'd need to sacrifice my perfectly fine pre-molars first, i was very hesitant at first but thanks to this video and bunch of research I am now able to trust my new dentist, thanks!!

For me, I'm actually happy with my smile, and my teeth alignment are considered fine, it's just the protrusion really bothered me and the fact that I can't close my lips properly. It's rather scary having to go extract healthy teeth and having the empty spaces while the treatment is going. Do the empty spaces bother you much? How long till they close?

Yes, I have thick pouty lips before extraction and braces, after like a year I noticed that my face became wider and my lips are like moved backward and smaller, my chin is more forwarded now and im happy with that, but i wish my lips remained the same :(

I had the same teeth extracted but only on my right side. Now I have braces, my treatment time is 24 months, but I'm already on my 3rd month!! I have some power chains on my canines and a coil spring on my left side (which hurts a lot).

@@yomomgay7170 Yes you can get braces after having the tooth extracted. I had to get 2 of my canine teeth taken out on my right side, top and bottom teeth, before I got my braces. Hopefully that helps you

My ortho waited a whole year and 2 months to have the extractions done. And even after 8 months now (I got the extractions back in feb 22) the gaps are still there. Like you could literally still fit an entire tooth in each of the gaps. And I’m so frustrated because I was supposed to get my braces off this October, but it looks likes it going to be a bit longer. Probably much longer at this rate… 🙃🙃🙃

Why did your Ortho wait so long? Were your teeth really crooked? To my knowledge, yes, you need to align and level your teeth first before you can close any gaps, but still that's pretty long...
Did they thought it could be fixed without extraction, then changed their mind after a year or was extraction always in the plan?
For me, I have bimaxillary protrusion, but my teeth are not crooked, only slight overcrowding in my lower incisors. My ortho extracted my top premolars within the first week and installed the top brace few days after that. Two months later the lower teeth and jaw followed. She needed the space for the teeth to wiggle around and aligned themselves, before finally closing the gap. The levelling and aligning she predicted 6-8 months.

I just got my 4 premolars removed. And the dentist said that Change in body is a very slow process and bone resorption and deposition rate varies from person to person. It may take 3-6 months for the gap closure but depends on the position of the jaw,bone,teeth, and your regular follow ups

May I know why the upper canine teeth were made curved? Most people have a pointy canine tooth but he has a flat bottom naturally why dentist still insist to move his canine from flat to pointy? Wouldnt the root made curved as a result?
I don't see any problem in protrusive teeth. It is at least better than having severely crooked/crowded teeth
the lips and muscle can't relax to close, it is also aesthetic

The condition of course. Unfavorable as in western media used to more commonly make fun of Asians by stereotyping this kind of teeth.

I think you can always reduce size but it’s harder to make it bigger as you age. So yes but the retraction May be less drastic since your bones are fused. My sister did it at that age and it worked

Sometimes the only option is extractions. I got four teeth pulled because there simply wasnt enough space for my teeth to move, and my upper teeth were still so protruded :/
I was given the choice to just have my teeth aligned, but the end result would be more protrusion, which ofc I didn't want

Braces causes sleep apnea because of the pulling of the teeth backwards you won't have space for resting the tongue this will cause you breathing problems
