I remember feeling a little upset when after dying to Cerberus three times the game was like "Easy difficulty unlocked" when i didn´t even got to his last phase to know all of his attacks which even changed according to which head of the Cerberus boss you destroyed first.
Most of these are based on scripted events such as choosing a lower difficulty. I like when games had a preset list of quotes when you die like Risk of Rain or Minecraft. Things like "player was *eviscerated* by enemy" or "player should have played on lower difficulty". Though to be honest, dying in Gmod by Gravity or Physics Prop still hits pretty hard.
I actually have a burning hatred for devs who intentionally make the game impossible to complete on easy mode. That’s not what easy mode is for. It’s a gateway for new players to try out the game without being intimidated or feeling incompetent. I think some of these are fine. They are the ones that don't really affect gameplay and are there for a joke, the best example being with Styx (I think that was his name? The orc guy I mean). The ones that give you a worse ending are the ones that are scummy. The consumer paid for the game, played it through entirely, and they are punished for it. That is not cool.

To me games should give players options whether they are playing casually or hardcore. Give rewards and extra things to people playing on the harder difficulties but to mock someone for choosing an option YOU GIVE THEM is ridiculous.
