  • According to Prometric guidelines for the nurse aide state exam, this is a step-by-step demonstration video for providing a modified bed bath to one arm, hand, underarm, neck, chest, abdomen, and back. This skill is one of the 21 testable skills on the Texas State CNA Exam. Remember to include indirect care with this skill.

    Thank you passed my exam! I had to modify bath, range of motion from elbow to wrist left side. And I had to take a pulse. My nurse examiner was very nice. Smiled and came back and gave me congratulations. It did take a few hours from start to go but, all went well

      @gladynina please did you wash only face, one arm, hand and underarm or you washed both neck and shoulder the way Dr Lee demonstrated?

      I did it like in the video. Both sides of the body and face. The tester will give you specific instructions if they want it done differently. Take your time and pause if you need to remember something, I was able to correct myself a couple times and she liked that I caught myself.

    Firstly thank you your videos are extremely helpful!
    Secondly, when you are washing behind one ear and to the other ear, should we change cloth/ towels or fold the cloth/towel using a clean side?
    Or does Prometrics consider it acceptable to just use one towel?

      Hello Lisa,
      Prometric does not require the candidate to change the cloth/towel when washing the body. Now, if the towel comes off the body you MUST change the position of the cloth/towel.

Doctor, what is the difference between prometric and credentia skills? I'm very much curious to know because of the proven differences in partial bed bath. I'm testing this weekend and I'm confused.       Hello, Each state can choose which agency to use as a tester for their citizens. Think of it as a credit card. Each store can decide to take Mastercard, Visa, or American Express.
      Texas in 2021 decided to go with Prometric as a testing agency for the nurse aides. Therefore, students must follow Prometric guidelines.
      Some states test under Credentia or Headmaster. This is why I stress to students to KNOW which company their state uses as a testing agency. The skills are different.

    Hi Dr.Lee, aren’t there supposed to be 4 big towels? A barrier towel for the table, one behind the patient, one to dry the front of the body and another to dry the back + pat access lotion?

      Hello Shea, Prometric does not grade us on how many towels we bring to the bedside. For this skill I will use 3 big towels. One for the table, one is placed under the patient, and one to dry the patient's body. If you use 4, that is Okay also.

    Hello, I was wondering does the order of washing as far as arm then neck matter?

    2. Neck

      Hello Ashlye, I like the unique way your name is spelled.
      After the eyes and face is washed, Prometric does not care about the order for the rest of the body.
      Prometric states you must start with the eyes first

      @@txcnaskills2599 Prometric clinical shills checklist also stated after face wash and rinse resident’s neck , hand, arm, chest, and abdomen. Wash from cleanest part of the body to dirtiest part.

    Two questions.
    1. My class was taught to change gloves upon removing the gown and after finishing the bath prior to removing the privacy blanket. Does Prometric require glove changes?
    2. We were also instructed that we must lift the breast using the back of our opposite hand to wash under it. Is that not correct?

      Hello Amie,
      Prometric does not require gloves to be worn during the modified bath, therefore you are not required to change gloves. Please see the indirect care instructions for the modified bath. (Prometric states, IF WORN).
      Prometric does not require the student to left the breast using the back of the opposite hand. You will be performing this skill on a manikin. The breast will not move. (LOL)

      Hello Austin, For the Prometric state exam the peri area cleaning is a separate skill. The state tells the client what part of the body to clean for this skill.

    So that towel that you used as a barrier for washing the arm does it not have to be moved when you wash the abdomen area? Also when doing a modified bed bath in reality you’d do both arms correct?

      Hello Destiny, I hope I am understanding your question correctly. The bath towel used for the neck, chest, and abdomen does not need to be changed. Think about when you bathe at home. Most people use one washcloth to bathe the entire body.
      For the second part of your question, yes the candidate is tested on completing a partial bed bath including ONE arm. In a real setting, I would wash the entire body as needed.

      Hello Lucy, Checking a client/patient's wristband for their name is NOT a requirement for the state exam. The student should knock on the door, announce their presence, call the patient by their name, and tell the patient who they are.
      I instruct my students to ask their partner what their name is before starting the exam. The evaluator will pair you up with someone before the exam starts. So, ask your partner what name should I call you during the exam.

      Hello Cassi,
      The candidate does not have to change their gloves during the bath or when putting up the dirty towels/emptying the dirty water. The candidate does not have to change the gloves when giving a back rub.
      The candidate will clean one upper part of the body as stated in the Prometric Skills Checklist.
      The candidate is asked to bathe a resident who is in bed and who is not able to help with his/her bathing. For testing purposes only, the candidate is asked to wash just part of the body so that technique can be evaluated. Instructions ask the candidate to wash the resident’s
      face, neck, back, one hand and arm, and the chest and abdomen. Soap and water should be used for the bathing. The resident also needs a back rub. When completing the skill, the mannequin is dressed in a clean hospital-style gown. A mannequin is used for the role of the resident.

    Hi Doctor Lee , my teacher said that we should change water on every wash , this includes change water after washing the face gloves including, change water after washing the hand and change water after washing the abdomen? How Truee is this ?

      Hello Julia, You must remember that the candidate will be timed approximately 30-40 minutes to complete the skills portion. Prometric DOES NOT require the changing of the bath water unless it is soapy or cold. Therefore, changing the water after cleaning each body part is not necessary.
      Think about when bathing at home. Do we change the water after washing different body parts?

      Hello Elizabeth, They are the same skill. Prometric calls the skill "Provide resident a partial bed bath and back rub".
      If you have ever studied for a Pearson Vue exam the skill is called "Modified Bath"

      Hello, Prometric REQUIRES the student to wash the chest and abdomen ALSO.
      It sounds like your teacher is reviewing a Credentia guideline. So, please find out which testing provider is used by your state.

      Hello, The evaluator does not care how we test the water. The important step is that the student tests the water BEFORE using the water on the patient.

      Hello KK, I can put the rinse towels back into the basin because they don't have soap on them. The towels are not making the basin water contaminated. Remember, most patients take a bath at home using one towel to wash and rinse with. Good questions.

      It is not required for this skill. When nurses are doing private duty in a person's home the nurse may not have s bed pad for use

      Yes you can. Remember when most bathe in their private homes, the person use the same dry towel over the body.

      Hello Joseph, Anytime a student applies extra lotion to a person, the candidate must pat dry it off. A student is allowed to self correct to gather supplies that was forgotten in the beginning of the skill.

    Hi I have a question! if we are raising and lowering the bed are we allowed to touch the bed controls with gloves on? For example, at the end of the skill when I go to lower the bed should I remove my gloves to lower it? It seems like the most sanitary option but also doesn't seem right because I have to wash my hands when I remove my gloves and I cannot leave the bed raised if I leave to go wash my hands first. Please let me know!

      Hello, The Prometric evaluator will NOT penalize you for raising or lowering the entire bed with your gloves on BECAUSE this is not a step that is being graded for the skill.

      Hello Danielle, Prometric requires the eye and then the face to be cleaned first. After the face is clean, it does not matter which part of the body you do next.

    Hi Dr.Lee, my students who have been selected for this particular skill at the prometric state exam are telling me that their skills card states that the patient cannot sit up and is too weak to help. They said they are having to roll the resident in order to put the towel under them. Just wanted to see if you were aware of that?

      Hello Cassi, The state guidelines in the Skills Checklist does not state the patient CANNOT sit up or is too weak to help. I am not aware of this requirement especially since this skill is done on a Mannequin and NOT a resident.
      The candidate is asked to bathe a resident who is in bed and who is not able to help with his/her bathing. For testing purposes only, the candidate is asked to wash just part of the body so that technique can be evaluated. Instructions ask the candidate to wash the resident’s face, neck, back, one hand and arm, and the chest and abdomen. Soap and water should be used for the bathing. The resident also needs a back rub. When completing the skill, the mannequin is dressed in a clean hospital-style gown. A mannequin is used for the role of the resident.

    Hi Dr Lee I have a question! on the skill mortified bed bath can I put the towel you dry off with on the bed as long as I have the bathe blanket on them that wouldn't be containmenting the sheet would it and I'm testing with prometrics as well and also when putting up the basin I can put the basin up with gloves on as long I have a clean paper towel to hold the basin as I put it back up

      I hope I understand your questions.
      1. Yes, you can place the towel you dry the client off with on the bed. The important thing with Prometric is that the bed linen stays dry and the top sheet stays dry.
      2. When you empty and rinse the basin, it becomes clean. Since we still have on the dirty gloves, we use a paper towel to pick up the basin and store it.
      I hope this makes sense.

If you review step 16 from Prometric for this skill (16 Replace basin of water during task if it becomes cold or soapy) that would infer not to place the soapy washcloth in the basin of clear water unless you want to keep changing the water in the basin. I hope this is clear.

      Hello Susan, Prometric does not count the number of washcloths the student use. They are concerned with the technique.

    Does it matter what order each body part is cleaned for example starting with the face to then moving on to the neck instead of the arm? Will they deduct points if that order is done?

      Hello Marilyn, Yes the order for doing the bath does matter.
      1. You must start with the eyes and then clean the rest of the face.
      2 After you have rinsed and dried the face you may do the rest of the body parts in any order.
      3. You will fail the exam if you don't start with the eyes. Remember no soap on the face.
