      You couldn't have said it any better lol!! This man is completely mind blowing in his knowledge of addiction and the proper approach to treatment and even his vast knowledge of almost every single drug out there. I could listen to this man speaking about addiction and the proper ways of treating each addict. We're all different so there is no one way approach to treatment. That's why I can't stand rehab because they are a one size fits all types of treating addicts is completely bullshit!! Of course Dr B is located in California and I'm in Philly because I'd do anything to be able to have him be my doctor in this 25 year battle with addiction that I'm suffering through every day! If anyone who reads this knows of any Philadelphia area based doctor who follows the same tactics of treatment please reach out to me 🙏 I'd owe you m life. Thanks for your post. It's exactly what I think every time I watch his videos

    In my over 25 year active addiction, I never just abused 1 substance at a time. In fact, I abused Alcohol, Opiates, Benzos, And Stimulates.

    I wish I was able to work with much longer. We Really need more Doctors like you.
    I would love to see you doing Seminars for Other Doctors!
    Your understanding and approach to Substance Abuse and Tapering is exactly what we need more of in this Country.

      There is a thin line between use & abuse though. I used to drink 1 monster energy drink a day, so nothing extreme, with maybe 1 or 2 pints in the evening, but at some point my nervous system got overloaded & i quit. Later on I almost completely stopped alcohol too when I used kratom daily which I still do & manage for pain. So ye like I said there's a thin line, alos between recreational & medicinal use as I've expierenced it.

      I drank alcohol on benzos. I was also lifting weights, and beat the crap out of my body. I was around weed, and Hass. I don't remember if I smoked some. It started with benzos. I'm iffy on psychiatry medication these days.

      @@ThatGirlMintz yes BUT the safest way to detox from benzodiazepines is to taper down. In patient treatment facilities yank you off of them completely and that’s can be fatal.

      Doctors have always told me that benzos are addictive. I say, I’m sure I am addicted. Then, I get the script. No doctor has ever told me what withdrawal is like…

    They are cracking down here on benzo scripts. I don't know whether it's state or federal, but my Dr has let me know she will soon just stop prescribing mine due to this crackdown. I've been on xanax for 17 years! She gave me no options on how to do it, & knows one of my great fears is withdrawal. I'm currently trying water titration. I understand not prescribing to new patients, but help the ones you walked into addiction!

    I really don't like people putting a dependence and then saying it's an addiction. I HATE Xanax, but If I don't take it, I can't function at all. There is an addiction but there is also dependence. America needs facilities strictly for Benzodiazepines and that would save way way more lives. It's just the stigma that people say addiction vs. dependence. We need facilities just for Benzodiazepines.

    I am so glad you talked about just being so open , when you go into treatment ,with all the drugs you are taking, abusing and or dependant on. I started my recovery 29 months ago . I wanted help for many years during my 20 years of active addiction but was afraid to admit to all the substances I was taking . Like a previous comment said I used many substances together . I do agree ( in my Case) the opiate addiction being addressed first was crucial . I began methadone treatment along with my group and individual session. And my anxiety levels dropped so much that I really had no issues with benzo and Crack/ cocaine withdrawal. Everyone is different but this video should a guideline for treatment of patients everywhere!!! I am always so amazed by your understanding of this disease Dr.

    Excellent video. Never have I ever heard of any doctor touching in this trifecta.
    But i personally found myself in this exact position. My meds were always prescribed, opioids 17 yrs, benzos, and adderall (stimulants) 15 yrs.
    Then I was forced to get off the benzos (they can’t prescribe if on opioids). Gave me 2 months worth of scripts and was told taper myself off. I did it, it was pure hell, but I survived it.
    After that I realized the adderall was only increasing my anxiety, and insomnia was going to be the death of me. So I tapered myself off within a few months.
    My Last battle was getting off the opioids, which I felt was the most painstaking of them all.
    I Went to detox for 7 days. After that, I was on my own. Without MAT. But,
    I’m still here. It been 5 months now. I’m still suffering mentally & physically.
    Looking back I’ve been going through some type of withdrawal for about 1&1/2 years. And it’s been the absolute worst year and a half of my life. My whole world was put on hold just trying to get through every day without wanting to take my life. So much precious time lost laying in bed, barely able to care for myself.
    If only I knew about you, how there is a doctor out the who is way ahead of the times, really understands that there are complex cases. Taking ALL things into consideration.
    You can provide life changing treatments that most doctors do not understand or don’t not dare to try to treat.
    I commend you on all your work, care, compassion and dedication!

    As a pharmacist with an interest in substance use disorders treatment, I 100% support your vision of separate addiction tx clinics. I have the same vision, because involvement of physicians and other allied and mid level health professionals, may be beneficial in management of this illness.

    It took me 7 years to get my clonazepam back and had multiple break downs going untreated just because i am prescribed suboxone. Almost every doc i saw dismissed me as a pill seaker and openly treated me like shit. My life is a lot more stable now

      I know exactly what you went through because I had to go through the same thing. I can get Subutex and Adderall prescribed to me with no issue however no benzos. Many different doctors just don't want the responsibility honestly.

      I was so lucky to find a dr who gave me my klonopin and vyvanse together ..I’m also on MMT. But he said His brother died of a heroin OD in the 90s so he didn’t treat me like just another “addict”. Sadly he just retired I was scared that the NP that I had to see was gonna cut me off all my medications but she didn’t.

      That’s crazy… my new doctor asked if I wanted some when she first prescribed my suboxone. This was online without being seen as I switched insurance and health care providers so I was going through withdrawal. I had to take a urinalysis when I picked up my meds but she also gave me promethazine loperamide and muscle relaxers also.

    Dr. B, We need to Clone You !! I wish that you were able to do Seminars with other Providers to understand what really needs to be done to help Society and all the addiction issues. You are so Spot On in your knowledge about all the drugs out there. You are so through in your assessment of patients. I am lucky enough to know you and have been cared by you. I'm not an addict at all. I came to you due to being cut off Cold Turkey My Benzo and I was suicidal. You were the only Dr B to understand what I was going through and you saved My life! I would love to see you be able to expand your Clinics and help more people who can't get the proper treatment elsewhere. Ins Co are the big problem. Thank you for sharing your knowledge. You Rock Dr. B !! Thank you for everything you and your Staff do for those who need help.

    I feel so lucky that my therapist is operating under the same guidelines described,
    in France Benzo and Opioids addictions imo is not that commun and she's been treating me really good with the switch from Oxy's to Bupre, stabilisation and tapering down of Benzo.
    Also feel lucky and blessed to be alive since I was taking horrible amounts of both

    Hi Dr. B. Wonderful advice for both doctors & patients. My friend is on suboxone and has to get it illicitly because he has a nasty benzo habit (8 Xanax a day at most 10 one mg). His Addictionologist will not give him Suboxone because when the doctor switched him to Klonopin he went into terrible withdrawal and went back on Xanax. I’m happy to see that you are open minded enough to really work with a person on benzos. I plan on sending him to your office. Keep up the great work!

      It's because benzo lowers respatory and add subs on top of that no doctor would prescribe those 2 together.

      @@daniburke9452 Not true. Some do. The chances of respiratory depression with a benzo added to a partial agonist opiate is slim to none. While the combo won’t shut down the brain telling the body to breath there’s a higher chance of benzo overdose. The person would have to essentially overdose on benzos. At that point the suboxones presence wouldn’t matter much anyway.

      @@daniburke9452 Many people have norco or oxycodone prescribed with Xanax. Norco & oxycodone are full agonist opiates. Suboxone is partial and does not shut down the brain telling the body to breathe.

      If I would've finished detoxing off Fentanyl in 2015, and I was at Day 11 cold turkey and 7-10 more days i would've beat it and u was on 20-30mg daily nasally/inhale, but I relapsed and went to a Methadone Clinic and got on Methadone and suprisingly the doctor basically PRECRIBED ME THE TRIFECTA... Im Canadian, and NO Healthcare Is not Free Here Universally, might be a lil cheaper for Upper Middle Class-Upper Class or Rich Folk, but not if your Low Income. If you got a Good LongTime Oilfield Job and worked your way up you might get 100% Blue-Cross Family Benefits, but you gotta be in a Senior Position or a Secretary. If your work is in Politics, Health Care Services, Local/Federal Law Enforcement, or like City/County/ Government work you can get Universal HC that pays between 50% to 85%, and 85%-100% if your a Mayor, higher up Cop, Prosecutor, Judge, Surgeon, or like a FireHouse Chief. So I dunno why other Countries think that. Canada isn't Denmark or Finland lol its a less populated version of the USA, and were just as rough and crime ridden In most areas areas, some more then others. It costs over 450$/month just for my "Generic" Prescriptions, anyways My Methadone Doctor and Addiction Physician didnt care at all, in Fact I was Prescribed 40mg Methadone and 3 other drugs I previously had not done, on purpose anyways, I hadn't tried Meth yet although it could of been and probly was in some of the Coke that I used 1-4 times a week from age 15-28, and old School Good pressed E or str8 MDMA is in the same family as well. But I started on the 40mgs of Methadone at first and they also added 20mg Dextro-Amphetamine 3mg Clonazepam, 5mg Clonidine, and 350mg Generic Seroquel daily, obvs my juice went up, all the way to 280mgs, and in the last 11 months of Weaning I'm finally down to 200mgs Methadone, 2-2.5mg Clonazepam, 4mgs Clonidine, 10mg Dexedrine, and 300mg Quetiapine... coming off all those at once, even if it's just a 1/5th reduction of my entire script, was hard as hell on the body and brain, so I actually paused my Reduction a 3 weeks ago and I feel great again, but I'm gunna wait til new years to start dropping again since that was my Resolution last New Years and I stuck to it so hopefully it works out, also got down to only smoke a cpl cigarettes a day, but I'm still an Indica or Hybrid only Marijuana user 1-3×day, and dropped the whiskey a month before Methadone. Crazy stuff! I'm praying yall make it or at least just stay alive please, I've lost too many friends including my Best friend of 20 years from age 9-29.

    How about subutex (1/2 of an 8mg tablet, intranasaly) adderall (7.5mg mixed and taken intranasally with suboxone), xanax and klonopin (3mg throughout the day) and between 2 and X bowls and/or hotrails of this sketchy p2p methamphetamine. I have over 12 years off of heroin, and had about 9 years of only suboxone and cannabis, but I obviously went off the rails. Or back on them. I have no idea what I'm in for after at least a year of all of those combined.

    That has to be tough on the adrenal system...fight or flight with the stimulants, respiratory depression with the opiate abd sedation with the benzo....
    The body must go haywire

    This is exactly the approach I took. I didn't need a doctor to tell me, get the fentanyl out of the picture. Then I dropped my Clonazepam to
    .5 most people say that's a high dose still however I've always cold turkey from a dose of 4-6 mg's. Plus all kinds of other crap. Today I dropped the benzo out, past the 24 hour mark. I am still on 4 mg's of Suboxone. I'll cut out a milligram a week. Bit fast but it's something I feel I can tolerate. I've cold turkey'd too many times now.

    Benzodiazepines doesn't do much for me, I mean the opiate effect is so much better (and so more addictive). Tho I still take diazepam, mostly for muscle relaxing but I often think it's more the placebo effect that does it for me lol.

    Do you think it's ever safe to treat chronic pain with opioids (like 10 to 20 morphine equivalent per day) if the patient has never shown signs of addiction, but is certainly dependent after 15 years of use? Patient is also being treated with Vyvanse 70mg for binge eating disorder and ADHD, Ambien for sleep, and occasional, break glass in case of emergency, .5mg of clonazepam for panic attacks (all of which have been prescribed for years with no issues.) I know on paper this looks like a bunch of red flags, but the patient has never shown drug seeking behavior, never broken pain contract, never run out of medication too soon, has been treated with years of physical therapy and multiple spinal surgeries, and due to GI bleed cannot take NSAIDS. Patient is successful professional with healthy relationships, active social life, and high level of community involvement. It's me, I'm the patient. 😬

    Dr. B - what if after you get them stabilized, you find out, they have a pain problem from physical issues and other anxiety problem. Do you leave them on low doses of Suboxone and Klonopin, both for life?

    My son started his addiction after getting lyme disease. All the doctors he has seen do not believe his addiction is related. I truly believe all his issues are a result from this. He gets extreme anxiety, insomnia and panic attacks after he was infected by a tick. They prescribed him Xanax at age 17 and it helped at first but then cut him off. From there his addiction started with opioids.

      I am also a Lyme cfs patient.
      My suggestion is he try to get into a pain management clinic that understand lyme CFS
      A lot of those Lyme doctors were complete idiots when prescribing.
      It’s really hard. The Lyme probably did mess up his neurological issues.

    Are you saying that the prescribed medications for me which include Subutex, Adderall, and Klonopin are a combination I should not be on even though it completely allows me to live my life without seeking pain meds, being crippled by my crippling severe anxiety and no other non-benzo drug can give me a 1/10 of the relief of my Xanax. Lastly I shouldn't be prescribed a stimulant because they have a high tendency to get abused even though I don't abuse any of my meds for many years.

    Wish the government/big pharma would invest in Alcohol abuse and coming up with novel treatments to help sufferers. Watching a close friend struggle w poly substance misuse, it seems he has the most trouble kicking Alcohol as there are replacement therapies for substances mentioned in this video.

    I'm not advising this but have you seen the research in Europe when they give them diamorphine prescriptions and they seem to get clean off of everything else get a job get their life together and get off of that even

    I take a huge amount of Valium a day. 4 10mg a day , prozac, lithium and Adderall. From time to time I get percs which is my guilty pleasure.

    I think benzos should be banned, or more regulated. I'm an addict, and it almost killed me. The fact that it's given out like candy is stupid to me. The mental health care system today just creates addicts.
