    He should have been using utility gloves when handling the instruments so not to poke himself. Also he never changes his gloves so he was going from dirty to clean with the same gloves that's cross contamination

      You are right I will give you that but don't lie you know you processed hella instruments without utility gloves and the way he's doing it is exactly how we do it so come on now don't be so judgy lol.

    By handling the sterilized items with gloves used to operate the autoclave, haven't they been cross contaminated?
    When the pouches are handled by the clinician, aren't his gloves/instruments then also contaminated?

    Did you put wet products in the autoclave?? With dirty gloves? ahhhhh noooo!!!

    This video is an outrage and should not be on TH-cam. There are quite a few errors in this video that should be addressed.
    1. Where is his P.P.E - should use (for handling of contaminated sharps and instruments to avoid injury and when dealing with chemicals to protect hands from chemical burns), (to minimise contact with aerosols, blood/body fluids and any other potential contaminants from entering through the mouth and nose and also to prevent inhaling large amounts of chemicals), (to prevent contaminants getting into hair, (protect against contamination of work clothes) and
    2. Ultrasonic cleaning (mechanical cleaning) - I do not see him rinsing matter of instruments prior to submerging them into the ultrasonic cleaner. This is an important step as precleaning by rinsing ensures that any gross visible material is removed to allow for effective cleaning during the ultrasonic cleaning process. It also acts to ensure soils have not dried on the instrument surface, making it harder to dislodge particles in the cleaner. He should not be placing lensed items such as mouth mirrors into the ultrasonic bath as the powerful cavitation and implosion action caused by the cleaner can cause lensed items to be scratched and damaged if put through repeatedly. Therefore, makes it harder for the end user to see out of the mirror when in use. Burs and ultrasonic tips can be put into bur baskets and placed into the ultrasonic.
    3. He should be using a new pair of gloves to disinfect the instrument tray. This is done to minimise the risk of cross-contamination. He should also allow solution to act on the tray for a certain contact time as specified in the manufacturers instructions for use (IFU)
    4. Instrument brush is clearly worn down at the bristles, and should be replaced when this happens to allow for effective manual cleaning and to prevent damage to instrument.
    5. No directional workflow from dirty...to clean.. to sterile
    6. Placing rinsed items into pouches when wet and on a dirty bench compromises sterilty because he failed to adequately dry items with a lint-free cloth beforehand after getting items out of ultrasonic.
    7. Failed to write all relevant batch control tracking info onto pouches
    8. using same dirty gloves when placing items into the autoclave and while operating. Covering items in a cloth makes no sense?
    9. No clear change of gloves when getting items out of autoclave. He continues to then setup a new procedure tray on the same dirty bench used when receiving the contaminated items. This is a major no no.
    10. No release process followed. Must check critical parameters have been met by reading the gages/read out on screen and the printout the machine produces, checking the indicators have changed colour also.
