Everytime someone says Portnoy is overrated I come here for comfort and reaffirmation, especially at 04:04. I really don't see any other drummer of his age doing something like this.
Travis Barker does alot of these fills but outside of these two, I don't hear it much anywhere. Symphony X sort of approached this without going full in, but that was the late 90's..... :\

Hands down one of the most well written and technically proficient drum pieces. The pull it creates during that guitar solo is massive! I used to re-listen from the solo breakdown onwards on my green iPod nano for hours on end whilst teaching myself to play the drums.
The thing I like about Portnoy, other than his amazing writing and playing ability, is the fact that he uses ALL the pieces in his kit at some point. There aren't any extraneous things that only get used once or twice per set (with the exception of the gong used in Six Degrees). The most amazing drummer ever.

Practice is one factor. Plus when you become acustom to your instrument, especially for playing it for that many years you really don't have too much trouble coming up with or remembering things.
