    This has been THE channel I watched for before and after TKR in May 2020. Tony answered more of my questions post op than anyone else, including my surgeon and PT ( and they were great!). If you read the comments, you’ll realize all of us follow a very similar trajectory. It gets better. Do your PT. By Nov 2020, I was fully functional. Now, Jan 2021, I’m completely back in my life. There’s nothing I cannot do on this knee.

    I’m now 10 weeks and doing brilliantly. I want to share this with you, there have been twice when I had a bad knock back. The first time I limped to my doctor in panic thinking something had gone wrong and got an Xray but all looked fine and a few days later it was back to normal. A few weeks later same thing, a couple of days and it cleared up again. It seems this is normal so don’t panic. I’ve had great reassurance from this site thanks a lot

    I was severely bow legged and have been suffering with wicked pain on my inner shins right below the knee implant and this explains why.
    Keep it up man, all of us going through this recovery could do with watching all your Videos.

    Tony is easily the best at explaining what's going on in our knees/bodies after TKR. I'm currently 6 weeks PO and 2 days ago started with bone pain & I'm slightly bowed legged and here's another of his videos explaining why and making me feel confident I will get there and that what's happening is normal. Thanks Tony! (Sarah watching from UK) 😊

    What a gift you are to us! I just wanted to say thank you so very much - you are a big part of my successful recovery from TKR. I have another one coming up soon. In your experience, have you noticed a quicker recovery or less painful recovery in patients that have had a robotic TKR vs the traditional method?

      @totaltherapysolutions  3 ปีที่แล้ว +2

      Thank you so much. There are certainly benefits to the robotic TKR and another bonus is you have already had a knee replacement so you know what to expect.

    I'm on my second knee replacement; about three weeks postop currently I had bilateral varus deformities and while my surgeon told me my anterolateral knee pain was due to the new alignment of bones, soft tissues, etc., this really does explain it well. My firstreplacement does have lingering tightness over my IT band as it inserts into Gerde's tubercle, but I manage it with stretching and oddly enough, a patellar strap when hiking. It's not 100% but strong and sturdy. This time, the surgeon was very careful with retraction of the soft tissues and didn't use a tourniquet. I'm hoping this outcome will be even better. I do have the tightness and anterolateral soreness. Trying to be patient. :)

    I just want you to know that your channel has been such a blessing. You have helped me understand so much the healing process because I had those pains in my knee after surgery and I was thinking that the surgery didn't work. Now I'm 4 months after surgery and those pains are very rare so now I know surgery was successful and the pain will eventually go away altogether.
    I also want to know what I need to be doing so that I can go up and down stairs without holding on to the banister?
    Once again thank you, thank you for giving advice as a therapist that really helps 💕

    I love your videos. You've answered so many of my questions. I am bowlegged, have rheumatoid arthritis and was dx with osteoarthritis in both knees. My rheumatologist referred me to ortho when my knee pain really impacted my life. My ortho referred me for PT and I used an uploader brace til we decided to go forth w/ a left tkr. Had it done Dec. 4, as an outpatient. Due to covid I was unable to access in home health but started PT again within the first week post op. (My rt knee is really bad also and I was going to have it done in March or April). My surgeon said he "shaved my bone to correct my bowleg(?)" I really struggled with swelling and my range of motion didnt improve . I had a manipulation under anesthesia a week ago and was referred for lymphedema therapy this week. Im using CPM 4 hours or so a day and my range of motion with assitance is between 90-95. Will it improve? I have a lot of pain on the top of my knee and to the right of my scar. Am I at risk for a second round of scarring? Is my RA part of the problem? Are these things indicative of problems if i get my other knee done? Love your opinion. My surgeon has a great reputation but is very busy and I struggle to get my questions answered during visits.

    • @totaltherapysolutions
      @totaltherapysolutions  3 ปีที่แล้ว +1

      Lots of great info here. I would love to hear what your PT is telling you. Your PT has the most information as s/he is working with you regularly. I have seen patients with severe bowing do really well. I have seen patients with RA do really well. Of course there are always obstacles to overcome, but with the right mindset and patience you will get to your goals.
      Simply put, your body is always changing. Take advantage of the good days, and let yourself rest during the rough days. Find things that keep you motivated and excited. Don't hesitate to reward yourself even when you maybe don't think you deserve it. That is usually when we need it the most.

    Dear Tony, I'm saying thank you at 04.00 in the morning. I've had very little sleep for a couple of weeks (7.5 weeks PO) and no matter what i take these bones pains and general restlessness won't let me sleep. I'm so exhausted that i can't do my physio and, of course, lack of sleep is debilitating in many ways😢. I call the bone pain 'growing pains' because its exactly what i remember from childhood. I had a bilateral tkr and they found quite an abnormal femur positioning in both legs so that's been corrected. I feel a bit like the little mermaid who had all that pain when she got legs!

Everyday seems a little better, but the tightness and pain around the knee seems the same.

      My physio recommended "always start off with your good leg" and it works. I can go down stairs although people stand thinking I'm gonna fall and no problem going up steps to plane at the weekend. My good leg is opposite to my leg that was operated on.. Just to make u laugh. I used to sit on the low wall opposite my house and I forgot about my leg and this time couldn't get up as I get painful sciatica so shouted like hell at my neighbour and he heard me and lifted me off the step. Havent done it since.... Yet

    Hello Toni. 8-28-23 I had total right knee replacement. Walking same day. Then I formed scar tissue. Scheduled a MUA. Week before MUA-in ER . Blood clot was found. Put on blood thinners for the next 3 months. MUA was a success. But there’s constant pain, mainly in the middle of my knee. At times it’s the inside of the knee in lots of pain. MUA was a success. Surgeon took picture after MUA, my heel was touching mg butt. Therapy is helping to a degree but I cannot get pass 85 flexion. On the hamstring machine, my left has no issue but my right knee cannot go all the way and the therapist has to push the knee to get it to bend. It’s going onto 4 months of constant painful knee pain and swelling. I can walk around but I’m very limited. Painful. I need your help please. My surgeon constant words are: I don’t understand why you are in so much pain and swelling. We’re at a point where we’re just take his answer any more. He’s the surgeon. Find the solution. I had to go back to work or else I would loose my position. Now my leg and toes are swelling up. Help please

      Hi William, thanks for the comment.
      What is it specifically that limits your ability to bowl? Does it hurt from the first ball to the last? Sometimes it can be one of those things that the first couple hurt, you get warmed up and feel decent for the next few, then it starts hurting again as you fatigue.
      It would also be interesting to know if you tried modifying your technique. I am not a bowler, but I know patients that didn't have the range of motion to bowl the way the used to, so now they have modified their approach and release and are back to the league.
      In regards to strengthening, I agree, strength is important. You are a tall guy, the worst thing would be to fall and not be able to get back up. If you returned to physical therapy specifically to work on lower extremity strength and your ability to get up/down from the ground, you could also get that therapist to help with your return to bowling.

    Wow that makes sense because I feel like the lateral side prevents me from full extension but the medial side is where I had partial replacement and it feels fine. I have a question about lower back. After I started to walk my lower back above my buttocks gets extremely tired and burning when I walk and prevents me from walking and standing for more than 5 minutes. I start to hunch over. Any exercise for the lower back or hips?I feel this is preventing me from more activity.

      @@totaltherapysolutions Hi Tony yes I am and he says it because I don’t have full extension/need to work on hamstring stretches and another therapist said I have to work on my abs. Its not painful but its like a burning that happens when you for example light weight lifting and you do too many repetitions. I recover once I rest. I looked for info about heel lifts and I came across your video. I may try that because when I stand my heels don’t seem to be flat on ground its lifted a little. Thanks for your help!

      Update : I tried putting a shoe insert on non surgical knee to raise that leg and it feels like my lateral knee pain is less and thigh muscles seems to work more on surgical side. Not sure if its from insert. I will experiment more. Pain in lateral side of surgical knee is not as bad or maybe my its my imagination?

      90-degrees at 5 weeks honestly isn't bad. There are so many other factors to consider. Your body will usually do what is best for you despite what your brain may want. Slow and steady. You will get there.

    Hi Tony, I’ve had a lot of problems with this. One of the extension exercises I did was to lie on my stomach on the bed with my surgical leg extended and unsupported. I had strong bone pain just below the knee joint in that position. My OS thought that this might be from attachment of the hamstrings. What do you think?

    • @totaltherapysolutions

      Hamstring doesn't usually present as "bone pain." I use the exercise you described for some patients. Here is a short playlist with other positions th-cam.com/play/PLVcY1xxDTSfy3Vi5-wX4TQMGy_uazZwTe.html

    My surgeon recommended replacement of both knees at once because he felt my bow legedness would make recovery difficult with unbalanced knee shapes. Do you have information about how to deal with bilateral TKR? Thanks for your informative videos.

    • @totaltherapysolutions
      @totaltherapysolutions  2 ปีที่แล้ว

      Honestly, once you figure out what is causing the pain, the next step is usually trying different things to resolve it. Sometimes this is walking differently or wearing different shoes. Sometimes cupping or taping helps. Sometimes it just takes time for your body to get used to the new knee and alignment.

Also have this pain at times shooting down the front/side of the leg to the ankle on the surgical leg. Very frustrated! Doing therapy multiple times a day and still not where I want to be.

    Is there a possible difference in pain when the stem used is long rather than shorter? Had a break on outside of tibial plateau in November. Surgeon says his solution is to use an extra long stem and that will stabilize break. Original pain and arthritis is on inside. I’m 5’3” female 72 years. Trying to find out If longer stem makes a difference in after pain and recovery. Thanks

      Honestly, I don't know. My best guess would be the different lengths would place different stress on different parts of the bone.

      Complex Regional Pain Syndrome, formally known as Reflex Sympathetic Dystrophy is where the body still perceives pain after the source of pain has been removed. It is a rare unique response to injury. I’m fifteen months post Uni-Knee Replacement and have burning, tightness and stiffness in my knee, that I had treated with nerve ablation. I’m amazed they have made this diagnosis in ninety days. My knee felt great from three to five months, then it became stiffer and I developed pain. It’s nothing like my pre-op pain. It’s more of a fun-sucking pain, than a debilitating pain. One of my fears has always been of developing RSI because it’s so capricious. I think I just have a lot of scar tissue, my doctor hasn’t even considered the RSI diagnosis. Fifteen percent of all TKR surgeries even when done right are less than perfect. Good Luck.

    I am 12 wks post TKR surgery and have great flexion ROM but not extension. At 9 wks I started using a mechanical JAS splint 3 times a day for 30 min. each session. How long does it take to achieve full extension? My PT is working on the fascia and stretching the muscles but has told me that b/c I haven't had full extension for 10+ yrs that I might not be able to achieve that goal. We took a measurement before I started using the JAS splint--11 so we can objectively track my improvement. At 3 weeks we are going to take another measurement. What say you?

    • @totaltherapysolutions
      @totaltherapysolutions  3 ปีที่แล้ว

      New shoes after a knee replacement are important. Depending on why the knee is turned in it could be coming from the hip or the ankle. Once you or your therapist can figure out what is causing it you can work on those areas to correct it.
