I had a frozen knee after TKR. Still have it really. But I’m working thru it. I’m extending snd flexing my leg to the point of discomfort holding for 10 seconds and then increasing repeat for 30 minute sessions 4 times a day. I’ve moved from 73 degrees to 103 degrees in a couple of weeks starting 7 weeks after surgery. I use a long rope like heat mat that also vibrates. As I approach the next step I also do deep message in the quad. And finally I’ve learned to relax and that this works best if I’m very distracted. A phone call works wonders or a very engaging tv show. I hope to reach 120 this week. He is very right. I am my own worst enemy by pushing myself into too much pain and then I shut down and can’t get past it. For me more discomfort and no pain with a good distraction and I get more gain. 😊

Thank you Tony for all the great information. Four months post op left TKR, two months post MUA. I have a lot of tightness stiffness and swelling. . I go to physical therapy three times a week. I just saw a second opinion. OS his recommendation was to stop physical therapy. Limit all my exercises to just a couple extension and flexion., and ALEVE 200 mg in AM. n PM. He said my knee was very angry with me and needed to rest, I’m afraid of losing my ROM. Currently at physical therapy and I’m at 122 and -4 Thanks

I only started watching your channel last October, after my first knee replacement. I’m three days away from my second knee replacement. That’s two replacements in two months. Of all the channels on the subject, yours feels more personal. I had great success on my first TKR, and hope the second is at least as good. I’m a little worried that my first isn’t quite ready to carry the weight, but I want this behind me before spring.

I'm so frustrated that I am dealing with this exact thing. My doctor says "no pain, no gain" I had a mua 2 days ago and am in so much muscle guarding pain. He wants me to do 4 hours of the pt exercises every single day for the next 2 weeks. I don't think that I can make it past today and the 2 hours I have done already. I am so happy to have found this group and know that I am not alone!!!

Excellent video. I am going to explore other options in your video that will help with manual massage and flexion. I have to agree that there was a lot of guarding on my part in the stretch knee fix prone. I can do this on my own and control my pain level.

Thank you! I did not have an MUA possibly due to the fact that I fell an cracked a small bone that was not protected by the knee replacement but was right beside it. So, my replacement was on 9/7/21 and my knee is still swollen and painful. Flexion is very painful. Its so good to hear that it most probably was not my fault that I ended up this way.

Thank you. This is me also almost exactly. No ROM and difficulty with standing after sitting in a chair after working for a period of time. Pain around my knee and behind my knee. I had TKR 10/21/20. 90ROM. MUA 1/29/21. 125ROM under anesthesia. Home and barely 90ROM. 7 months post op. Just had MRI on knee showed no tears in tendons or muscles, back of knee looked good, Posterior capsule intact. Patella tendon swollen and inflamed. Some atrophy in my thigh muscle, nothing wrong with implant. No infection and no fluid in joint. Xray of hip and knee. Looking for other issues like arthritis in hip causing me to not be able to stand. Do have little arthritis in hip but not enough to cause my problems. Knee xray showed no signs of loosening. I also quit PT for 4 weeks as I had extensive pain behind my knee and bruising after exercise. It was not worth it having my knee foced to bend. So I remain frustrated because I know what I dont have wrong. I just dont know what I do have wrong. Live in a 3 story with a non op knee that needs a TKR, 5 years bone on bone and its telling me its not going to help me anymore. I am exhausted, have high blood pressure from pain, lower back hurts from walking wonky. Using a cane. I appreciate your response to her question. It helps me understand and not beat myself up so much. I to have a new exercise program for home to increase flexion but not as intense and ride my stationary bike 2x a day. They are looking for slow steady progress. I just watched your ROM video on measuring for improvement which also wont be so stressful for me now. Thank you and good luck everyone. This has been a long long road.

I had a total Knee replacement at the end of January, manipulation in February because I was at 120 and should have been at 130 ... After manipulation, I was at 124 and after therapy, and daily home work, I reached 142. In May, I went back on the tennis courts ( I was playing tennis before) but I have problems running or taking off. I continue my exercise 5 to 7 times a week for anywhere between 1 hour and 2 hours, including knee flexion twice a day but I don't think that I am progressing in my knee flexion. Your presentation helped me understand and realize that I am doing OK. Thank you.

I'm 83. My TKR was May 11. On May 26 my range was 120. I was walking well w/o walker or cane when I returned to my home (was at my daughter's from day of surgery to May 27). The surgeon, 2 physical therapist at daughter's and one at my home on May 29 all said I didn't need p/t. I took it upon myself to make a p/t appointment for evaluation today, June 21. My range is still little over 120 and I passed all tests except one done by p/t today. I was on my side with leg up and flexed a little. I was -4. I was given exercises to do at home and return in a week. Tony, I'm glad you explained sensations of cramps in the quad. I had that yesterday. I am walking with my knee slightly bent. I noticed last week when I got on the floor (using your video to get down and up safely) that my back of knee did not touch the floor when I did the exercises to press knee down. On the bed where I did the exercises, it felt like the knee went flat. So today p/t walked around with me while I made sure I touched heel to floor and straighten knee. Hooray. I walked like a human being.
I get a lot of help from your videos and look forward to walking better soon. Unfortunately my 8 degerated discs in the low back and arthritis in my hip contribute to high level pain which also affects walking and standing. But for the first time in over 10 years I can sleep on my right side with no knee pain.

    • @bettecarlson3660
      @bettecarlson3660 2 ปีที่แล้ว

Tony, yesterday I went back to p/t and my range was 29!!! I get my back injections tomorrow so I hope that will help me walk better. When I step down it feels like a sword goes into my hip joint. That affects my balance. I plan to go to our small gym in my mobile complex and walk on the treadmill to work on my walking balance. Its amazing my thighs etc are very strong considering I'm a couch potato. I just can't stand or walk without severe back pain and spasms that stops me in my tracks. Then I have to ice etc. I do back exercises now my knee is 2 months old. I've asked my doctor to refer me for p/t for my back. The referral for my back didn't go through. With 8 collapsed discs and mild scoliosis its really difficult even to sleep.

Second knee was 93 degrees the day after surgery and day 5 after therapy it blewup.Had a MOA that helped quite a bit. Trying to get more range now that all my spine surgeries are done. Tony thank you for taking your precious time from your day to record these videos. I am just 12 days out of R TKR and I do believe God guided me to your channel. I had 4 PT sessions in home & one out. I was given exercises to do at home like everyone! I know you will tell me to consult with my doc but what is your opinion, please. If I'm in tears doing a third set of an exercise, should I back off? I was instructed that my knee needs to be at 90° by 2 weeks post-op. I don't think that will happen!
I have surfed a few videos and just knowing others are going through the same journey really helps. I briefly saw your pup photobomb too! 🐶 I think you said his name is Tucker. Is that correct?

It’s a brain game for me, and I can’t do exercises until it’s desensitized, longer duration with lower intensity, with a flexed hip-kneeling helps. Deep squats on the Squatty-Potty stool:). I have the JAS device - 30 mins/x2 daily. Rub with a dry rag or ice it for 10mins during JAS!! Thanks a million!!

Left TKR 8/23/21 I could straighten my leg completely from 8/26/21 to about 9/11/21
and bend my knee to 100. I started having violent cramps (some during the day and every night every couple of hours). One of these caused me to fall, I swelled up and had a lot of pain.
The cramps kept my knee swollen. MUA on 10/25/21 I think dr said he achieved 5/105. Cramps got worse & woke me every hour in extreme pain after MUA. Now I’m stuck at 30/90.
FYI - I had left THR in 2000. left hernia repair 1994. Meshoma - infected hernia mesh remove 2/2011. So neurologist & I think the cramps are due to so many surgeries in the same area.
I am doing PT 3 days a week & just started at a therapy pool.

I wish I knew to watch you before my TKR L knee almost 8weeks.
You speak with calm & clarity
My R leg is 1" + shorter than my L leg.
I had my 6 week check up with doctor of the orthopaedic team last week
I cannot get my L knee flat or walk probably (Without realising) I suggested to the doctor of the team I may have walked this way before my op? He also queried why I walk bent.
I'm under a well known Spinal team .Had 2 lower back ops:
    2012
    2013
Also a Spinal cord stimulater implanted implanted: 2018.
The doctor is referring me backnto the surgeon
I felt so deflated & confused as to why I am walking bent & why hasn't the Spinal team done anything to help me walk upright?
Re my knee replacement: I'm always active & have been doing the exercises as demonstrated. My knee bend is quite good & physio suggested I may have got to my peak. It is at the top spec. My walking without straight knee is really concerning me
    Positive note.
While in hospital the physio team have got me an appointment with Orthotic department to have an shoe implant sorted. No one has ever bothered before
I think my shorter leg isn't helping my Left knee re walking?
Thank you again I'm seeking to join your face book page.

Tony, this os Nancy Santiago from Puerto Rico. I had a total TKR back in July 27, 2021. I was on rehab center for 10 days. The pain was horrible. Still had the therapy, passive motion machine, waking, strength exercises, the works. I went home with “at home therapies” 3 times a week. I tried exercise myself. My knee stiffed completely. Went through a revision surgery on October 26,2021. Back to rehab therapy another 10 days and back home with therapy 3 times a week again.
I am regaining ROM but the pain when my knee is forced is very hard to take. I still take it. I am having horrible deppression and anxiety after 6 months of these nightmare. You just said there is nothing else you guys can do about after that pain ensues. Then MUA is recommended? I am freaking out about any more procedures, specially after knowing it may not work. I just turn 60 before my revision surgery. I was in good shape, no overweight. The tightness never goes away. Even 4 days after my revision surgery I was feeling the same. Please help!

After 2 MUAs and most recently an open arthrolysis I am still struggling to get my active flexion to 90. Passively, it's only 90. It was better than this 2 years ago pre-TKR. I am one of those who did everything advised. If I could achieve 105, I would be so happy! I can relate to all those struggling. I have not yet found anyone in as bad a situation as I am with knee flexion after TKR. I am a keen walker and cyclist but fear I will never ride a bike again!

Thank you for this video Tony. I had TKR on Jan. 6, 2022. Stiff from the beginning. At 6 weeks, I was only at 90 degrees. I had a MUA on Feb. 25, 2022. Two weeks after manipulation, I reached 116. I continued daily physical therapy for another month and only increased to 120 degrees with extreme stretching by the physical therapist in a month's time. Just went to the doctor on April 8, 2022 and saw his PA. He just told me to keep going to physical therapy for another 4-6 weeks. My extension is 0 but I want to be able to ride a bike again, hike and walk stairs normally. I am off the pain meds and am just taking extra strength Tylenol which doesn't kick the pain and makes it more difficult to stretch. Feel discouraged and am wondering what else I can do. I'm 3 months post op, and 6 weeks post MUA. Are there other things I should do? Can I still make progress?

Hi Tony, I found your site a few weeks ago and find it the most useful. I fractured my kneecap over five months ago but still cannot get past about 75 degrees on a good day. Are your exercises the same for a fracture as a replacement? Me fracture was a simple fracture with no displacement, but I feel the brace which was put on too tight and very close to the bottom of my kneecap could have caused some of my problems. I am 73 but up to my fall was fairly fit and walked 7 to 10 miles nearly every day. Now I can bear
walk around the house without my knee swelling and becoming quite painful. I still take at least 6 paracetamols a day. Have you any ideas you could share on your site.

Going to PT every afternoon and doing home exercises every morning, only at 90 degrees flexion/2 degrees extension as of 3/13 limited by a great deal of pain on the inside of my knee. Dr. is disappointed in my lack of progress and is referring to pain mgmt. for nerve ablation. Is that the next best step or is there something else I should consider?

I'm sitting in a different sofa right now to receive a big dose of bright sunshine! I lifted my leg up to the coffee table (a no no in my youth😃) with a "scream" from my knee cap. I had to start singing😂😂 as my puppies were getting upset. I bet your pup is protective of you☺too! In 24 hours, I'll be 14 days post-op! I must give over my "fear" of not bending & straightening my knee over to Jesus everyday.

Thank you so much for sharing these helpful videos. I had a TKR on August 30 and I have been having 3 days a week oof therapy since September 7th. I only got to 92 degrees flexion after six weeks, so I had an MUA last Tuesday and my doctor said he got125 degrees of flexion under anesthesia. I went back to therapy right away every day after that but can only get to 102 degrees of flexion. My knee is a bit swollen and stiff. I do very well in therapy sessions, but I still cannot get more knee flexion. I have had 6 weeks of therapy already and it's costly. Any advice will be appreciated.

I still have pain, stiffness and swelling post MUA 3- weeks. A lot of bruising. OS showed me photo of my knee at full flexion under anesthesia
In PT, I'm screaming for them to stop bending my knee!
Taking pain meds, lots of ice, lidocaine.
My OS recommended a Dynaspint to use at home to gently flex my knee everyday for 1- hr/2x day.
    Just got mine today.

After 15 days, my left knee bends to 112 degrees and my right knee bends to 103. Why some of the physical therapy has been very difficult I just do it, knowing that I’ll improve and get better.

I had total knee replacement surgery Aug 23, 2022. At approximately 4+ weeks, the physical therapist measured me at 110 deg. after my session. I continued with my home PT, and my next in office PT session, another therapist introduced some different exercises - standing and sliding each leg forward, back, and out to the side. Only mild discomfort during the exercise. The next morning, I woke up, my knee was extremely painful (before only very mild discomfort) and I lost a lot of range of motion. I could not get my knee to bend 90 degrees. I focused on the stretching PT exercises that day. The following day, I had in office PT, and told the therapist what had happened. He focused more on stretching exercises. At the end I had 90 deg. I am now at 8 weeks post surgery. Range of motion is 102 deg. I feel my progress is moving forward, but at a snail's pace. Should I be concerned at this point? I walk about 8,000-9,0000 steps/day (this includes normal activity, not a deliberate attempt to get that many steps. I am actively moving every hour (at least 250 steps). I am able to ride my exercise bike if I keep the heel of my surgery leg on the pedal. Then I can get full revolutions. My surgeon thinks I am doing fine. I am worried about scar tissue building faster than I can gain range of motion. Thank you for your very informative videos. Wish I had found these much sooner post surgery. Pre-surgery, I was a bicycle commuter, so I was in excellent shape prior to my replacement.

Plan is to Go to gym 3 days a week to ride bike. Only have been once and trying to make a full rotation. Hot springs 2x a week. Walking every day along with various exercises. Doc told me my brain is stopping the bend because of the pain. Well how do I get past the pain? Do I need to go to therapist for hypnosis? I bought a tens unit and will learn tomorrow how to use it. I decided to revisit my surgeon on my 2 year TKR anniversary date just for a "wellness check". The surgeon took X-rays and said everything looks perfect. He measured my flexibility and was SHOCKED 😮 I can bend up to 120° which is only 5° short of how far I could bend before surgery. He was so shocked that he thought he had his measuring tool upside down. LOL. He says he has NEVER had a patient regain that much bend after TKR surgery & MUA! He asked me what I was doing to gain and maintain such an incredible bending range! 😁(pilates, treadmill, elliptical, recumbent bike, walking....)

I do not want to go through this procedure for fear I will go back to what I experienced after surgery. I took oxycodone for 3 weeks and became dependent on it and didn't like how I felt. I couldn't sleep or eat. I stopped cold turkey and it took well over a month to recover. I was weak and didn't feel well at all. I did PT during this time but not successful. I just started Aqua therapy. Not sure if this will help but im stretching and doing bending with bands and holding for 3 to 5 minutes during commercials while watching tv. I do this several times a day and during the night when i get stiff which is maybe once or twice during the night. My question to you... can I accomplish gaining ROM over time without MUA? @suaverp2170 ปีที่แล้ว +2

I have a few opinions on differences in ROM. I think pain tolerance plays a huge role, as much as how bad was the structure of that knee prior to TKR. Did the patient let their knee go longer than it should have vs getting it done sooner? Are they taking their pain meds as instructed, or are they trying to ween themselves off too soon? Way too many times the pain is tolerable when sitting or light walking, so they feel they dont require the pain meds as much, but forget to realize those meds help with reducing swelling and to tolerate knee flexion. How many reps and frequency? I've seen some therapist give 2 x 10 at 3 times per day. IMHO, that is too much stress on the knee causing increased inflammation. I feel the perfect amount is 1 x 10 at 3 times per day, followed by ice application and elevtion immediately to reduce swelling. Are they performing heelslides with belt assist in supine or sitting? Sitting disrupts knee flexion as our stomachs can get in the way, as well as sitting puts you in a poor angle for pulling. There are so many reasons for the results some people have. Early education and consistency is the key. We have too many therapist who approach this differently, and can cause setbacks.

I think you are on the money! I am 58 and had a TKR 6 weeks ago. I saw a lot of older folk in Hospital that put off the inevitable of replacing knees and hips and really struggled post operative. I have experienced several physiotherapists from my 2 week stay in hospital/rehab and my local physio and they are all different in amount of exercise you should do. To much is definitely counter-productive as I have experienced this myself firsthand. It is a balancing act to know how much medication to take prior to doing the exercise to assist in pain management and then applying preventative swelling techniques e.g. elevation and ice packs. I have been told to try and wean myself off pain medication which I have but there are days where you definitely just need to take it. There is no need to be in pain for the sake of it.

Very good points Tony! Kathy's degree of flexion after the MUA sounds like mine. I hit 130 degrees in the OR at my two MUA's but only got in the low 90's after the first MUA and the low 100's after the second MUA. Also, I would not allow a vulnerable position for my pushes during my therapy. I'd be too guarded if I was placed on my stomach and have the therapist push like that. So, I agree that a trust must be there between the patient and the therapist and that the patient cannot be put in a vulnerable position if we hope for a good session.

I was wondering when you said you had frozen knee did you mean like frozen shoulder? Because frozen shoulder is directly related to sugar in the blood. If you have excess sugar in the blood, it mixes with the collagen in the weakest joint and turns into a thick, stiff, crunchy mess. I like it too thick, raw honey, that is already crystallized. When I had my surgery, I made sure I did not do any sugar whatsoever. I’ve had frozen shoulder and I can’t imagine having it in my knee. I just wanted to put it out there for all of you that are experienced. What seems like frozen me

That is really great. 90 decrees by 4 weeks is good. You are doing great.

Is getting flexion back is pose to be fast or it’s a slow process

    • @totaltherapysolutions
      @totaltherapysolutions  3 ปีที่แล้ว +2

It is a slow process. I think extension typically comes faster.

I’ve been crying a lot. I have the problem of bursitis in my hip, the nerve thing running down the right side of my leg (my surgery), blood clots in my left side of that leg. I was doing great I was to 98 degrees , my leg swelled up so tight in the back of my leg, now I can’t bend my knee . It feels like a knife being run up under that “knee cap” . I’m doing great with most the my exercises except certain ones with deep bends with my knee.

Is the blood clot causing the swelling? Unfortunately the blood clot takes priority. Have you tried elevating the leg above the heart? I have some videos on this channel showing you how to get the leg up on the back of a couch or on a stack of pillows. If you get reduce the swelling without compromising the blood clot you might improve the pain and ROM.

@@totaltherapysolutions I put my leg up but it ends causing a lot of pain due to bursitis in my hip and top of my leg. I can’t get the vascular doctor to see me. The orthopedic doctor won’t give me a steroid shot in that area. I’m So scared of having that forced thing. I was at 98 and my therapist thought I was progressing so well then I started regressing! I’ve gotten worse. Yes I feel like I’ve done something wrong. Then again when the swelling got real bad that weekend then now I can’t bend my knee. It’s like my new knee cap isn’t wanting to work. I’m so frustrated because now I’m waiting to see if a pain clinic will take my case and give a shot in the bursa area.

Had a M U A 2 days ago and in therapy the next day. The pain from the M U A is like “Why did I have that TKR” The pain medication was not as strong as I expected and with the therapy sessions are painful but I bear it and know it’s going to better. I wish I had a piece of wood so I can chomp on it like in the movies when they removed bullets from someone !!! 😮

Yes, rocking it good, I did it sitting on a wooden carved box slightly higher than my comfortable sitting height, got my flexion back in both my knees and following my Posterior Cruciate Ligament. Even placing the other foot/ankle over your operated knee, forcing the lower leg back. I hope that makes sense.

Lyn R so do you just rock? Did you have a rocking chair?

Oh my gosh. I was doing that also. Wasn't sure it was a good thing, but it felt great. I also propped my feet on a foot stool while rocking or sitting on porch swing. I just kept going back a little further each time, nothing drastic
