Hi mam , I have a doubt suppose a patient's ar value is -2.5 sph -1.75 cyl 90 , in subjective refraction after giving -2.5 sph a patient is able to read 6/12 , can I start with + 1.00 cyl at 180 instead of + .5 cyl 180 above u mention ur video . In case a patient is able to read further line , when i give +1.0 cyl whether can I give -1.50 -1.00 @90 or - 2.0 -1.00 @90

If the patient is comfortable then you can prescribe it; however I would try the transposed (minus cylinder) form in the right eye - so that would be +0.75 / -0.75 x 20. Try this both uniocularly and binoculary and see which the patient is more comfortable with. The idea of transposing is so that both eyes would be in the minus cylinder format

There is a mistake in the numbers mentioned on the power cross. I have included a caption at the bottom that says that the +2.50 should read as -2.50

In this case, I would say always start with 0.5. you can start work 1D if the cylinder is say 3D or so
